Abstract

The present application relates to compositions for oral immunotherapy of peanut allergies. Further, the present application relates to methods for the preparation of the compositions for immunotherapy, and their use in immunotherapy.

Claims (54)

1. A method of selecting a subject for treatment of a peanut allergy by oral immunotherapy, the method comprising:

administering to the subject a plurality of initial escalating doses of a pharmaceutical composition in a single day according to an initial dose escalation schedule, the pharmaceutical composition comprising,

a peanut protein having an amount of 0.5 mg to 6 mg in each initial escalating dose and

one or more diluents in an amount from 1% to 99% w/w, from 60% to 90% w/w, or from 5% to 20% w/w of the pharmaceutical composition,

one or more glidants in an amount from 0.01% to 10% w/w of the pharmaceutical composition, and

one or more lubricants in an amount from 0.01% to 10% w/w of the pharmaceutical composition,

wherein the administering is initiated by using a first initial escalating dose of the pharmaceutical composition comprising 0.5 mg of the peanut protein; and

selecting the subject for the treatment by oral immunotherapy based on the subject's tolerance of the plurality of initial escalating doses.

2. The method of claim 1 , further comprising monitoring the subject for symptoms during the administering of the plurality of initial escalating doses.

3. The method of claim 1 , wherein the subject is selected for the treatment by oral immunotherapy only if the subject tolerates a 1.5 mg dose of the peanut protein.

4. The method of claim 1 , wherein the subject is selected for the treatment by oral immunotherapy only if the subject tolerates a 3 mg dose of the peanut protein.

5. The method of claim 1 , further comprising initiating an up-dosing phase of the oral immunotherapy if the subject tolerates at least one of the plurality of initial escalating doses.

6. The method of claim 1 , wherein the plurality of initial escalating doses comprises a final dose of 6 mg of the peanut protein.

7. The method of claim 1 , wherein the plurality of initial escalating doses comprises two, three, four, or five dose levels.

8. The method of claim 1 , wherein the plurality of initial escalating doses comprises five dose levels.

9. The method of claim 8 , wherein the plurality of initial escalating doses comprises the five dose levels of 0.5 mg, 1 mg, 1.5 mg, 3 mg, and 6 mg of the peanut protein.

10. The method of claim 1 , wherein the plurality of initial escalating doses is administered within 30 minutes of one another.

11. A method for treating peanut allergy in a subject by oral immunotherapy, the method comprising:

selecting the subject for the treatment according to the method of claim 1 ; and administering to the subject escalating doses of the pharmaceutical composition comprising up to 300 mg of the peanut protein in each escalating dose according to an up-dosing schedule.

12. A method for treating peanut allergy in a subject by oral immunotherapy, the method comprising:

administering to the subject a plurality of initial escalating doses of a pharmaceutical composition in a single day according to an initial dose escalation schedule, the pharmaceutical composition comprising:

a peanut protein having an amount of 0.5 mg to 6 mg in each initial escalating dose and

one or more diluents in an amount from 1% to 99% w/w, from 60% to 90% w/w, or from 5% to 20% w/w of the pharmaceutical composition,

one or more glidants in an amount from 0.01% to 10% w/w of the pharmaceutical composition, and

one or more lubricants in an amount from 0.01% to 10% w/w of the pharmaceutical composition,

wherein the administering is initiated by using a first initial escalating dose of the pharmaceutical composition comprising 0.5 mg of the peanut protein.

13. The method of claim 12 , further comprising monitoring the subject for symptoms during the administering of the plurality of initial escalating doses.

14. The method of claim 12 , further comprising administering to the subject escalating doses of the pharmaceutical composition comprising up to 300 mg of the peanut protein in each escalating dose according to an up-dosing schedule.

15. The method of claim 14 , wherein the up-dosing schedule is initiated if the subject tolerates at least one of the plurality of initial escalating doses.

16. The method of claim 14 , wherein the up-dosing schedule is initiated only if the subject tolerates a 1.5 mg dose of the peanut protein.

17. The method of claim 14 , wherein the up-dosing schedule is initiated only if the subject tolerates a 3 mg dose of the peanut protein.

18. The method of claim 14 , wherein each dose level of the up-dosing schedule is administered daily over a period of at least two weeks before the next escalating dose level.

