Abstract

An implantable medical device is configured to control a therapy module to couple a capacitor array comprising a plurality of capacitors to a plurality of extra-cardiovascular electrodes and control the therapy module to deliver a composite pacing pulse to a patient's heart via the plurality of extra-cardiovascular electrodes by sequentially discharging at least a portion of the plurality capacitors to produce a series of at least two individual pulses that define the composite pacing pulse.

Claims (56)

1. A medical device comprising:

a therapy module having a capacitor array comprising a plurality of capacitors and an output signal line, the therapy module configured to generate a composite pacing pulse comprising a series of at least two individual pulses by:

generating a first pulse of the at least two individual pulses by selectively coupling a first portion of the plurality of capacitors to the output signal line, the selectively coupled first portion of the plurality of capacitors having a first effective capacitance and the first pulse of the at least two individual pulses having a first decay rate corresponding to the first effective capacitance, and

generating a second pulse of the at least two individual pulses by selectively coupling a second portion of the plurality of capacitors to the output signal line, the selectively coupled second portion of the plurality of capacitors having a second effective capacitance different than the first effective capacitance and the second pulse of the at least two individual pulses having a second decay rate different than the first decay rate, the second decay rate corresponding to the second effective capacitance.

2. The device of claim 1 , wherein the therapy module is further configured to:

generate the first pulse by coupling the first portion of the plurality of capacitors to the output signal line for a first pulse width;

generate the second pulse by coupling the second portion of the plurality of capacitors to the output signal line for a second pulse width, the second pulse width being different than the first pulse width.

3. The device of claim 2 , wherein the therapy module is further configured to:

set the first pulse width to terminate the first pulse decaying at the first decay rate at a first terminating voltage amplitude that is at least a threshold voltage amplitude; and

set the second pulse width to terminate the second pulse decaying at the second decay rate to a second terminating voltage amplitude that is at least the threshold voltage amplitude.

The device of claim 1 , wherein the therapy module is configured to generate a third pulse of the composite pacing pulse by selectively coupling a third portion of the plurality of capacitors to the output signal line, the third portion of the plurality of capacitors having a third effective capacitance.

5. The device of claim 4 , wherein the third effective capacitance is different than the second effective capacitance.

6. The device of claim 4 , wherein the therapy module is configured to generate the third pulse by coupling the third portion of the plurality of capacitors to the output signal line for the first pulse width.

7. The device of claim 4 , wherein the therapy module is configured to generate the composite pacing pulse by:

generating the first pulse by coupling the first portion of the plurality of capacitors to the output signal line for the first pulse width;

generating the third pulse of the composite pacing pulse consecutively following the first pulse by coupling the third portion of the plurality of capacitors to the output signal line for the first pulse width; and

generating the second pulse consecutively following the third pulse of the composite pacing pulse by coupling the second portion of the plurality of capacitors to the output signal line for the second pulse width.

8. The device of claim 7 , wherein the therapy module is configured to:

select the second portion of the plurality of capacitors to have the second effective capacitance that is greater than the first effective capacitance and greater than the third effective capacitance; and

set the second pulse width to be greater than the first pulse width.

9. The device of claim 1 , wherein the therapy module is configured to:

generate the first pulse having a first pulse energy that is less than a pacing capture threshold;

generate the second pulse having a second pulse energy that is less than the pacing capture threshold; and

generate the composite pacing pulse comprising at least the first pulse and the second pulse, the composite pacing pulse having a cumulative pulse energy comprising at least the first pulse energy and the second pulse energy that is at least the pacing capture threshold.

10. The device of claim 1 , further comprising:

switching circuitry configured to sequentially couple the first portion of the plurality of capacitors and the second portion of the plurality of capacitors to the output line.

11. A method comprising:

generating a composite pacing pulse comprising a series of at least two individual pulses by:

generating a first pulse of the at least two individual pulses by selectively coupling a first portion of a plurality of capacitors to an output signal line, the selectively coupled first portion of the plurality of capacitors having a first effective capacitance and the first pulse of the at least two individual pulses having a first decay rate corresponding to the first effective capacitance, and

generating a second pulse of the at least two individual pulses by selectively coupling a second portion of the plurality of capacitors to the output signal line, the selectively coupled second portion of the plurality of capacitors having a second effective capacitance different than the first effective capacitance and the second pulse of the at least two individual pulses having a second decay rate different than the first decay rate, the second decay rate corresponding to the second effective capacitance.

12. The method of claim 11 , wherein:

generating the first pulse comprises coupling the first portion of the plurality of capacitors to the output signal line for a first pulse width; and

generating the second pulse comprises coupling the second portion of the plurality of capacitors to the output signal line for a second pulse width, the second pulse width being different than the first pulse width.

13. The method of claim 12 , further comprising:

setting the first pulse width to terminate the first pulse decaying at the first decay rate at a first terminating voltage amplitude that is at least a threshold voltage amplitude; and

setting the second pulse width to terminate the second pulse decaying at the second decay rate to a second terminating voltage amplitude that is at least the threshold voltage amplitude.

14. The method of claim 11 , further comprising generating a third pulse of the composite pacing pulse by selectively coupling a third portion of the plurality of capacitors to the output signal line, the third portion of the plurality of capacitors having a third effective capacitance.

15. The method of claim 14 , further comprising selecting the third portion of the plurality of capacitors to have a third effective capacitance that is different than the second effective capacitance.

16. The method of claim 14 , further comprising generating the third pulse by coupling the third portion of the plurality of capacitors to the output signal line for the first pulse width.

17. The method of claim 14 , wherein generating the composite pacing pulse further comprises:

generating the first pulse by coupling the first portion of the plurality of capacitors to the output signal line for the first pulse width;

generating the third pulse of the composite pacing pulse consecutively following the first pulse by coupling the third portion of the plurality of capacitors to the output signal line for the first pulse width; and

generating the second pulse consecutively following the third pulse of the composite pacing pulse by coupling the second portion of the plurality of capacitors to the output signal line for the second pulse width.

18. The method of claim 17 , further comprising:

selecting the second portion of the plurality of capacitors to have the second effective capacitance that is greater than the first effective capacitance and greater than the third effective capacitance; and

setting the second pulse width to be greater than the first pulse width.

19. The method of claim 11 , wherein generating the composite pacing pulse comprises:

generating the first pulse having a first pulse energy that is less than a pacing capture threshold;

generating the second pulse having a second pulse energy that is less than the pacing capture threshold; and

generating the composite pacing pulse having a cumulative pulse energy comprising at least the first pulse energy and the second pulse energy that is at least the pacing capture threshold.

20. The method of claim 11 , further comprising:

sequentially coupling the first portion of the plurality of capacitors and the second portion of the plurality of capacitors to the output line via switching circuitry.

21. A non-transitory, computer-readable storage medium comprising a set of instructions which, when executed by a control module of a medical device, cause the medical device to:

generate a composite pacing pulse comprising a series of at least two individual pulses by:

generating a first pulse of the at least two individual pulses by selectively coupling a first portion of a plurality of capacitors to an output signal line, the selectively coupled first portion of the plurality of capacitors having a first effective capacitance and the first pulse of the at least two individual pulses having a first decay rate corresponding to the first effective capacitance, and

generating a second pulse of the at least two individual pulses by selectively coupling a second portion of the plurality of capacitors to the output signal line, the selectively coupled second portion of the plurality of capacitors having a second effective capacitance different than the first effective capacitance and the second pulse of the at least two individual pulses having a second decay rate different than the first decay rate, the second decay rate corresponding to the second effective capacitance.
