Abstract

Various exemplary devices and methods are provided for performing surgical procedures. In general, one or more adjuncts can be used in conjunction with surgical instruments. The adjunct(s) can have medicant(s) thereon and/or therein. The medicant(s) can vary depending on the desired effect of the medicant(s) on surrounding tissue. As a non-limiting example, medicant(s) can be provided to influence hemostasis, inflammation, macrophages, and/or fibroblasts. When used in conjunction with a surgical stapler, the adjunct(s) can be disposed between and/or on jaws of the stapler, incorporated into a staple cartridge disposed in the jaws, or otherwise placed in proximity to the staples. When staples are deployed, the adjunct(s) can remain at the treatment site with the staples.

Claims (38)

1. A surgical system, comprising:

an adjunct configured to be released from an end effector of a surgical stapler and stapled to tissue of a patient, the adjunct including an upper outer wall, a lower outer wall, and a plurality of heterogeneous fiber lattice sections, wherein

each of the fiber lattice sections is formed from a plurality of fibers that extend between and interconnect the upper outer wall and the lower outer wall,

each of the upper and lower outer walls are formed from a synthetic material, and

each adjacent pair of the fiber lattice sections defines a reservoir therebetween such that the adjunct includes one or more reservoirs; and

a hemostatic agent configured to promote hemostasis, wherein

the hemostatic agent is disposed in each of the one or more reservoirs, and

the upper and lower outer walls are configured to degrade in the patient from a first state, in which the upper and lower outer walls prevent the hemostatic agent from being released from the one or more reservoirs, to a second state, in which the upper and lower outer walls allow the hemostatic agent to be released from the one or more reservoirs and into contact with the tissue.

2. The system of claim 1 , wherein the hemostatic agent includes cellulose.

3. The system of claim 1 , wherein the hemostatic agent includes at least one of fibrin, thrombin, autologous platelet plasma, gelatin, hyaluronic acid, and a polymeric sealant.

The system of claim 1 , wherein each of the one or more reservoirs has an upper side defined by the upper outer layer, a lower side defined by the lower outer layer, a first side defined by one of the adjacent pair of the fiber lattice sections, and a second, opposite side defined by the other one of the adjacent pair of the fiber lattice sections.

The system of claim 1 , wherein the adjunct is configured to compressible from an expanded configuration, in which the plurality of fibers are not compressed and the adjunct has a first height, to a collapsed configuration, in which the plurality of fibers are compressed and the adjunct has a second, smaller height.

6. The system of claim 5 , wherein each of the plurality of fibers has a first terminal end attached to the upper outer wall and a second terminal end attached to the lower outer wall.

7. The system of claim 5 , wherein in each of the fiber lattice sections, a subset of the plurality of fibers extend in a first direction and a reminder of the plurality of fibers in the fiber lattice section extend in a second direction that is transverse to the first direction.

8. The system of claim 1 , further comprising a cartridge body with a plurality of staple cavities on a tissue-facing surface thereof, each staple cavity having a staple disposed therein that is configured to be deployed into tissue;

wherein the upper outer wall or the lower outer wall of the adjunct is disposed on the tissue-facing surface of the cartridge body.

9. The system of claim 8 , further comprising the end effector;

wherein the end effector includes a jaw, and the cartridge body is configured to be seated in the jaw.

10. The system of claim 1 , further comprising an anvil with a plurality of staple forming cavities formed on a tissue-facing surface thereof;

wherein the upper outer wall or the lower outer wall of the adjunct is disposed on the tissue-facing surface of the anvil.

11. A surgical system, comprising:

a first jaw having a cartridge with a plurality of staple cavities configured to seat staples therein, the staple cavities opening on a tissue-facing surface of the cartridge;

a second jaw having an anvil with a plurality of staple-forming cavities formed on a tissue-facing surface of the anvil, the first and second jaws being configured to clamp tissue therebetween;

an adjunct configured to be stapled to tissue with the staples, wherein

the adjunct includes an upper outer wall, a lower outer wall, and a plurality of heterogeneous fiber lattice sections,

each of the fiber lattice sections is formed from a plurality of fibers that extend between and interconnect the upper outer wall and the lower outer wall,

each of the upper and lower outer walls are formed from a synthetic material, and

each adjacent pair of the fiber lattice sections defines a reservoir therebetween such that the adjunct includes one or more reservoirs, and

the upper outer wall or the lower outer wall of the adjunct is releasably attached to the tissue-facing surface of one of the cartridge and the anvil; and

a hemostatic agent configured to promote hemostasis, wherein

the hemostatic agent is disposed in each of the one or more reservoirs, and

the upper and lower outer walls are configured to degrade in the patient from a first state, in which the upper and lower outer walls prevent the hemostatic agent from being released from the one or more reservoirs, to a second state, in which the upper and lower outer walls allow the hemostatic agent to be released from the one or more reservoirs and into contact with the tissue.

12. The system of claim 11 , wherein the hemostatic agent includes cellulose.

13. The system of claim 11 , wherein the hemostatic agent includes at least one of fibrin, thrombin, autologous platelet plasma, gelatin, hyaluronic acid, and a polymeric sealant.

The system of claim 11 , wherein each of the one or more reservoirs has an upper side defined by the upper outer layer, a lower side defined by the lower outer layer, a first side defined by one of the adjacent pair of the fiber lattice sections, and a second, opposite side defined by the other one of the adjacent pair of the fiber lattice sections.

The system of claim 11 , wherein the adjunct is configured to compressible from an expanded configuration, in which the plurality of fibers are not compressed and the adjunct has a first height, to a collapsed configuration, in which the plurality of fibers are compressed and the adjunct has a second, smaller height.

16. The system of claim 15 , wherein each of the plurality of fibers has a first terminal end attached to the upper outer wall and a second terminal end attached to the lower outer wall.

17. The system of claim 15 , wherein in each of the fiber lattice sections, a subset of the plurality of fibers extend in a first direction and a reminder of the plurality of fibers in the fiber lattice section extend in a second direction that is transverse to the first direction.
