Abstract

An electrosurgical device ( 10 ) is provided that is operable to deliver microwave energy within a controlled angular expanse to cause targeted tissue ablation. The device ( 10 ) comprises a blocking or reflecting material such as cylindrical members ( 34 ) that are laterally spaced from the antenna ( 20 ) that is operable to emit the microwave energy. The reflecting material creates regions in and/or surrounding the device into which sensors ( 51 ), such as thermocouple wires, may be placed to monitor a condition associated with the device or the patient's body.

Claims (52)

1. A device for delivering electromagnetic energy to a target tissue within a human or animal body comprising:

an elongate body comprising a proximal end, a distal end, and at least one lumen interconnecting the proximal and distal ends;

an antenna located within the at least one lumen and configured to emit electromagnetic energy therefrom sufficiently strong to cause tissue ablation; and

at least one elongate cylindrical member located within the same or a different lumen as the antenna, the at least one elongate cylindrical member being disposed laterally from the antenna, the at least one elongate cylindrical member comprising a distal end segment that is secured to the elongate body distal end,

wherein the at least one elongate cylindrical member being configured to reflect a portion of the electromagnetic energy emitted from the antenna and thereby provide a directional pattern for delivery of the electromagnetic energy to the target tissue.

2. The device of claim 1 , wherein the device comprises at least two of the elongate cylindrical members, and wherein the at least two elongate cylindrical members are affixed together.

3. The device of claim 1 , wherein the device comprises at least two of the elongate cylindrical members, and wherein the at least two elongate cylindrical members have different diameters.

4. The device of claim 1 , wherein at least a distal end of the at least one elongate cylindrical member comprises one or more conductive elements.

5. The device of claim 1 , wherein the at least one elongate cylindrical member comprises a wire.

6. The device of claim 1 , wherein the at least one elongate cylindrical member comprises at least one tube.

7. The device of claim 6 , wherein the device is configured to circulate a cooling fluid through the at least one tube and into the at least one lumen.

The device of claim 6 , wherein the device further comprises at least one sensor configured to detect an operating condition of the device or a condition associated with the body in which the target tissue is located, wherein the at least one tube comprises a conductive pathway for the at least one sensor and/or provides shielding from microwave interference or heating of the at least one sensor.

9. The device of claim 1 , wherein the device comprises a thermocouple, the thermocouple being shielded from the electromagnetic energy emitted from the antenna by the at least one elongate cylindrical member.

10. The device of claim 1 , wherein the antenna comprises a coaxial transmission cable.

11. The device of claim 10 , wherein the coaxial transmission cable comprises an outer conductor that is affixed to one of the at least one elongate cylindrical member.

12. The device of claim 1

wherein the elongate body comprises a synthetic resin material.

13. The device of claim 12 , wherein the elongate body comprises at least two lumens interconnecting the proximal and distal ends, wherein adjacent lumens are separated by a wall structure extending between the proximal and distal ends.

The device of claim 1 , wherein the elongate body comprises an outer tubular member having a proximal and a distal end, and an inner tubular member having a proximal and a distal end, the proximal end of the inner tubular member being received through the distal end of the outer tubular member, the inner tubular member comprising a window section formed therein that is configured to transmit the electromagnetic energy emitted from the antenna, there being an electromagnetically transparent sleeve positioned around at least a portion of the inner tubular member in covering relationship to the window section.

The device of claim 1 , wherein the device comprises a swivel connector coupled to the proximal end of the elongate body and to a base portion of the device, the base portion of the device being operably connected to a source of electromagnetic energy and/or a pump for circulating a cooling fluid within the device, the swivel connector permits free axial rotation of the base portion relative to the elongate body.

The device of claim 1 , wherein the device comprises a base portion that is connected to the proximal end of the elongate body, the base portion being operably connected to a source of electromagnetic energy and/or a pump for circulating a cooling fluid within the device via a formable transmission cable, the formable transmission cable being configured to freely support the weight of the elongate body and base portion without additional assistance.

17. The device of claim 1 , wherein the device comprises at least two of the elongate cylindrical members, and wherein the at least two elongate cylindrical members have the same diameters.

