Abstract

A percutaneous transluminal angioplasty device includes a catheter defining one or more lumens. A filter is coupled to the catheter adjacent a distal end of the catheter, and the filter is movable between an unexpanded and expanded configuration via a filter activation wire that extends through a lumen. An expandable balloon is coupled to the catheter proximally of the filter, and a stent is disposed over at least a portion of the balloon. To deploy the stent to a target site, the filter is first moved into its expanded position via the filter activation wire. Then, the stent is expanded, and the balloon is inflated to expand the stent further radially. The balloon is then deflated, the filter is contracted, and the catheter, balloon, and filter are removed from the body.

Claims (28)

1. A method of performing an angioplasty comprising:

advancing a catheter to a site of a vascular stenosis;

disposing an integrated filter assembly within a blood vessel downstream of the vascular stenosis;

disposing a stent radially inward of the vascular stenosis;

tensioning an activation wire using a first actuation knob on a proximal handle, thereby deploying the filter assembly;

tensioning a sheath wire using a second actuation knob on the proximal handle, thereby moving a sheath proximally to expose the stent,

allowing the stent to expand against the vascular stenosis;

post-dilatating the stent using a balloon;

deflating the balloon;

contracting the filter assembly; and

removing the catheter from the body.

2. The method of claim 1 , further comprising tensioning the sheath wire using the second actuation knob to expose the filter assembly prior to deploying the filter assembly.

3. The method of claim 1 , wherein a filter frame of the filter assembly has a shape memory of a contracted configuration, and tensioning the activation wire pulls the filter frame into an expanded configuration.

4. The method of claim 3 , further comprising pulling the filter frame into an expanded configuration having a diameter that corresponds to an inner diameter of the blood vessel.

5. The method of claim 1 , wherein tensioning an activation wire comprises translating the activation wire within an activation wire lumen of the catheter.

6. The method of claim 1 , wherein a distal end of the filter activation wire is coupled to a distal portion of a filter frame of the filter assembly, and wherein tensioning the activation wire comprises sliding the distal portion of the filter frame toward a proximal portion of the filter frame.

The method of claim 6 , wherein a proximal end of a filter membrane is coupled to the filter frame, a distal end of the filter membrane is coupled to the catheter distal to a distal end of the filter frame, and the distal portion of the filter frame slides proximally relative to the distal end of the filter membrane.

8. The method of claim 1 , further comprising releasing the tension on the activation wire, thereby allowing the filter to self-contract.

9. The method of claim 1 , wherein tensioning a sheath wire comprises translating the sheath wire proximally within a sheath wire lumen of the catheter.

10. The method of claim 1 , wherein the stent is self-expanding.

11. The method of claim 10 , further comprising restraining the stent with the sheath prior to moving the sheath proximally to expose the stent.

12. The method of claim 1 , wherein exposing the stent comprises actuating a stent deployment wire.

13. The method of claim 1 , further comprising repeating the step of post-dilatating the stent.

14. The method of claim 1 , further comprising moving a fluid through a balloon inflation lumen defined by the catheter.

15. The method of claim 1 , further comprising capturing embolic particles within the filter assembly.

16. The method of claim 15 , wherein embolic particles are captured within a filter membrane of the filter assembly.

17. The method of claim 1 , wherein the vascular stenosis is in a carotid artery.

18. The method of claim 1 , further comprising moving a guidewire through a guidewire port defined by an external side surface of the catheter and moving the guidewire within a guidewire lumen defined by a distal portion of the catheter.
