Patient-specific guides for latarjet procedure
Patient-specific guides for the Latarjet procedure, as well as surgical systems and methods of performing the Latarjet procedure to treat glenohumeral instability using such patient-specific guides are disclosed. A patient-specific coracoid guide and a patient-specific glenoid guide may be configured based on preoperatively generated three-dimensional models of the patient's shoulder anatomy. Guides may be configured for coracoid graft preparation and glenoid decortication. The coracoid graft may be placed in the desired position based on three-dimensional (3D) preoperative planning.
1. A surgical assembly for performing a Latarjet procedure on a patient, the surgical assembly comprising:
a coracoid guide configured to conform to a coracoid process of the patient in a single orientation, the coracoid guide comprising:
a first cutting slot configured to receive a cutting element therethrough to guide removal of the coracoid process from a scapula of the patient;
a second cutting slot configured to receive the cutting element therethrough to guide preparation of a surface of the removed coracoid process; and
one or more first guide holes defined in the coracoid guide; and
a glenoid guide configured to conform to the glenoid of the patient in a single orientation, the glenoid guide comprising:
a third cutting slot configured to receive the cutting element therethrough to guide decortication of a margin of the glenoid; and
one or more second guide holes defined in the glenoid guide;
wherein the one or more first guide holes in the coracoid guide are adapted to guide the creation of holes in the coracoid process and further wherein the one or more second guide holes in the glenoid guide are adapted to guide the creation of holes in the glenoid which align with holes made in the coracoid process such as to facilitate attachment of the coracoid process to the glenoid using one or more fasteners; and
wherein the surface of the removed coracoid prepared using the coracoid guide is configured to correspond to the decorticated margin of the glenoid prepared using the glenoid guide.
2. The surgical assembly of claim 1 , wherein the first cutting slot is adapted to be positioned a distance from a distal end of the coracoid process at a base of the coracoid process anterior to ligaments of the coracoid process.
3. The surgical assembly of claim 1 , wherein the second cutting slot is configured to guide preparation of a medial surface of the removed coracoid process.
4. The surgical assembly of claim 1 , where the coracoid guide comprises a patient-specific coracoid geometry configured to correspond to the coracoid of the patient.
5. The surgical assembly of claim 1 , where the glenoid guide comprises a patient-specific glenoid geometry configured to correspond to the glenoid of the patient.
6. The surgical assembly of claim 1 , wherein the coracoid guide and the glenoid guide are adapted to prepare the coracoid process for attachment to the glenoid in a preoperatively planned position.
7. The surgical assembly of claim 1 , further comprising a drill guide clamp, the drill guide clamp comprising:
a first insert defining one or more drill guide holes;
a second insert defining one or more clearance slots; and
a clamp configured to hold the first and second inserts against the coracoid process.
8. A surgical assembly for performing a Latarjet procedure on a patient, the surgical assembly comprising:
a coracoid guide configured to conform to a coracoid process of the patient in a single orientation, the coracoid guide comprising:
a first cutting slot configured to receive a cutting element therethrough to guide removal of the coracoid process from a scapula of the patient;
a second cutting slot configured to receive the cutting element therethrough to guide preparation of a surface of the removed coracoid process; and
one or more first guide holes defined in the coracoid guide;
a glenoid guide configured to conform to the glenoid of the patient in a single orientation, the glenoid guide comprising:
a third cutting slot configured to receive the cutting element therethrough to guide decortication of a margin of the glenoid; and
a coracoid graft positioning guide configured to conform to the glenoid of the patient in a single orientation and to receive the removed coracoid process; the coracoid graft positioning guide comprising:
one or more second guide holes defined in the coracoid graft positioning guide;
wherein the one or more first guide holes in the coracoid guide are adapted to guide the creation of holes in the coracoid process and further wherein the one or more second guide holes in the coracoid graft positioning guide are adapted to guide the creation of holes in the glenoid which align with holes made in the coracoid process such as to facilitate attachment of the coracoid process to the glenoid using one or more fasteners; and
wherein the surface of the removed coracoid prepared using the coracoid guide is configured to correspond to the decorticated margin of the glenoid prepared using the glenoid guide.
9. The surgical assembly of claim 8 , wherein the first cutting slot is adapted to be positioned a distance from a distal end of the coracoid process at a base of the coracoid process, anterior to ligaments of the coracoid process.
10. The surgical assembly of claim 8 , wherein the second cutting slot is configured to guide preparation of a medial surface of the removed coracoid process.
11. The surgical assembly of claim 8 , where the coracoid guide comprises a patient-specific coracoid geometry configured to correspond to the coracoid of the patient.
12. The surgical assembly of claim 8 , where the glenoid guide comprises a patient-specific glenoid geometry configured to correspond to the glenoid of the patient.
13. The surgical assembly of claim 8 , where the coracoid graft positioning guide comprises a patient-specific glenoid geometry configured to correspond to the glenoid of the patient.
14. The surgical assembly of claim 8 , wherein the coracoid guide and the glenoid guide are adapted to prepare the coracoid process for attachment to the glenoid in a preoperatively planned position.
15. The surgical assembly of claim 8 , further comprising a drill guide clamp, the drill guide clamp comprising:
a first insert defining one or more drill guide holes;
a second insert defining one or more clearance slots; and
a clamp configured to hold the first and second inserts against the coracoid process.