18. A device for delivering electromagnetic energy to a target tissue within a human or animal body comprising:

an elongate body comprising a proximal end, a distal end, and at least one lumen interconnecting the proximal and distal ends;

an antenna located within the at least one lumen and configured to emit electromagnetic energy therefrom sufficiently strong to cause tissue ablation; and

at least one elongate cylindrical member located within the same or a different lumen as the antenna, the at least one elongate cylindrical member being disposed laterally from the antenna, the at least one elongate cylindrical member comprising a distal end segment that is secured to the elongate body distal end,

wherein the at least one elongate cylindrical member comprises at least one tube,

wherein the at least one tube is configured to provide a conduit for fluid and/or drug delivery to the human or animal body in which the target tissue is located, wherein the conduit has an open end configured to emit the fluid and/or drug from the device and into the human or animal body.

19. A device for delivering electromagnetic energy to a target tissue within a human or animal body comprising:

an elongate body comprising a proximal end, a distal end, and at least one lumen interconnecting the proximal and distal ends;

an antenna located within the at least one lumen and configured to emit electromagnetic energy therefrom sufficiently strong to cause tissue ablation; and

at least one elongate cylindrical member located within the same or a different lumen as the antenna, the at least one elongate cylindrical member being disposed laterally from the antenna,

the elongate body distal end comprising a rigid spacer located at the distal end thereof, a distal portion of the antenna being received in the spacer, a portion of the spacer that is located within the elongate body contacting and supporting the distal portion of the antenna and the distal portion of the at least one elongate cylindrical member and maintaining the position of the antenna relative to the at least one elongate cylindrical member.

20. A method of ablating tissue within a human or animal body comprising:

inserting at least one directional device for delivering electromagnetic energy to the tissue into the body containing the tissue to be ablated, wherein the at least one device comprises:

an elongate body comprising a proximal end, a distal end, and at least one lumen interconnecting the proximal and distal ends;

an antenna located within the at least one lumen and configured to emit electromagnetic energy therefrom sufficiently strong to cause tissue ablation; and

at least one elongate cylindrical member located within the same or a different lumen as the antenna, the at least one elongate cylindrical member being disposed laterally from the antenna, the at least one elongate cylindrical member comprising a distal end segment that is secured to the elongate body distal end, wherein the at least one elongate cylindrical member being configured to reflect a portion of the electromagnetic energy emitted from the antenna and thereby provide a directional pattern for delivery of the electromagnetic energy to the tissue;

positioning the device antenna into or adjacent to the tissue to be ablated; and

activating the device thereby causing the antenna to emit electromagnetic radiation along a predetermined angular expanse that is sufficiently strong to cause ablation of the tissue,

wherein the tissue to be ablated comprises a tumor, and wherein the method comprises inserting a plurality of the devices into the body containing the tumor, the device antennas being positioned within the body substantially outside the margins of the tumor, the activating of the devices causing ablation of the tumor from the outside of the tumor in toward the center of the tumor.

The method of claim 20 , wherein the tissue to be ablated is located within an organ of the body, and wherein the at least one device is inserted percutaneously, bronchoscopically, laparoscopically, endoscopically, endo-luminally, endo-vascularly, or during open surgery into the body and the antenna positioned in direct contact with, but without penetrating, the organ.

22. The method of claim 20 , wherein the device is percutaneously, bronchoscopically, laparoscopically, endoscopically, endo-luminally, endo-vascularly, or during open surgery inserted into the body and into an organ in which the tissue to be ablated is located.

23. The method of claim 20

wherein the method further comprises cutting or cauterizing tissue located within the body by delivering radio frequency energy from a device tip located at the distal end of the elongate body.

24. The method of claim 20 , wherein the method further comprises using the device to sense an electrical characteristic of a tissue located within the body.

25. A method of ablating a tumor within a body comprising:

inserting a directional microwave ablation device into the tumor to be ablated, the device comprising an elongate body comprising a proximal end, a distal end, and at least one lumen interconnecting the proximal and distal ends, an antenna that is located within the at least one lumen and configured to emit microwave energy therefrom that is sufficiently strong to cause tissue ablation, at least one elongate cylindrical member located within the same or different lumen as the antenna, the at least one elongate cylindrical member being configured to reflect a portion of the microwave energy emitted from the antenna and thereby provide a directional pattern for delivery of the microwave energy to the target tissue;

activating the device thereby causing the antenna to emit microwave energy along a predetermined angular expanse that is sufficiently strong to cause ablation of a portion of the tumor while the antenna remains within the at least one lumen and within the elongate body;

rotating the device within the tumor without removing the device from the tumor; and

ablating another portion of the tumor with microwave energy emitted from the device.

26. The method of claim 25 , wherein a different level of electromagnetic radiation is emitted from the device when ablating each portion of the tumor, thereby forming an asymmetrical ablation pattern within the tumor.
