Abstract

An implantable medical device comprises a sensing module configured to obtain electrical signals from one or more electrodes and a control module configured to process the electrical signals from the sensing module in accordance with a tachyarrhythmia detection algorithm to monitor for a tachyarrhythmia. The control module detects initiation of a pacing train delivered by a second implantable medical device, determines a type of the detected pacing train, and modifies the tachyarrhythmia detection algorithm based on the type of the detected pacing train.

Claims (84)

1. A device comprising:

a therapy module configured to deliver electrical stimulation therapy to a patient;

a sensing module configured to sense a cardiac electrical signal of the patient; and

a control module configured to:

process the cardiac electrical signal in accordance with a tachyarrhythmia detection algorithm to monitor for a tachyarrhythmia;

detect initiation of a pacing train delivered by a second device;

determine, based on a cycle length of the detected pacing train, that the detected pacing train is an anti-bradycardia pacing train;

modify the tachyarrhythmia detection algorithm based on determining that the detected pacing train is an anti-bradycardia pacing train;

determine, based on the modified tachyarrhythmia detection algorithm, whether the cardiac electrical signal indicates an arrhythmia; and

control, based on determining that the cardiac electrical signal indicates the arrhythmia, the therapy module to deliver the electrical stimulation therapy to the patient.

2. The device of claim 1 , wherein determining that the pacing train is an anti-bradycardia train comprises detecting a fast bradycardia pacing train.

3. The device of claim 2 , wherein to detect the fast bradycardia pacing train, the control module is configured to:

compute the cycle length of the detected pacing train; and

detect the fast bradycardia pacing train when the cycle length of the detected pacing train is greater than 400 milliseconds.

4. The device of claim 2 , wherein the control module is further configured to:

determine whether a heart rate exceeds a heart rate threshold;

in response to determining that the heart rate exceeds the heart rate threshold, modify the tachyarrhythmia detection algorithm; and

in response to determining that the heart rate does not exceed a heart rate threshold, continue to use the tachyarrhythmia detection algorithm without modification.

5. The device of claim 1 , wherein to determine whether the cardiac electrical signal indicates the arrhythmia, the control module is configured to implement a beat-based morphology analysis to monitor a consistency of a morphology of beats within the cardiac electrical signal.

The device of claim 5 , wherein the cardiac electrical signal includes a cardiac electrical signal segment, wherein the cardiac electrical signal segment indicates a sequence of cardiac events, and wherein the control module is configured to implement the beat-based morphology analysis to monitor the consistency of the morphology of beats by:

comparing a morphology of a first cardiac event of the sequence of cardiac events with a morphology of each cardiac event of one or more cardiac events following the first cardiac event in the sequence of cardiac events;

classifying each of the comparisons as a match or non-match, wherein each cardiac event is classified as match when a matching score is greater than or equal to a matching score threshold and otherwise is classified as non-match;

detecting that a number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is less than a threshold number of cardiac events;

characterizing the cardiac electrical signal segment as shockable when gross morphology criteria are met and the number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is less than the threshold number of cardiac events; and

detecting the arrhythmia requiring the electrical stimulation therapy based characterizing the cardiac electrical signal segment as shockable,

and

wherein the control module is configured to control the therapy module to deliver the electrical stimulation therapy to the patient based on detecting the arrhythmia requiring the electrical stimulation therapy.

The device of claim 5 , wherein the cardiac electrical signal includes a cardiac electrical signal segment, wherein the cardiac electrical signal segment indicates a sequence of cardiac events, and wherein the control module is configured to implement the beat-based morphology analysis to monitor the consistency of the morphology of beats by:

comparing a morphology of a first cardiac event of the sequence of cardiac events with a morphology of each cardiac event of one or more cardiac events following the first cardiac event in the sequence of cardiac events;

classifying each of the comparisons as a match or non-match, wherein each cardiac event is classified as match when a matching score is greater than or equal to a matching score threshold and otherwise is classified as non-match;

detecting that a number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is greater than or equal to a first threshold number of cardiac events and greater than a second threshold number of cardiac events;

characterizing the cardiac electrical signal segment as shockable when gross morphology criteria are met and the number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is greater than or equal to the first threshold number of cardiac events and greater than the second threshold number of cardiac events;

detecting the arrhythmia requiring the electrical stimulation therapy based on characterizing the cardiac electrical signal segment as shockable,

and

wherein the control module is configured to control the therapy module to deliver the electrical stimulation therapy to the patient based on detecting the arrhythmia requiring the electrical stimulation therapy.

The device of claim 5 , wherein the cardiac electrical signal includes a cardiac electrical signal segment, wherein the cardiac electrical signal segment indicates a sequence of cardiac events, and wherein the control module is configured to implement the beat-based morphology analysis to monitor the consistency of the morphology of beats by:

comparing a morphology of a first cardiac event of the sequence of cardiac events with a morphology of each cardiac event of one or more cardiac events following the first cardiac event in the sequence of cardiac events;

classifying each of the comparisons as a match or non-match, wherein each cardiac event is classified as match when a matching score is greater than or equal to a matching score threshold and otherwise is classified as non-match;

detecting that a number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is greater than or equal to a first threshold number of cardiac events and less than or equal to a second threshold number of cardiac events;

characterizing the cardiac electrical signal segment as non-shockable when the number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is greater than or equal to the first threshold number of cardiac events and less than or equal to a second threshold number of cardiac events;

detecting the arrhythmia not requiring the electrical stimulation therapy based on characterizing the cardiac electrical signal segment as non-shockable,

and

wherein the control module is configured to control the therapy module not to deliver the electrical stimulation therapy to the patient based on detecting the arrhythmia not requiring the electrical stimulation therapy.

