Abstract

Devices and methods for performing a surgical step or surgical procedure with visual guidance using an optical head mounted display are disclosed.

Claims (43)

1. A system of preparing a physical joint in a patient comprising:

at least one computer processor configured to generate a virtual surgical guide, wherein the virtual surgical guide is a virtual axis; and

a see-through optical head mounted display configured to display the virtual axis at a position, orientation or position and orientation onto a surface of the physical joint visible directly through the see-through optical head mounted display, so as to superimpose the virtual axis onto the surface of the physical joint,

wherein the position, orientation or position and orientation of the virtual axis determines a resection level, a varus or valgus angle, a flexion, a slope, or combination thereof for a placement of an implant component in relationship to one or more anatomic structures,

wherein the virtual axis is configured to allow superimposition and alignment between the virtual axis and at least one physical pin, physical drill or combination thereof,

wherein the at least one physical pin, physical drill or combination thereof is visible directly through the see-through optical head mounted display, and

wherein the system is configured to prepare the physical joint for one or more bone cuts or bone resections of the physical joint.

2. The system of claim 1 , wherein system comprises a robot, wherein the robot is configured to guide the at least one physical pin, physical drill or combination thereof.

3. The system of claim 2 , wherein the robot comprises a robotic arm.

The system of claim 1 , wherein the at least one physical pin, physical drill or combination thereof is configured to be advanced by a physical power tool, a physical power instrument, or wherein the at least one physical pin, physical drill or combination thereof is configured to be advanced through a physical drill guide, a physical cut block, or a combination thereof.

5. The system of claim 1 , wherein the virtual axis comprises at least one of a virtual pin, virtual drill, virtual predetermined path, virtual predetermined trajectory or combination thereof.

The system of claim 5 , wherein the virtual axis intersects with the surface of the physical joint and wherein the surface of the physical joint comprises an articular surface, a cartilage surface, a subchondral bone surface, a cortical bone surface, an opposing joint surface, a cut bone surface, a reamed bone surface, an altered bone surface, a skin, a subcutaneous tissue, a ligament, a ligament remnant, a meniscus, a labrum, an intra-articular structure or a combination thereof.

The system of claim 1 , wherein the system comprises two or more virtual axes, wherein the two or more virtual axes are configured to allow superimposition and alignment of two or more physical pins, physical drills or combinations thereof, wherein the two or more physical pins, physical drills or combinations thereof are configured for affixing a physical cut block to the physical joint or a physical bone, wherein the physical cut block comprises a slot configured for inserting a physical saw blade for executing a bone cut.

8. The system of claim 1 , wherein the virtual axis is configured to allow at least partial superimposition between the at least one physical pin, physical drill or combination thereof with the virtual axis.

The system of claim 1 , wherein the at least one computer processor is configured to maintain the display of the virtual axis at the position, orientation or position and orientation in relationship to the one or more anatomic structures when the at least one physical pin, physical drill or combination thereof moves in the field of view of a user.

10. The system of claim 1 , wherein the at least one computer processor is configured to maintain the display of the virtual axis at the position, orientation or position and orientation in relationship to the one or more anatomic structures when the physical joint of the patient moves.

The system of claim 1 , wherein the resection level, varus or valgus angle, flexion, slope or combination thereof comprises at least one predetermined start position, predetermined start point, predetermined intermediate position, predetermined intermediate point, predetermined end position, predetermined end point, or combination thereof, or wherein the resection level, varus or valgus angle, flexion, slope or combination thereof comprises at least one predetermined angle, predetermined start orientation, predetermined start alignment, predetermined intermediate orientation, predetermined intermediate alignment, predetermined end orientation, predetermined end alignment, or combination thereof.

