Abstract

The present disclosure relates to methods of characterizing disease. In particular, this invention relates to methods for selecting a treatment, treating, and predicting survival time in subjects suffering from sepsis, based on the characterization of genes associated with a reactive oxygen species (ROS) molecular signature.

Claims (72)

1. A method of determining a risk of death from sepsis by detecting altered gene expression in a subject with sepsis, comprising:

a) obtaining a sample from said subject wherein the sample is selected from the group consisting of whole blood, plasma, serum, and urine; and

b) detecting the level of expression of CSDE1, DNAJC8, DNAJB9, PRDX5, GCLM, DNAJA3, CCT7, GSTP1, FTH1, NCF1, CCT8, CYBA, DNAJB11, PRDX2, GSR, HSPA1A, DNAJC5, SOD2, PRDX3, DNAJB6, and KEAP1 by performing polymerase chain reaction on the sample and detecting amplified nucleic acids in the sample; and

c) comparing said level of gene expression in said subject with sepsis to the level in a subject without sepsis;

d) calculating a risk score as a linear combination of expression values of said genes, wherein a positive number score is indicative of said subject being at risk of death from sepsis;

e) treating the subject for sepsis;

f) obtaining a biological sample from the subject after treatment for sepsis;

g) determining gene expression levels of CSDE1, DNAJC8, DNAJB9, PRDX5, GCLM, DNAJA3, CCT7, GSTP1, FTH1, NCF1, CCT8, CYBA, DNAJB11, PRDX2, GSR, HSPA1A, DNAJC5, SOD2, PRDX3, DNAJB6, and KEAP1 in the biological sample obtained after treatment for sepsis by performing polymerase chain reaction on the sample and detecting amplified nucleic acids in the sample;

h) calculating a risk score as a linear combination of expression values of said genes in the subject after treatment for sepsis; and

i) determining the treatment as a favorable treatment if the risk score decreases after treatment.

2. The method of claim 1 , wherein each risk score is calculated using the formula:

where S is the risk score of the patient; n is 21 and represents the number of genes in claim 1 ; W i denotes the weight of gene i as shown in Table 2; e i denotes the expression level of gene i; and μ i and τ i are the mean and standard deviation of the gene expression values for gene i across all samples, respectively.

3. A method of monitoring the treatment of sepsis in a human subject, the method comprising:

obtaining a biological sample from a subject being treated for sepsis;

determining gene expression levels of CSDE1, DNAJC8, DNAJB9, PRDX5, GCLM, DNAJA3, CCT7, GSTP1, FTH1, NCF1, CCT8, CYBA, DNAJB11, PRDX2, GSR, HSPA1A, DNAJC5, SOD2, PRDX3, DNAJB6, and KEAP1 in the biological sample by performing polymerase chain reaction on the sample and detecting amplified nucleic acids in the sample;

comparing the gene expression levels to reference gene expression levels, wherein the reference gene expression levels are gene expression levels of CSDE1, DNAJC8, DNAJB9, PRDX5, GCLM, DNAJA3, CCT7, GSTP1, FTH1, NCF1, CCT8, CYBA, DNAJB11, PRDX2, GSR, HSPA1A, DNAJC5, SOD2, PRDX3, DNAJB6, and KEAP1 for one or more of subjects not having sepsis and subjects having non-life threatening sepsis; and

calculating a risk score as a linear combination of expression values of said genes in the subject being treated for sepsis and identifying the treatment as a favorable treatment response if the risk score of the subject being treated for sepsis is about the same as a risk score calculated from one or more of subjects not having sepsis and subjects having non-life threatening sepsis;

treating the subject for sepsis;

obtaining a biological sample from the subject after treatment for sepsis;

determining gene expression levels of CSDE1, DNAJC8, DNAJB9, PRDX5, GCLM, DNAJA3, CCT7, GSTP1, FTH1, NCF1, CCT8, CYBA, DNAJB11, PRDX2, GSR, HSPA1A, DNAJC5, SOD2, PRDX3, DNAJB6, and KEAP1 in the biological sample obtained after treatment for sepsis by performing polymerase chain reaction on the sample and detecting amplified nucleic acids in the sample;

calculating a risk score as a linear combination of expression values of said genes in the subject after treatment for sepsis; and

determining the treatment as a favorable treatment if the risk score decreases after treatment.

4. The method of claim 3 , wherein the biological sample comprises peripheral blood mononuclear cells.

5. The method of claim 3 , further comprising communicating the effectiveness of the treatment to the subject or a health care provider.

6. The method of claim 3 , wherein each risk score is calculated using the formula:
