Abstract

An automated measurement device and method for coronary artery disease scoring is disclosed. An example device includes a processor configured to obtain a computerized model of a plurality of vascular segments of a patient and create an unstenosed computerized model from the computerized model by virtually enlarging at least some locations of the vascular segments of the computerized model. The processor also determines vascular state scoring tool (“VSST”) scores based on characteristics of vascular locations along the vascular segments. The processor further determines a severity of stenosis for the vascular locations based on comparisons of first blood flow parameter values at the vascular locations in the computerized model to corresponding second blood flow parameter values at the same vascular locations in the unstenosed computerized model. A user interface of the device displays the severity of stenosis in conjunction with the VSST scores for the vascular locations.

Claims (52)

1. An apparatus for coronary artery disease scoring, the apparatus comprising:

a vascular tree reconstruction processor configured to:

obtain a computerized model that is representative of vascular segments of a patient, and

create an unstenosed computerized model from the computerized model by virtually enlarging at least some locations of the vascular segments of the computerized model;

a vascular state score calculator communicatively coupled to the vascular tree reconstruction processor and configured to:

determine vascular state scoring tool (“VSST”) scores for vascular locations along the vascular segments based on at least two of (a) a morphometric quantity of the vascular location, (b) a distance of the vascular location from a branch point in the plurality of vascular segments, (c) a sub-tree dominance of the vascular location, (d) a branch position related to the vascular location, (e) an indication of occlusion that is related to the vascular location, (f) an indication of tortuosity that is related to the vascular location, (g) an estimated calcification that is related to the vascular location, (h) an estimated thrombus that is related to the vascular location, (i) a bifurcation/trifurcation classification that is related to the vascular location, (j) a bifurcation/trifurcation angulation that is related to the vascular location, or (k) a vascular position of the vascular location, the VSST scores being indicative of a complexity of coronary artery disease as an aid for planning treatment,

determine a severity of stenosis for the vascular locations based on comparisons of first blood flow parameter values at the vascular locations in the computerized model to corresponding second blood flow parameter values at the same vascular locations in the unstenosed computerized model,

determine a first recommendation for a percutaneous coronary intervention action when at least one of the VSST scores of the vascular locations is within a first range, and

determine a second recommendation for a coronary artery bypass surgery action when at least one of the VSST scores of the vascular locations is within a second range that is greater in value than the first range; and

a user interface configured to display:

the severity of stenosis in conjunction with the VSST scores for the vascular locations, and

at least one of the first recommendation and the second recommendation.

2. The apparatus of claim 1 , wherein the computerized model includes a three-dimensional model or a one-dimensional model.

3. The apparatus of claim 1 , wherein the vascular state score calculator is configured to relate at least one of the severity of stenosis or the VSST scores of the vascular locations to corresponding locations in the computerized model or the unstenosed computerized model.

4. The apparatus of claim 1 , wherein the vascular state score calculator is configured to:

relate the VSST scores for the vascular locations to corresponding locations in at least one of the computerized model or the unstenosed computerized model; and

relate the severity of stenosis for the vascular locations to the corresponding locations in at least one of the computerized model or the unstenosed computerized model, and

wherein the user interface is configured to display at least one of the computerized model or the unstenosed computerized model with the related VSST scores and severity of stenosis shown adjacent to or overlaid on the corresponding vascular locations within the representation of the vascular segments.

5. The apparatus of claim 1 , wherein the computerized model is based on vascular image data that is recorded by a medical imaging device, and

wherein the vascular image data includes at least one of X-ray angiograms, multi-detector computed tomography (“MDCT”) images, computed tomography (“CT”) images, magnetic resonance imaging (“MRI”) images, positron emission tomography (“PET”) images, optical coherence tomography (“OCT”) images, and intravascular ultrasound (“IVUS”) images.

6. The apparatus of claim 1 , wherein the vascular tree reconstruction processor is configured to:

determine centerlines through the representation of the vascular segments in the computerized model; and

determine at least one of a cross-sectional area, a diameter, or a radius at the vascular locations along the determined centerlines.

The apparatus of claim 6 , wherein the vascular state score calculator is configured to use the at least one of the cross-sectional area, the diameter, or the radius at the vascular locations for determining at least one of the VSST scores for the vascular locations or the severity of stenosis for the vascular locations.

8. The apparatus of claim 7 , wherein the vascular tree reconstruction processor is configured to determine at least one of vessel curvature or tortuosity for at least some of the vascular locations.

The apparatus of claim 6 , wherein the morphometric quantity includes at least one of (i) the cross-sectional area, the diameter, or the radius at the vascular location, (ii) a vascular width at the vascular location, (iii) a vascular curvature at the vascular location, (iv) a flow capacity related to the vascular location, (v) a vascular elasticity at the vascular location, or (vi) a vascular wall composition at the vascular location.

10. The apparatus of claim 1 , wherein the vascular state score calculator is configured to determine at least one of the first recommendation and the second recommendation while the patient is undergoing catheterization.

11. The apparatus of claim 1 , wherein the VSST scores include SYNTAX Scores.

12. A method for coronary artery disease scoring, the method comprising:

obtaining, in a processor, a computerized model that is representative of vascular segments of a patient;

creating, via the processor, an unstenosed computerized model from the computerized model by virtually enlarging at least some locations of the vascular segments of the computerized model;

determining, via the processor, vascular state scoring tool (“VSST”) scores for vascular locations along the vascular segments based on at least two of (a) a morphometric quantity of the vascular location, (b) a distance of the vascular location from a branch point in the plurality of vascular segments, (c) a sub-tree dominance of the vascular location, (d) a branch position related to the vascular location, (e) an indication of occlusion that is related to the vascular location, (f) an indication of tortuosity that is related to the vascular location, (g) an estimated calcification that is related to the vascular location, (h) an estimated thrombus that is related to the vascular location, (i) a bifurcation/trifurcation classification that is related to the vascular location, (j) a bifurcation/trifurcation angulation that is related to the vascular location, or (k) a vascular position of the vascular location, the VSST scores being indicative of a complexity of coronary artery disease;

determining, via the processor, a severity of stenosis for the vascular locations based on comparisons of first blood flow parameter values at the vascular locations in the computerized model to corresponding second blood flow parameter values at the same vascular locations in the unstenosed computerized model;

determining, via the processor, a first recommendation for a percutaneous coronary intervention action when at least one of the VSST scores of the vascular locations is within a first range;

determining, via the processor, a second recommendation for a coronary artery bypass surgery action when at least one of the VSST scores of the vascular locations is within a second range that is greater in value than the first range;

displaying, via a user interface that is in communication with the processor, the severity of stenosis in conjunction with the VSST scores for the vascular locations; and

displaying, via the user interface, at least one of the first recommendation and the second recommendation.

13. The method of claim 12 , wherein the representation of the vascular segments corresponds to arterial vasculature of the patient's heart, and

wherein the VSST scores include SYNTAX Scores.

The method of claim 12 , wherein obtaining the computerized model, the creation of the unstenosed computerized model, the determination of the VSST scores for the vascular locations, and the determination of the severity of stenosis for the vascular locations occur while the patient is undergoing catheterization.

15. The method of claim 12 , further comprising:

relating, via the processor, the VSST scores for the vascular locations to corresponding locations in at least one of the computerized model or the unstenosed computerized model;

relating, via the processor, the severity of stenosis for the vascular locations to the corresponding locations in at least one of the computerized model or the unstenosed computerized model; and

displaying, via the user interface, at least one of the computerized model or the unstenosed computerized model with the related VSST scores and severity of stenosis shown adjacent to or overlaid on the corresponding vascular locations within the representation of the vascular segments.

16. The method of claim 12 , further comprising:

determining, via the processor, centerlines through the representation of the vascular segments in the computerized model; and

determining, via the processor, at least one of a cross-sectional area, a diameter, or a radius at the vascular locations.

17. The method of claim 16 , wherein at least one of the VSST scores for the vascular locations or the severity of stenosis for the vascular locations is determined based, in part, on the at least one of the cross-sectional area, the diameter, or the radius of the vascular locations.

18. The method of claim 12 , further comprising:

receiving, in the processor, vascular image data of the vascular segments of the patient; and

creating, via the processor, the computerized model that is representative of the vascular segments of the patient using the vascular image data,

wherein the vascular image data includes at least one of X-ray angiograms, multi-detector computed tomography (“MDCT”) images, computed tomography (“CT”) images, magnetic resonance imaging (“MRI”) images, positron emission tomography (“PET”) images, optical coherence tomography (“OCT”) images, and intravascular ultrasound (“IVUS”) images.
