Abstract

The present invention is directed to systems and methods for thermal therapy, especially to detection-guided, -controlled, and temperature-modulated interstitial thermal therapy. Thermal therapy may be used to treat the tissues of a patient. In the case of interstitial thermal therapy, energy is applied to generate heating of the tissue to affect treatment, such as, for example, thermally inducing tissue damage (e.g. thermally-induced tissue necrosis), which may be useful in treating tumors and/or other diseased tissues. Since targets for thermal therapy are internal to the patient, the use of detection guidance may be useful in locating and monitoring treatment of a target tissue.

Claims (41)

1. A method of delivering energy to a target area, comprising:

locating a plurality of closed integrated apparatuses proximate to the target area at substantially predetermined locations with a guide having an extension extending from the guide;

providing a transfer fluid to a fluid input of each of said plurality of closed integrated apparatuses for closed circulation to an output of each of said plurality of integrated apparatuses therein to at least exchange heat with an energy delivery component of each of said plurality of closed integrated apparatuses and at least a portion of the target area;

providing energy to the at least a portion of the target area via energy delivery components located in said plurality of closed integrated apparatuses; and

detecting a position and orientation of the guide and the extension member with at least one first fiducial marker attached to the guide and at least one second fiducial marker attached to the extension;

wherein the locations of the plurality of closed integrated apparatuses, the locations of the energy delivery components therein, the energy and transfer fluid provided to said closed integrated apparatuses, are each independently controlled to substantially apply a predetermined thermal profile to at least a portion of said target area to affect a temperature-induced change in a portion of said target area.

2. The method of claim 1 , further comprising scattering or diffusing the energy delivered via the transfer fluid and at least one of said energy delivery components.

3. The method of claim 1 , wherein said transfer fluid is re-circulated into and out of each of said plurality of integrated apparatuses prior to and during said providing of energy.

4. The method of claim 1 , further comprising:

locating said plurality of integrated apparatuses through the guide comprising:

at least a configuration of a plurality of channels, each of said plurality of channels defining a trajectory; and

the at least one first fiducial marker detectable by a detection device; and

retaining said integrated apparatuses in said channels.

5. The method of claim 4 , further comprising orienting said guide relative to the target to substantially align the trajectory of at least one of said channels to at least a portion of said target area.

6. The method of claim 5 , wherein orienting the guide to the target further includes positioning the guide with the extension member coupled to the guide.

The method of claim 6 , further comprising detecting with the detection device the position and orientation of the guide and the extension member by detecting the at least one first fiducial marker attached to the guide and the at least one second fiducial marker attached to the extension and a change in temperature.

8. The method of claim 1 , further comprising independently controlling the location of each integrated apparatus and the energy delivery components therein prior to and during the delivery of energy.

9. The method of claim 1 , further comprising altering the temperature of at least a portion of said target area prior to delivering energy via said integrated apparatuses.

10. A method of delivering energy to a target area, comprising:

positioning a guide having a plurality of channels and at least one first fiducial marker relative to the target area;

positioning a plurality of closed treatment devices in at least a subplurality of the channels of the guide;

determining at least one energy delivery characteristic of each of the plurality of closed treatment devices;

delivering energy to the target from the plurality of the closed treatment devices using the energy delivery characteristics;

using a detector to detect a temperature change in the target area and a position and orientation of the guide; and

independently controlling each of the plurality of closed treatment devices, including a transfer fluid circulated through an input and output to each closed treatment device.

11. The method of claim 10 , further comprising positioning an extension member coupled to the guide to position the guide relative to the target.

12. The method of claim 11 , further comprising detecting the position and orientation of the guide and the extension member by detecting the at least one first fiducial marker attached to the guide and at least one second fiducial marker attached to the extension with the detection device.

13. The method of claim 10 , further comprising scattering or diffusing the energy delivered via the transfer fluid and at least one of the treatment devices.

14. The method of claim 10 , further comprising:

providing the transfer fluid to each input of each of the plurality of closed treatment devices for circulation out of each output of each of the plurality of the closed treatment devices therein to at least exchange heat with an energy delivery component in each of the plurality of treatment devices and at least a portion of the target area.

15. The method of claim 14 , wherein the transfer fluid is circulated prior to and during the delivering of energy to the target from the plurality of treatment devices.

16. A method of delivering energy to a target area, comprising:

positioning a guide having a plurality of channels and an extension member coupled to the guide relative to the target area;

positioning a plurality of closed treatment devices in at least a sub-plurality of the channels of the guide;

using a detector to detect a temperature change in the target area and a position and orientation of the guide and the extension member; and

independently controlling each of the plurality of closed treatment devices to independently deliver energy to the target from the plurality of closed treatment devices and independently circulate a transfer fluid into and out of each of the plurality of the closed treatment devices.

17. The method of claim 16 , further comprising determining at least one energy delivering characteristic of each of the plurality of treatment devices.

18. The method of claim 17 , wherein delivering energy to the target from the plurality of treatment devices includes using the energy delivery characteristics.

19. The method of claim 16 , further comprising altering a temperature of at least a portion of the target area prior to delivering energy via the plurality of treatment devices.

20. The method of claim 16 , further comprising:

detecting the position and orientation of the guide and the extension member by detecting at least a first fiducial marker attached to the guide and at least a second fiducial marker attached to the extension.
