Abstract

The subject invention provides a robust, quantitative, and reproducible process and assay for diagnosis of a neurological condition in a subject. The invention provides measurement of two or more biomarkers in a biological fluid such as CSF or serum resulting in a synergistic mechanism for determining the extent of neurological damage in a subject with an abnormal neurological condition and for discerning subtypes thereof or tissue types subjected to damage.

Claims (32)

1. A process for determining the neurological condition of a subject or cells from the subject, wherein the subject is known to have or is suspected of having a mild to moderate traumatic brain injury (TBI), wherein mild TBI is characterized as the subject having a Glasgow Coma Score (GCS) of 13-15 and moderate TBI is characterized as the subject having a GCS of 9-12, the process comprising:

measuring a sample obtained from the subject or cells from the subject at a first time for a quantity of a first biomarker, and a quantity of at least one additional neuroactive biomarker, wherein the first biomarker is glial fibrillary acidic protein (GFAP), and wherein the first time is within 12 hours of the TBI or the suspected TBI;

comparing the quantity of said first biomarker and the quantity of said at least one additional neuroactive biomarker to normal levels of said first biomarker and said at least one additional neuroactive biomarker;

predicting a presence of an intracranial lesion in the subject based on the quantity of said first biomarker and said additional neuroactive biomarker relative to the normal levels of said first biomarker and said at least one additional neuroactive biomarker,

wherein a presence of an intracranial lesion is predicted if said first biomarker and/or said at least one additional neuroactive biomarker are elevated relative to the normal levels of said first biomarker and said at least one additional neuroactive biomarker;

wherein an absence of an intracranial lesion is predicted if said first biomarker and said at least one additional neuroactive biomarker are not elevated relative to the normal levels of said first biomarker and said at least one additional neuroactive biomarker; and

performing a head CT on the subject when the presence of an intracranial lesion is predicted or

declining a head CT on the subject when the absence of an intracranial lesion is predicted.

2. The process of claim 1 , wherein the sample is cerebrospinal fluid, plasma, or blood serum.

3. The process of claim 1 , wherein the subject has been exposed to a drug candidate or an environmental contaminant and the sample is a culture of cells from the subject.

The process of claim 1 , wherein said at least one additional neuroactive biomarker is selected from the group of ubiquitin carboxyl-terminal hydrolase L1 (UCH-L1), neuron specific enolase (NSE), spectrin breakdown products 150 (SBDP150), SBDP145, SBDP120, 5100 calcium binding protein B (S100b), microtubule-associated protein 2 (MAP2), MAP1, MAP3, MAP4, MAPS, myelin basic protein (MBP), Tau, Neurofilament protein (NF), Cannabinoid Receptor CB, CAM, Synaptic protein, collapsin response mediator proteins (CRMP), inducible nitric oxide synthase (iNOS), Neuronal Nuclei protein (NeuN), 2′,3′-cyclic nucleotide-3′-phosphohydrolase (CNPase), Neuroserpin, alpha-internexin, microtubule-associated protein 1 light chain 3 (LC3), Neurofascin, the glutamate transporters (EAAT), Nestin, Cortin-1, and BIII-Tubulin.

The process of claim 1 , further comprising measuring a second quantity of said first biomarker and a second quantity of said at least one additional neuroactive biomarker at a second time to yield a kinetic profile for said first biomarker and said at least one additional neuroactive biomarker, wherein the second time is within 12 hours of the TBI or the suspected TBI.

6. The process of claim 1 , further comprising comparing the quantity of said first biomarker and the quantity of said at least one additional neuroactive biomarker between normal levels in the subject to other individuals of the same gender as the subject.

7. The process of claim 1 , wherein said at least one additional neuroactive biomarker is UCH-L1.

8. The process of claim 1 , further comprising determining if the subject or cells from the subject has been exposed to some degree of traumatic brain injury ranging from mild to moderate.

9. The process of claim 8 , wherein said at least one additional neuroreactive biomarker is UCH-L1, and the process further comprises identifying the magnitude of TBI based on the quantity of UCH-L1 and the quantity of GFAP.

10. The process of claim 8 , wherein mild traumatic brain injury and moderate traumatic brain injury have detection cutoffs for UCH-L1 and GFAP in serum of 0.39 ng/ml and 1.4 ng/ml, respectively.

11. The process of claim 1 , wherein the at least one additional neuroactive biomarker is S100b.

12. The process of claim 1 , wherein the at least one additional neuroactive biomarker is selected from the group of SBDP150, SBDP150i, SBDP145, or SBDP120.

13. The process of claim 1 , wherein the at least one additional neuroactive biomarker is NSE.

14. The process of claim 1 , wherein the at least one additional neuroactive biomarker is selected from the group of MAP2, MAP1, MAP3, MAP4, or MAPS.

15. A method of assaying a biomarker combination after a traumatic brain injury (TBI), comprising:

obtaining a blood or cerebral spinal fluid (CSF) sample collected from a subject having the TBI within 12 hours after occurrence of the TBI; and

measuring levels of ubiquitin carboxyl-terminal hydrolase L1 (UCH-L1) and glial fibrillary acidic protein (GFAP) by assaying the blood or CSF sample.

16. The method of claim 15 , wherein the blood or CSF sample is a blood sample.

17. The method of claim 16 , wherein the blood sample is collected from the subject within 2 hours after the occurrence of the TBI.

18. A method of performing a head CT scan, comprising:

measuring levels of UCH-L1 and GFAP by performing the method of claim 16 ; and

performing the head CT scan on the subject when the levels of UCH-L1 and/or GFAP exceed predetermined cutoff values.

19. A method of refraining from performing a head CT scan on a subject having a TBI, comprising:

measuring levels of UCH-L1 and GFAP by performing the method of claim 16 ; and

refraining from performing the head CT scan on the subject when the levels of UCH-L1 and GFAP do not exceed predetermined cutoff values.