19. The method of claim 14 , further comprising mixing a dose of the up-dosing schedule with a food prior to administration.

20. The method of claim 14 , wherein the up-dosing schedule comprises administering a first dose of an escalating dose level under clinical observation.

21. The method of claim 12 , wherein the initial escalating doses are administered within 30 minutes of one another.

22. The method of claim 12 , wherein the plurality of initial escalating doses comprise two, three, four, or five dose levels.

23. The method of claim 12 , wherein the plurality of initial escalating doses comprises five dose levels.

24. The method of claim 23 , wherein the plurality of initial escalating doses comprises the five dose levels of 0.5 mg, 1 mg, 1.5 mg, 3 mg, and 6 mg of the peanut protein.

25. The method of claim 12 , wherein the plurality of initial escalating doses comprises a final dose of 6 mg of the peanut protein.

26. A method for treating peanut allergy in a subject by oral immunotherapy, the method comprising:

administering to the subject a plurality of initial escalating doses of a pharmaceutical composition in a single day according to an initial dose escalation schedule, the pharmaceutical composition comprising,

a peanut protein having an amount of 0.5 mg to 6 mg in each initial escalating dose and

one or more diluents in an amount from 1% to 99% w/w, from 60% to 90% w/w, or from 5% to 20% w/w of the pharmaceutical composition,

one or more glidants in an amount from 0.01% to 10% w/w, and

one or more lubricants in an amount from 0.01% to 10% w/w,

wherein the administering is initiated by using a first initial escalating dose of the pharmaceutical composition comprising 0.5 mg of the peanut protein;

monitoring the subject for symptoms during the administering of the plurality of initial escalating doses; and

administering to the subject escalating doses of the pharmaceutical composition comprising up to 300 mg of the peanut protein in each escalating dose according to an up-dosing schedule.

27. The method of claim 26 , wherein the plurality of initial escalating doses comprises a final dose of 6 mg of the peanut protein.

28. The method of claim 26 , wherein each dose level of the up-dosing schedule is administered daily over a period of at least two weeks before the next escalating dose level.

29. The method of claim 26 , wherein the up-dosing schedule comprises administering a first dose of an escalating dose level under clinical observation.

30. The method of claim 1 , wherein the one or more diluents comprise one or more of alginic acid and salts thereof; cellulose derivatives, carboxymethylcellulose, methylcellulose, hydroxypropylmethylcellulose, hydroxyethylcellulose, hydroxypropylcellulose, ethylcellulose, microcrystalline cellulose; silicified microcrystalline cellulose; microcrystalline dextrose; amylose; magnesium aluminum silicate; polysaccharide acids; bentonites; gelatin; polyvinylpyrrolidone/vinyl acetate copolymer; crospovidone; povidone; starch; pregelatinized starch; tragacanth, dextrin, sucrose, glucose, dextrose, molasses, mannitol, sorbitol, xylitol, lactose, lactose monohydrate, lactose anhydrous; dicalcium phosphate; acacia, tragacanth, ghatti gum, mucilage of isapol husks, polyvinylpyrrolidone, larch arabogalactan, polyethylene glycol, waxes, sodium alginate, starch, corn starch, potato starch, pregelatinized starch, sodium starch glycolate; cross-linked starch, sodium starch glycolate; cross-linked polymer; cross-linked polyvinylpyrrolidone; alginate, sodium alginate; clay, magnesium aluminum silicate; agar, guar, locust bean, Karaya, pectin; sodium starch glycolate; bentonite; a surfactant; a resin, cation-exchange resin; citrus pulp; sodium lauryl sulfate; sodium lauryl sulfate; and combinations thereof.

31. The method of claim 1 , wherein the one or more glidants comprise colloidal silicon dioxide, talc, or a combination thereof.

32. The method of claim 1 , wherein the one or more lubricants comprise stearic acid, calcium hydroxide, talc, corn starch, sodium stearyl fumarate, an alkali-metal salt, an alkaline earth metal salt, a polyethylene glycol, a methoxypolyethylene glycol, sodium oleate, glyceryl behenate, glyceryl palmitostearate, glyceryl benzoate, magnesium lauryl sulfate, sodium lauryl sulfate, or combinations thereof.
