Abstract

A method of performing a transperineal biopsy procedure including sliding a displacement member of a biopsy guide relative to a guide member of the biopsy guide such that the displacement member is proximate the perineal access site of the patient. The guide member may be secured to the ultrasound probe, and the guide member may include a pair of guide rails each having a planar top surface. The displacement member may include a platform having a planar bottom surface and at least one needle receiving port for guiding a needle, wherein the planar bottom surface of the platform opposes the planar top surfaces of the pair of guide rails during the sliding of the displacement member relative to the guide member. The method may further include inserting a needle into the perineal access site of the patient.

Claims (41)

1. A method of performing a transperineal biopsy procedure on a prostate of a patient using an ultrasound probe configured to provide imaging in at least a sagittal imaging plane, the patient comprising a perineal access site leading to the prostate, the method comprising:

contacting a distal end of a pair of guide rails of a guide member of a biopsy guide with the perineal access site of the patient, the guide member secured to the ultrasound probe, the pair of guide rails being spaced apart from each other and positioned on opposite sides of the sagittal imaging plane;

inserting an access needle into the perineal access site of the patient by sliding a displacement member of the biopsy guide relative to the guide member of the biopsy guide such that the displacement member is proximate the perineal access site of the patient, the displacement member comprising a platform and at least one needle receiving port, the access needle positioned within the sagittal imaging plane and positioned in one of the at least one needle receiving ports of the displacement member; and

inserting a needle through the access needle and into the prostate of the patient with the distal end of the pair of guide rails in contact with the perineal access site of the patient.

2. The method of claim 1 , wherein a length of the displacement member is shorter than a length of the pair of guide rails of the guide member.

3. The method of claim 1 , further comprising injecting a substance into the patient via the needle.

4. The method of claim 1 , further comprising taking a biopsy of the patient via the needle.

5. The method of claim 1 , wherein a portion of the pair of guide rails overhangs the ultrasound probe.

6. The method of claim 5 , wherein the portion of the pair of guide rails is a distal portion of the guide rails that extends beyond a distal edge of a fastener, the fastener secured to the ultrasound probe.

7. The method of claim 5 , wherein the portion of the pair of guide rails is a proximal portion of the guide rails that extends beyond a proximal edge of a fastener, the fastener secured to the ultrasound probe.

8. A method of performing a transperineal biopsy procedure on a patient having a perineum and a prostate, the transperineal biopsy procedure utilizing a biopsy needle, an access needle having a lumen, and a transrectal probe for imaging the prostate, the method comprising:

attaching a biopsy guide to the transrectal probe via a mount of the biopsy guide, the biopsy guide comprising a guide member and a displacement member, the guide member coupled to the mount and comprising a frame extending upwardly from the mount, the displacement member releasably coupled to the guide member and comprising a needle receiving port, wherein the frame comprises a pair of guides and a channel formed there between, the displacement member configured to slide along the pair of guides and within the channel;

inserting the access needle through the needle receiving port of the displacement member;

contacting a distal end of the guide member with the perineum of the patient;

inserting the access needle into the perineum of the patient with the distal end of the guide member in contact with the perineum of the patient; and

inserting the biopsy needle through the lumen of the access needle and into the prostate to obtain a first tissue sample.

9. The method of claim 8 , further comprising inserting the biopsy needle through the lumen of the access needle and into the prostate to obtain a second tissue sample.

10. The method of claim 9 , wherein the access needle remains in the perineum of the patient while obtaining the first and second tissue samples.

11. The method of claim 8 , further comprising securing a position of the displacement member along a length of the guide member.

12. The method of claim 11 , wherein the position is a distal-proximal position.

13. The method of claim 8 , wherein the access needle is positionable at a plurality of heights relative to the guide member.

14. The method of claim 8 , wherein the displacement member is constrained to slide on the frame of the guide member along an axis.

15. The method of claim 14 , wherein the axis is parallel to a longitudinal axis of the transrectal probe.

16. The method of claim 14 , wherein rotation of the transrectal probe causes rotation of the access needle when: the biopsy guide is attached to the transrectal probe;

the displacement member is releasably coupled to the guide member; and the access needle is through the needle receiving port of the displacement member.

17. A method of performing a transperineal biopsy procedure on a patient having a perineum and a prostate, the transperineal biopsy procedure utilizing a biopsy needle, an access needle having a lumen, and a transrectal probe for imaging the prostate, the method comprising:

attaching a biopsy guide to the transrectal probe via a mount of the biopsy guide, the biopsy guide comprising a guide member and a displacement member, the guide member coupled to the mount and comprising a frame extending upwardly from the mount, the frame comprising a pair of guides and a channel formed there between, the displacement member configured to slide along the pair of guides and within the channel, wherein the displacement member is constrained to slide on the frame of the guide member along an axis;

contacting a distal end of the pair of guides with the perineum of the patient;

engaging the access needle with the displacement member at one of a plurality of positions above the transrectal probe;

inserting the access needle into the perineum of the patient using the contact between the distal end of the pair of guides and the perineum as a guide for insertion of the access needle; and

inserting the biopsy needle through the lumen of the access needle and into the prostate to obtain a first tissue sample.

18. The method of claim 17 , further comprising inserting the biopsy needle through the lumen of the access needle and into the prostate to obtain a second tissue sample.

19. The method of claim 18 , wherein the access needle remains in the perineum of the patient while obtaining the first and second tissue samples.

20. The method of claim 17 , wherein rotation of the transrectal probe causes rotation of the access needle when: the biopsy guide is attached to the transrectal probe; the displacement member is releasably coupled to the guide member; and the access needle is engaged with the displacement member.

21. The method of claim 17 , further comprising securing a position of the displacement member along a length of the guide member.

22. The method of claim 21 , wherein the position is a distal-proximal position.

23. The method of claim 17 , wherein the axis is parallel to a longitudinal axis of the transrectal probe.

24. The method of claim 17 , wherein each of the plurality of positions is parallel to a longitudinal axis of the transrectal probe.

25. The method of claim 17 , wherein engaging the access needle with the displacement member at one of a plurality of positions above the transrectal probe comprises inserting the access needle through one of a plurality of needle receiving ports of the displacement member.

The method of claim 1 , further comprising, with the access needle in the perineal access site and with the guide rails in contact with the perineal access site, adjusting a trajectory of the access needle, and inserting the needle again through the access needle and into another location of the prostate.

27. The method of claim 1 , wherein the pair of guide rails do not intersect the sagittal imaging plane when the pair of guide rails are positioned on opposite sides of the sagittal imaging plane.
