Abstract

Described here are methods and systems for the manipulation of ovarian tissues. The methods and systems may be used in the treatment of polycystic ovary syndrome (PCOS). The systems and methods may be useful in the treatment of infertility associated with PCOS.

Claims (26)

1. A system for performing an ovarian procedure, the system comprising:

a docking device having a proximal region comprising a deployment feature, a distal region configured to penetrate an ovary, an elongated body having a lumen, and an alignment element at the proximal region;

a therapeutic element slidably disposed within the lumen of the docking device and configured to be deployed from the lumen of the docking device within the ovary upon actuation of the deployment feature, the therapeutic element comprising an electrode configured to emit radiofrequency energy to ablate ovarian tissue from within the ovary;

a sensor disposed on the therapeutic element and configured to measure temperature of the ovarian tissue;

an adapter configured to be removably coupled to an ultrasound probe, the adapter comprising a receiving alignment channel having a geometry that allows engagement with the alignment element in a manner that maintains planar orientation and limits rotation of the docking device and therapeutic element relative to the ultrasound probe; and

a computing device having a processor configured to:

cause the electrode to emit radiofrequency energy within the ovary up to a maximum power of 10 Watts to achieve up to a maximum temperature of 100° C.; and

execute a feedback control loop to dynamically modify a parameter of energy emission if the measured temperature received from the sensor exceeds the maximum temperature.

2. The system of claim 1 , wherein the therapeutic element comprises a second electrode, and wherein the sensor is disposed between the electrode and the second electrode.

3. The system of claim 1 , wherein the sensor is disposed on the electrode.

4. The system of claim 1 , wherein the sensor is configured to measure temperature at the electrode during an ablation procedure.

5. The system of claim 1 , wherein the sensor is configured to measure tissue temperature during an ablation procedure.

6. The system of claim 1 , further comprising an impedance sensor configured to measure tissue impedance during an ablation procedure.

7. The system of claim 1 , wherein the distal region of the docking device comprises a needle tip configured to penetrate the ovary at a single entry point.

8. The system of claim 7 , wherein the docking device is configured to be reoriented within the ovary through the single entry point.

9. The system of claim 1 , wherein the therapeutic element forms a curved configuration upon deployment from the lumen of the docking device within the ovary.

10. The system of claim 9 , wherein the curved configuration comprises a radius of curvature ranging between about 3.0 mm and about 10 mm.

11. The system of claim 1 , wherein the parameter of energy emission comprises at least one of energy emission, power, voltage, current, intensity, frequency, pulse, pulse width, temperature, type of energy emission, flow rate, or pressure.

12. The system of claim 1 , wherein the feedback control loop is configured to terminate energy emission if the measured temperature exceeds the maximum temperature.

13. The system of claim 1 , wherein the processor is configured to receive user input comprising at least one of duration of energy emission, power, target temperature, or mode of operation.

14. The system of claim 1 , further comprising a generator comprising the computing device, the generator configured to supply radiofrequency energy to the electrode.

The system of claim 14 , wherein the computing device is programmed to cause the generator to supply radiofrequency energy to the electrode to cause the electrode to emit radiofrequency energy to ablate 3% to 20% of the ovarian tissue to treat infertility associated with polycystic ovary syndrome.

16. The system of claim 14 , wherein the computing device is programmed to cause the generator to supply radiofrequency energy to the electrode to cause the electrode to emit radiofrequency energy to ablate ovarian tissue from within the ovary for 20 to 60 seconds.

17. The system of claim 1 , wherein the distal region of the docking device comprises an echogenic material.

18. The system of claim 1 , wherein the distal region of the docking device is configured to be introduced transvaginally to penetrate a vaginal wall and the ovary.

19. The system of claim 1 , wherein the therapeutic element remains in a visualization plane of the ultrasound probe upon deployment from the lumen of the docking device during an ablation procedure.
