Abstract

Some embodiments of a medical device anchor system includes an anchor sleeve and a catheter (or other medical instrument) to advance though a working channel of the anchor sleeve. The anchor sleeve may have a subcutaneous cuff device arranged along an outer surface. In such circumstances, the embedded cuff device can inhibit the migration of infection from outside the skin and into the blood stream.

Claims (30)

1. A catheter anchor device for securing a position of a catheter relative to a percutaneous opening in a skin layer occupied by the catheter, comprising:

a proximal end, a distal end, and a length extending therebetween;

a catheter engagement body configured to releasably secure with a catheter;

a subcutaneous anchor mechanism having first and second flexible anchors that are adjustable to a deployed configuration in which the first and second flexible anchors are configured to abut an underside of the skin layer in a subcutaneous region;

a subcutaneous cuff device coupled to the catheter engagement body at a location along the catheter engagement body that is distal of the first and second flexible anchors, the subcutaneous cuff device configured to receive ingrowth of bodily tissue when disposed in the subcutaneous region, wherein the subcutaneous cuff device is positioned closer to the distal end of the catheter anchor device than to the proximal end of the catheter anchor device and distal of the first and second flexible anchors; and

an actuator adjustable relative to the subcutaneous cuff device between a first position and a second position to shift the first and second flexible anchors to the deployed configuration and to simultaneously shift a gripper portion to compress an outer surface of the catheter, the first and second flexible anchors retracted within the catheter engagement body when the actuator is in the first position, the first and second flexible anchors located closer to the actuator and the proximal end than to the subcutaneous cuff device when in the deployed configuration.

2. The device of claim 1 , wherein when the catheter is moved through the percutaneous opening in the skin layer, a cuff-to-tip distance between the subcutaneous cuff device and a distal tip of the catheter is manually selectable.

3. The device of claim 1 , wherein a distance between the actuator and the first and second flexible anchors when in the deployed configuration is less than a distance between the subcutaneous cuff device and distal end.

4. The device of claim 1 , wherein the catheter engagement body has a tapered region that tapers towards the distal end, and the subcutaneous cuff device is located on the tapered region.

The device of claim 1 , wherein the subcutaneous cuff device is coupled to the catheter engagement body at a position such that the subcutaneous cuff device is configured to reside in the subcutaneous region along the underside of the skin layer while at least a proximal portion of the catheter engagement body resides external to the skin layer.

6. The device of claim 1 , wherein the catheter engagement body comprises a sleeve body.

7. The device of claim 6 , wherein the sleeve body comprises a lumen configured to receive the catheter, and wherein the actuator adjustable relative to the subcutaneous cuff device between a first position and a second position by sliding over a portion of the catheter engagement body while the catheter is within the lumen.

8. The device of claim 1 , wherein the subcutaneous cuff device inhibits migration of infection when the subcutaneous cuff device is embedded in the subcutaneous region along the underside of the skin layer.

9. The device of claim 1 , wherein the catheter engagement body includes a gripper portion that is adjustable to frictionally engage at least a portion of an outer circumferential surface of the catheter.

The device of claim 1 , wherein the catheter engagement body is configured to releasably secure to the catheter by shifting the gripper portion to releasably compress against an outer circumferential surface of the catheter after each of the catheter and the first and second flexible anchors is inserted through the percutaneous opening in the skin layer.

11. A catheter anchor device for securing a position of a catheter relative to a percutaneous opening in a skin layer occupied by the catheter, comprising:

a proximal end, a distal end, and a length extending therebetween;

a catheter engagement body configured to releasably secure with a catheter;

a subcutaneous anchor mechanism having first and second flexible anchors that are adjustable to a deployed configuration in which the first and second flexible anchors are configured to abut an underside of the skin layer in a subcutaneous region;

a subcutaneous cuff device coupled to the catheter engagement body at a location along the catheter engagement body that is distal of the first and second flexible anchors, the subcutaneous cuff device configured to receive ingrowth of bodily tissue when disposed in the subcutaneous region, wherein the subcutaneous cuff device is positioned closer to the distal end of the catheter anchor device than to the proximal end of the catheter anchor device and distal of the first and second flexible anchors; and

an actuator adjustable relative to the subcutaneous cuff device between a first position and a second by sliding longitudinally along a portion of the catheter engagement body between a first position and a second position to shift the first and second flexible anchors to the deployed configuration and to simultaneously shift a gripper portion to compress an outer surface of the catheter, the first and second flexible anchors retracted within the catheter engagement body when the actuator is in the first position, wherein the catheter engagement body includes anchor holes, the first and second flexible anchors extending through the anchor holes in the deployed configuration;

wherein when in the deployed configuration a distance between the actuator and the first and second flexible anchors is less than a distance between the subcutaneous cuff device and the distal end, the first and second flexible anchors are located closer to the proximal end than to the subcutaneous cuff device, and the first and second flexible anchors located closer to the actuator than to the subcutaneous cuff device.

12. The device of claim 11 , wherein when the catheter is moved through the percutaneous opening in the skin layer, a cuff-to-tip distance between the subcutaneous cuff device and a distal tip of the catheter is manually selectable.

13. The device of claim 12 , wherein the catheter engagement body has a tapered region that tapers towards the distal end, and the subcutaneous cuff device is located on the tapered region.

The device of claim 11 , wherein the subcutaneous cuff device is coupled to the catheter engagement body at a position such that the subcutaneous cuff device is configured to reside in the subcutaneous region along the underside of the skin layer while at least a proximal portion of the catheter engagement body resides external to the skin layer.

15. The device of claim 1 , wherein the first and second flexible anchors are in a stressed condition retracted within the catheter engagement body when the actuator is in the first position.

16. The device of claim 7 , wherein the catheter engagement body includes anchor holes, the first and second flexible anchors extending through the anchor holes in the deployed configuration.

17. The device of claim 15 , wherein the first and second flexible anchors are not integral with the actuator.

18. The device of claim 5 , wherein the actuator is coupled with the proximal portion of the catheter engagement body at a position such that the actuator is configured to reside external to the skin layer while the first and second flexible anchors abut an underside of the skin layer in the deployed configuration.

19. The device of claim 15 , wherein the first and second flexible anchors have a convex curvature that abuts against the underside of the skin layer.