The system of claim 1 , wherein the display of the virtual axis at the position, orientation or position and orientation determines a position, orientation, varus or valgus angle, flexion, rotation, or combination thereof of a femoral component or a position, orientation, varus or valgus angle, slope, rotation, or combination thereof of a tibial component in a knee joint replacement, or wherein the display of the virtual axis at the position, orientation or position and orientation determines a position, orientation, version or combination thereof of an acetabular component in a hip joint replacement, or wherein the display of the virtual axis at the position, orientation or position and orientation determines a position, orientation, version or combination thereof of a glenoid component in a shoulder joint replacement, or wherein the display of the virtual axis at the position, orientation or position and orientation determines a position, orientation, rotation, or combination thereof of a tibial or talar component in an ankle joint replacement.

The system of claim 2 , wherein the at least one physical pin, physical drill or combination thereof is configured to be advanced by a physical power tool, a physical power instrument, or wherein the at least one physical pin, physical drill or combination thereof is configured to be advanced through a physical drill guide, a physical cut block, or a combination thereof.

The system of claim 1 , wherein the computer processor is configured to obtain x, y, and z coordinates of the one or more anatomic structures, the physical joint, the see-through optical head mounted display, the physical pin, the physical drill or combinations thereof in a coordinate system and wherein the system comprises at least one infrared marker, an RF marker, an active marker, a passive marker, an IMU, an LED, an optical marker, a geometric pattern, a surgical navigation system, a camera, a video system, an image capture system, a depth sensor, a laser scanner, a 3D scanner, a patient specific marker, a patient specific template, an x-ray, an imaging system, or combination thereof configured for obtaining the x, y, and z coordinates.

The system of claim 1 , wherein the one or more anatomic structures comprise an articular surface, a cartilage surface, a subchondral bone surface, a cortical bone surface, an opposing joint surface, a cut bone surface, a reamed bone surface, an altered bone surface, a biomechanical axis, a mechanical axis or combination thereof, or wherein the surface of the physical joint comprises an articular surface, a cartilage surface, a subchondral bone surface, a cortical bone surface, an opposing joint surface, a cut bone surface, a reamed bone surface, an altered bone surface, a skin, a subcutaneous tissue, a ligament, a ligament remnant, a meniscus, a labrum, an intra-articular structure or a combination thereof.

16. A system of preparing a physical joint in a patient comprising:

at least one computer processor configured to generate two or more virtual surgical guides, wherein the virtual surgical guides are two or more virtual axes; and

a see-through optical head mounted display configured to display the two or more virtual axes at two or more positions, orientations or positions and orientations onto a surface of the physical joint visible directly through the see-through optical head mounted display, so as to superimpose the two or more virtual axes onto the surface of the physical joint,

wherein the two or more positions, orientations or positions and orientations of the two or more virtual axes are different,

wherein the two or more positions, orientations or positions and orientations of the two or more virtual axes determine a resection level, a varus or valgus angle, a flexion, a slope, or combination thereof for a placement of an implant component in relationship to one or more anatomic structures,

wherein the two or more virtual axes are configured for superimposition and alignment of two or more physical pins, physical drills or combinations thereof,

wherein the two or more physical pins, physical drills or combinations thereof are visible directly through the see-through optical head mounted display, and

wherein the two or more physical pins, physical drills or combinations thereof are configured for affixing a physical cut block to the physical joint or a physical bone, wherein the physical cut block comprises a slot configured for inserting a physical saw blade for executing a bone cut.

17. The system of claim 16 , wherein system comprises a robot, wherein the robot is configured to guide the two or more physical pins, physical drills or combinations thereof.

18. The system of claim 17 , wherein the robot comprises a robotic arm.

19. The system of claim 16 , wherein the two or more physical pins, physical drills or combinations thereof are configured to be advanced by a physical power tool, a physical power instrument, or wherein the two or more physical pins, physical drills or combinations thereof are configured to be advanced through a physical drill guide, a physical cut block, or a combination thereof.

20. The system of claim 16 , wherein the two or more virtual axes comprise at least one virtual pin, virtual drill, virtual predetermined path, virtual predetermined trajectory or combination thereof.

