Abstract

An instrument and method for tissue thermotherapy including an inductive heating means to generate a vapor phase media that is used for interstitial, intraluminal, intracavity or topical tissue treatment. In one method, the vapor phase media is propagated from a probe outlet to provide a controlled vapor-to-liquid phase change in an interface with tissue to thereby apply ablative thermal energy delivery.

Claims (33)

1. A method for altering a function of a wall of a duodenum of a digestive tract of an individual, the method comprising:

advancing a working end of a catheter device into the duodenum using a trans-esophageal approach, where the catheter device is configured deliver a flow media

converting the flow media to a heated flow media using a heating mechanism remote from the working end;

causing an inflow the heated flow media to the working end using a pressure source through a first flow channel to deliver a controlled thermal energy to a wall of the duodenum in a plurality of adjacent sections to produce a plurality of thin layers of ablated tissue in adjacent sections of the wall of the duodenum such that plurality of thin layers of ablated tissue alter the function of the digestive tract;

causing an outflow of the heated flow media through a second flow channel extending through the catheter device from the working end; and

controlling the heating mechanism and the pressure source using a controller to control heating of the heated flow media and inflow of the heated flow media to the working end.

2. The method of claim 1 , further comprising inserting a scope into a stomach of the individual.

3. The method of claim 1 , further comprising delivering a cooling fluid to an expandable member located on the catheter device to cool a surface of the wall after delivering the controlled thermal energy to the wall.

4. The method of claim 1 , wherein the catheter device is configured to apply a heated flowable media to produce the controlled thermal energy.

5. The method of claim 4 , where a temperature of the heated flowable media is at least 60 degrees Celsius.

6. The method of claim 1 , wherein applying the controlled thermal energy occurs over a time interval ranging from 1 second to 30 seconds.

7. The method of claim 1 , further comprising applying the controlled thermal energy sequentially to at least one adjacent region of the wall in the duodenum.

8. The method of claim 1 , wherein a source of the flow media is provided is remote from the working end and where a heating mechanism is coupled to an inflow channel in the catheter device.

9. The method of claim 1 , wherein the catheter device is configured to circulate a heated flowable media within the catheter device.

10. A method for treating a targeted site in a lumen of a small intestine of a patient, the method comprising:

manipulating a working end of an elongated catheter to a target site in the lumen of the small intestine via a trans-esophageal approach;

positioning an energy applicator portion of the working end of the elongated catheter at the target site;

engaging at least one balloon coupled to the working end against a wall of the small intestine;

heating a flow media using an energy delivery source to convert the flow media to a heated flow media;

producing pressure from a pressure source to cause a flow of the heated flow media through a first flow channel in the elongated catheter to the energy applicator portion;

allowing an outflow of the heated flow media from the energy applicator portion through a second flow channel extending through the elongated catheter; and

using a controller to control the energy delivery source and the pressure source to control the flow of the heated flow media at a selected temperature such that the heated flow media delivered through the energy applicator portion applies thermal energy to the wall of the small intestine at the targeted site to ablate a thin layer of the wall causing an alteration of hormonal function of the patient.

11. The method of claim 10 , wherein the thermal energy is applied to the wall of the lumen for less than 120 seconds.

12. The method of claim 10 , wherein the thermal energy is applied to the wall of the lumen for 1 to 30 seconds.

13. The method of claim 10 , where the flow media has a temperature of at least 60 degrees Celsius.

14. The method of claim 10 , where the flow media has a temperature of at least 70 degrees Celsius.

15. The method of claim 10 , where the flow media has a temperature of at least 80 degrees Celsius.

16. The method of claim 10 , where the flow media has a temperature of at least 90 degrees Celsius.

17. The method of claim 10 , further comprising a temperature sensor in the working end operatively coupled to the controller for sensing a temperature of the flow media.

18. The method of claim 17 , further comprising a temperature sensor in the working end adapted to send signals of the temperature of the flow media to the controller.

19. The method of claim 18 , wherein the controller is configured to control the heating mechanism in response to the signals from the temperature sensor.

20. The method of claim 10 , wherein the controller is configured to control a cooling mechanism for cooling a flow media and to control a pressure mechanism for providing an inflow of cooled flow media through a flow channel to the working end.

21. The method of claim 10 , wherein the controller is configured to control a pressure mechanism for inflating at least one balloon carried by the working end.