9. A method comprising:

sensing, by a sensing module of a device, a cardiac electrical signal of a patient; and

processing, by a control module of the device, the cardiac electrical signal in accordance with a tachyarrhythmia detection algorithm to monitor for a tachyarrhythmia;

detecting, by the control module, initiation of a pacing train delivered by a second device;

determining, by the control module based on a cycle length of the detected pacing train, that the detected pacing train is an anti-bradycardia pacing train;

modifying, by the control module, the tachyarrhythmia detection algorithm based on determining that the detected pacing train is an anti-bradycardia pacing train;

determining, by the control module based on the modified tachyarrhythmia detection algorithm, the cardiac electrical signal indicates an arrhythmia; and

controlling, by the control module based on determining that the cardiac electrical signal indicates the arrhythmia, the therapy module to deliver the electrical stimulation therapy to the patient.

10. The method of claim 9 , wherein determining that the pacing train is an anti-bradycardia train comprises detecting a fast bradycardia pacing train.

11. The method of claim 10 , wherein detecting the fast bradycardia pacing train comprises:

computing the cycle length of the detected pacing train; and

detecting the fast bradycardia pacing train when the cycle length of the detected pacing train is greater than 400 milliseconds.

12. The method of claim 10 , wherein the method further comprises:

determining, by the control module, whether a heart rate exceeds a heart rate threshold;

in response to determining that the heart rate exceeds the heart rate threshold, modifying, by the control module, the tachyarrhythmia detection algorithm; and

in response to determining that the heart rate does not exceed a heart rate threshold, continuing, by the control module, to use the tachyarrhythmia detection algorithm without modification.

13. The method of claim 9 , wherein determining whether the cardiac electrical signal indicates the arrhythmia comprises implementing a beat-based morphology analysis to monitor a consistency of a morphology of beats within the cardiac electrical signal.

The method of claim 13 , wherein the cardiac electrical signal includes a cardiac electrical signal segment, wherein the cardiac electrical signal segment indicates a sequence of cardiac events, and wherein implementing the beat-based morphology analysis to monitor the consistency of the morphology of beats comprises:

comparing a morphology of a first cardiac event of the sequence of cardiac events with a morphology of each cardiac event of one or more cardiac events following the first cardiac event in the sequence of cardiac events;

classifying each of the comparisons as a match or non-match, wherein each cardiac event is classified as match when a matching score is greater than or equal to a matching score threshold and otherwise is classified as non-match;

detecting that a number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is less than a threshold number of cardiac events;

characterizing the cardiac electrical signal segment as shockable when gross morphology criteria are met and the number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is less than the threshold number of cardiac events; and

detecting the arrhythmia requiring the electrical stimulation therapy based on characterizing the cardiac electrical signal segment as shockable,

and

wherein the method further comprises controlling, by the control module, the therapy module to deliver the electrical stimulation therapy to the patient based on detecting the arrhythmia requiring the electrical stimulation therapy.

The method of claim 13 , wherein the cardiac electrical signal includes a cardiac electrical signal segment, wherein the cardiac electrical signal segment indicates a sequence of cardiac events, and wherein implementing the beat-based morphology analysis to monitor the consistency of the morphology of beats comprises:

comparing a morphology of a first cardiac event of the sequence of cardiac events with a morphology of each cardiac event of one or more cardiac events following the first cardiac event in the sequence of cardiac events;

classifying each of the comparisons as a match or non-match, wherein each cardiac event is classified as match when a matching score is greater than or equal to a matching score threshold and otherwise is classified as non-match;

detecting a the number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is greater than or equal to a first threshold number of cardiac events and greater than a second threshold number of cardiac events;

characterizing the cardiac electrical signal segment as shockable when gross morphology criteria are met and the number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is greater than or equal to the first threshold number of cardiac events and greater than the second threshold number of cardiac events;

detecting the arrhythmia requiring the electrical stimulation therapy based characterizing the cardiac electrical signal segment as shockable,

and

wherein the method further comprises controlling, by the control module, the therapy module to deliver the electrical stimulation therapy to the patient based on detecting the arrhythmia requiring the electrical stimulation therapy.

The method of claim 13 , wherein the cardiac electrical signal includes a cardiac electrical signal segment, wherein the cardiac electrical signal segment indicates a sequence of cardiac events, and wherein wherein implementing the beat-based morphology analysis to monitor the consistency of the morphology of beats comprises:

comparing a morphology of a first cardiac event of the sequence of cardiac events with a morphology of each cardiac event of one or more cardiac events following the first cardiac event in the sequence of cardiac events;

classifying each of the comparisons as a match or non-match, wherein each cardiac event is classified as match when a matching score is greater than or equal to a matching score threshold and otherwise is classified as non-match;

detecting that a number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is greater than or equal to a first threshold number of cardiac events and less than or equal to a second threshold number of cardiac events;

characterizing the cardiac electrical signal segment as non-shockable when the number of subsequent cardiac events having morphologies that match the morphology of the first cardiac event of the cardiac electrical signal segment is greater than or equal to the first threshold number of cardiac events and less than or equal to a second threshold number of cardiac events;

detecting the arrhythmia not requiring the electrical stimulation therapy based on characterizing the cardiac electrical signal segment as non-shockable,

and

wherein the method further comprises controlling, by the control module, the therapy module not to deliver the electrical stimulation therapy to the patient based on detecting the arrhythmia not requiring the electrical stimulation therapy.