The system of claim 20 , wherein the two or more virtual axes intersect with the surface of the physical joint, and wherein the surface of the physical joint comprises an articular surface, a cartilage surface, a subchondral bone surface, a cortical bone surface, an opposing joint surface, a cut bone surface, a reamed bone surface, an altered bone surface, a skin, a subcutaneous tissue, a ligament, a ligament remnant, a meniscus, a labrum, an intra-articular structure or a combination thereof.

The system of claim 16 , wherein the at least one computer processor is configured to maintain the display of the two or more virtual axes at the two or more positions, orientations or positions and orientations in relationship to the one or more anatomic structures when at least one of the two or more physical pins, physical drills or combinations thereof move in the field of view of a user.

The system of claim 16 , wherein the at least one computer processor is configured to maintain the display of the two or more virtual axes at the two or more positions, orientations or positions and orientations in relationship to the one or more anatomic structures when the physical joint of the patient moves.

The system of claim 17 , wherein the two or more physical pins, physical drills or combinations thereof are configured to be advanced by a physical power tool, a physical power instrument, or wherein the two or more physical pins, physical drills or combinations thereof are configured to be advanced through a physical drill guide, a physical cut block, or a combination thereof.

The system of claim 16 , wherein the at least one computer processor is configured to maintain the display of the two or more virtual axes at the two or more positions, orientations or positions and orientations when the two or more physical pins, physical drills or combinations thereof are superimposed at least partially with the two or more virtual axes.

The system of claim 16 , wherein the resection level, varus or valgus angle, flexion, slope or combination thereof comprises at least one predetermined start position, predetermined start point, predetermined intermediate position, predetermined intermediate point, predetermined end position, predetermined end point, or combination thereof, or wherein the resection level, varus or valgus angle, flexion, slope or combination thereof comprises at least one predetermined angle, predetermined start orientation, predetermined start alignment, predetermined intermediate orientation, predetermined intermediate alignment, predetermined end orientation, predetermined end alignment, or combination thereof.

27. The system of claim 16 , wherein the display of the two or more virtual axes at the two or more positions, orientations or positions and orientations determines a position, orientation, varus or valgus angle, flexion, rotation, or combination thereof of a femoral component or a position, orientation, varus or valgus angle, slope, rotation, or combination thereof of a tibial component in a knee joint replacement.

The system of claim 16 , wherein the display of the two or more virtual axes at the two or more positions, orientations or positions and orientations determines a position, orientation, version or combination thereof of an acetabular component in a hip joint replacement, or wherein the display of the two or more virtual axes at the two or more positions, orientations or positions and orientations determines a position, orientation, version or combination thereof of a glenoid component in a shoulder joint replacement, or wherein the display of the two or more virtual axes at the two or more positions, orientations or positions and orientations determines a position, orientation, rotation, or combination thereof of a tibial or talar component in an ankle joint replacement.

The system of claim 16 , wherein the computer processor is configured to obtain x, y, and z coordinates of the one or more anatomic structures, the physical joint, the see-through optical head mounted display, the physical pin, the physical drill or combinations thereof in a coordinate system and wherein the system comprises at least one infrared marker, an RF marker, an active marker, a passive marker, an IMU, an LED, an optical marker, a geometric pattern, a surgical navigation system, a camera, a video system, an image capture system, a depth sensor, a laser scanner, a 3D scanner, a patient specific marker, a patient specific template, an x-ray, an imaging system, or combination thereof configured for obtaining the x, y, and z coordinates.

The system of claim 16 , wherein the one or more anatomic structures comprise an articular surface, a cartilage surface, a subchondral bone surface, a cortical bone surface, an opposing joint surface, a cut bone surface, a reamed bone surface, an altered bone surface, a biomechanical axis, a mechanical axis or combination thereof, or wherein the surface of the physical joint comprises an articular surface, a cartilage surface, a subchondral bone surface, a cortical bone surface, an opposing joint surface, a cut bone surface, a reamed bone surface, an altered bone surface, a skin, a subcutaneous tissue, a ligament, a ligament remnant, a meniscus, a labrum, an intra-articular structure or a combination thereof.
