Abstract

A uterine manipulator can include a shaft including a first end, a second end, and a channel along an axis of the shaft, a handle coupled to the first end of the shaft, and a triangular balloon coupled to the second end of the shaft. The triangular balloon can be configured to inflate upon insertion into a vagina via a fluid injected into the channel of the shaft.

Claims (44)

1. A method of performing a procedure on a uterus of a patient using a uterine manipulator including an elongate shaft, an expandable anchor, and a light source disposed within the expandable anchor, the method comprising:

introducing the expandable anchor into the uterus;

expanding the expandable anchor within the uterus;

operating the light source to internally illuminate the uterus with red light having wavelength between about 630 nm and about 700 nm;

insufflating an abdominal cavity of the patient;

introducing an endoscope into the abdominal cavity of the patient; and

directly examining the outer surface of the uterus using the endoscope wherein the light source is of sufficient intensity to illuminate the uterus internally and project light outside of the uterus to permit observation.

2. The method of claim 1 , further comprising inspecting the patient's uterus externally to search for fibroids.

3. The method of claim 1 , wherein the light source comprises a plurality of LEDs having different color output attributes.

4. The method of claim 3 , wherein the plurality of LEDs have a light output between about 2,500 lumen and 10,000 lumens.

5. The method of claim 1 , wherein the uterine manipulator further includes a cutting head attached to the distal end of the shaft, and further wherein the method further comprises:

engaging a cervix of the patient with the cutting head; and

operating the cutting head to separate the uterus from a vagina of the patient.

The method of claim 5 , wherein the cutting head has a cup and a cutter coupled to the cup, wherein operating the cutting head comprises advancing the cutter relative to the cup to deploy a distal portion of the cutter through the tissue of the vagina into an abdominal cavity of the patient, and rotating the cutter about a longitudinal axis of the uterine manipulator to separate the uterus from the vagina.

7. The method of claim 6 , wherein rotating the cutter about the longitudinal axis of the uterine manipulator comprises rotating the cup about the longitudinal axis of the uterine manipulator.

8. The method of claim 1 , wherein the expandable anchor includes an inflatable body configured to be inflated by an inflation fluid.

The method of claim 1 , wherein the expandable anchor includes a triangular inflatable body configured to be inflated by an inflation fluid, wherein the triangular inflatable body includes a pair of distal tips configured to be oriented toward an entrance from the uterus into a respective Fallopian tube.

10. The method of claim 9 , further comprising a beneficial agent into each said Fallopian tube through an opening defined in each distal tip of the triangular inflatable body.

11. The method of claim 10 , wherein the beneficial agent includes a dye.

12. A method of performing a myomectomy on a uterus of a patient using a uterine manipulator including an elongate shaft, an expandable anchor, and a light source disposed within the expandable anchor, the method comprising:

introducing the expandable anchor into the uterus;

expanding the expandable anchor within the uterus;

operating the light source to internally illuminate a lining of the uterus with red light having wavelength between about 630 nm and about 700 nm;

insufflating an abdominal cavity of the patient;

introducing an endoscope into the abdominal cavity of the patient;

directly examining the outer surface of the uterus using the endoscope wherein the light source is of sufficient intensity to illuminate the uterus internally with the red light and project light outside of the uterus to permit observation

identifying at least one fibroid disposed on or in said uterine lining;

removing said at least one fibroid; and

confirming that the uterine lining is not perforated after the myomectomy is complete.

13. The method of claim 12 , wherein the red light is generated by a plurality of LEDs having a light output between about 2,500 lumen and 10,000 lumens.

14. The method of claim 12 , wherein the expandable anchor includes an inflatable body configured to be inflated by an inflation fluid.

The method of claim 12 , wherein the expandable anchor includes a triangular inflatable body configured to be inflated by an inflation fluid, wherein the triangular body includes a pair of distal tips configured to be oriented toward an entrance from the uterus into a respective Fallopian tube.

16. A method of performing a myomectomy on a uterus of a patient using a uterine manipulator including an elongate shaft, an expandable anchor, a cutting head attached to the distal end of the shaft, wherein the cutting head has a cup and a cutter coupled to the cup, and a light source disposed within the expandable anchor, the method comprising:

introducing the expandable anchor into the uterus;

expanding the expandable anchor within the uterus;

operating the light source to internally illuminate at least a portion of the uterus with red light having wavelength between about 630 nm and about 700 nm;

insufflating an abdominal cavity of the patient;

introducing an endoscope into the abdominal cavity of the patient;

engaging a cervix of the patient with the cutting head; and

operating the cutting head to separate the uterus from a vagina of the patient under direct visualization by way of the endoscope while the light source is illuminated, wherein operating the cutting head comprises advancing the cutter relative to the cup to deploy a distal portion of the cutter through the tissue of the vagina into an abdominal cavity of the patient, and rotating the cutter about a longitudinal axis of the uterine manipulator to separate the uterus from the vagina.

The method of claim 16 , wherein the uterine manipulator further comprises a stopper operably coupled to a sheath that surrounds the elongate shaft, wherein the sheath is operably coupled to the cup proximate a distal end of the sheath, and further wherein the stopper includes an actuator configured to clamp against the elongate shaft to hold the position of the sheath in place with respect to the elongate shaft, further wherein the method further comprises clamping the actuator against the shaft to maintain a predetermined longitudinal and rotational orientation of the expandable anchor with respect to the cutter as the cutter is advanced through tissue to separate the uterus from the vagina.

18. The method of claim 16 , wherein the red light is generated by a plurality of LEDs having a light output between about 2,500 lumen and 10,000 lumens.

19. The method of claim 16 , wherein the expandable anchor includes an inflatable body configured to be inflated by an inflation fluid.

20. The method of claim 16 , wherein the expandable anchor includes a triangular inflatable body configured to be inflated by an inflation fluid, wherein the triangular body includes a pair of distal tips configured to be oriented toward an entrance from the uterus into a respective Fallopian tube.
