Abstract

A dilation introducer for orthopedic surgery is provided for minimally invasive access for insertion of an intervertebral implant. The dilation introducer may be used to provide an access position through Kambin's triangle from a posterolateral approach. A first dilator tube with a first longitudinal axis is provided. A second dilator tube may be introduced over the first, advanced along a second longitudinal axis parallel to but offset from the first. A third dilator tube may be introduced over the second, advanced along a third longitudinal axis parallel to but offset from both the first and the second. An access cannula may be introduced over the third dilator tube. With the first, second, and third dilator tubes removed, surgical instruments may pass through the access cannula to operate on an intervertebral disc and/or insert an intervertebral implant.

Claims (23)

1. A method of inserting an intervertebral implant into an intervertebral space, the method comprising:

defining a trajectory through a Kambin's triangle of the patient and towards an intervertebral disc by an instrument extending through the Kambin's triangle;

removing at least a portion of the intervertebral disc so as to create the intervertebral space;

advancing the intervertebral implant in an unexpanded state through the instrument through the Kambin's triangle generally along said trajectory into the intervertebral space, wherein the intervertebral implant includes upper and lower body portions that each define a respective enclosed aperture extending therethrough along a direction, and the upper and lower body portions are spaced from each other along the direction so as to define a first height when the intervertebral implant is in the unexpanded state; and

after the advancing step, expanding the intervertebral implant from the unexpanded state to an expanded state whereby the top and bottom surfaces are spaced from each other so as to define a second height that is greater than the first height.

The method of claim 1 , wherein the implant defines a leading end and a trailing end opposite the leading end, wherein the upper and lower body portions extend continuously from the leading end to the trailing end, and the advancing step comprises advancing the intervertebral implant in a direction from the trailing end to the leading end.

The method of claim 1 , further comprising the step of engaging an upper vertebral body with a top surface of the upper body portion, and engaging a lower vertebral body with a bottom surface of the lower body portion, wherein the upper and lower vertebral bodies combine to define the intervertebral space.

4. The method of claim 3 , wherein the top and bottom surfaces are toothed, and the engaging step causes the toothed top surface to grip the upper vertebral body, and further causes the toothed bottom surface to grip the lower vertebral body.

5. The method of claim 3 , further comprising the step of facilitating osseointegration of the implant to the upper and lower vertebral bodies through the apertures that extend through the upper and lower body portions.

6. The method of claim 5 , comprising introducing bone graft into the intervertebral space.

7. The method of claim 5 , wherein the enlarging step is performed prior to the advancing step.

8. The method of claim 1 , further comprising the step of enlarging the Kambin's triangle about said trajectory by the instrument to define a pathway positioned about said about said trajectory.

9. The method of claim 8 , wherein the enlarging step is performed prior to the advancing step.

10. A method of inserting an intervertebral implant into an intervertebral space, the method comprising:

defining a trajectory through a Kambin's triangle of the patient and towards an intervertebral disc by an instrument extending through the Kambin's triangle;

advancing the intervertebral implant in an unexpanded state through the instrument through the Kambin's triangle generally along said trajectory into the intervertebral space without enlarging the Kambin's triangle, wherein the intervertebral implant includes upper and lower body portions that each define a respective enclosed aperture extending therethrough along a direction, and the upper and lower body portions are spaced from each other along the direction so as to define a first height when the intervertebral implant is in the unexpanded state; and

after the advancing step, expanding the intervertebral implant from the unexpanded state to an expanded state whereby the top and bottom surfaces are spaced from each other so as to define a second height that is greater than the first height.

11. The method of claim 10 , further comprising the step of removing at least a portion of the intervertebral disc so as to create the intervertebral space.

The method of claim 10 , wherein the implant defines a leading end and a trailing end opposite the leading end, wherein the upper and lower body portions extend continuously from the leading end to the trailing end, and the advancing step comprises advancing the intervertebral implant in a direction from the trailing end to the leading end.

The method of claim 10 , further comprising the step of engaging an upper vertebral body with a top surface of the upper body portion, and engaging a lower vertebral body with a bottom surface of the lower body portion, wherein the upper and lower vertebral bodies combine to define the intervertebral space.

14. The method of claim 13 , wherein the top and bottom surfaces are toothed, and the engaging step causes the toothed top surface to grip the upper vertebral body, and further causes the toothed bottom surface to grip the lower vertebral body.

15. The method of claim 13 , further comprising the step of facilitating osseointegration of the implant to the upper and lower vertebral bodies through apertures that extend through the upper and lower body portions.

16. The method of claim 10 , further comprising the step of enlarging the Kambin's triangle about said trajectory by the instrument to define a pathway positioned about said about said trajectory.
