Abstract

Described are methods of treating urinary incontinence using a urinary sling and a tissue path that passes through the obturator foramen, along with related surgical implants, devices, systems and kits.

Claims (51)

1. A method, comprising:

forming an elongate implantable sling having a central support portion and elongate end portions sized and shaped to extend between an incision substantially adjacent to a patient's obturator foramen, through the obturator foramen, and to a location below a urethra,

creating a medial incision at the perineum,

exposing bulbospongiosus muscle,

dissecting bulbospongiosus muscle to expose corpus spongiosum, and

placing the central support portion to contact the corpus spongiosum.

2. The method of claim 1 , further comprising:

creating a pair of lateral incisions substantially adjacent a patient's left and right obturator foramen, and

extending the end portions internally through the obturator foramen to the lateral incisions substantially adjacent to the obturator foramen.

3. The method of claim 2 , further comprising:

placing tension on the end portions of the sling to approximate pelvic tissue.

4. The method of claim 3 , further comprising:

approximating the urethra in a proximal direction.

5. The method of claim 3 , further comprising:

approximating the urethra in a proximal direction by a distance in the range from 1 to 5 centimeters.

6. The method of claim 1 , wherein the implant comprises a widened central support portion, and the method comprises placing the widened central support portion below the corpus spongiosum.

7. The method of claim 6 , further comprising:

securing the widened central support portion to the corpus spongiosum.

8. The method of claim 6 , further comprising:

securing the widened central support portion to the corpus spongiosum by use of a suture.

9. The method of claim 1 , further comprising:

approximating the urethra without placing compressive forces on the urethra.

10. The method of claim 1 , further comprising:

placing the central support portion in contact with the corpus spongiosum at a location below a membranous urethra.

11. A method, comprising:

creating a pair of lateral incisions substantially adjacent a patient's left and right obturator foramen,

creating a medial incision at the perineum,

dissecting bulbospongiosus muscle to expose corpus spongiosum,

placing a central support portion of an implant proximate the corpus spongiosum, and

extending end portions of the implant internally through the obturator foramen to the lateral incisions substantially adjacent to the obturator foramen.

12. The method of claim 11 , further comprising:

exposing the bulbospongiosus muscle.

13. The method of claim 11 , wherein the central support portion of the implant is wider than each of the end portions of the implant.

14. A method, comprising:

providing an elongate implantable sling having a central support portion and elongate end portions,

creating a medial incision at the perineum,

exposing bulbospongiosus muscle,

dissecting bulbospongiosus muscle to expose corpus spongiosum, and

placing the central support portion to contact the corpus spongiosum.

15. The method of claim 14 , further comprising:

creating a pair of lateral incisions substantially adjacent a patient's left and right obturator foramen, and

extending the end portions internally through the obturator foramen to the lateral incisions substantially adjacent to the obturator foramen.

16. The method of claim 15 , further comprising:

placing tension on the end portions of the sling to approximate pelvic tissue.

17. The method of claim 16 , further comprising:

approximating a urethra in a proximal direction.

18. The method of claim 16 , further comprising:

approximating a urethra in a proximal direction by a distance in the range from 1 to 5 centimeters.

19. The method of claim 14 , wherein the implant comprises a widened central support portion, and the method comprises placing the widened central support portion below the corpus spongiosum.

20. The method of claim 19 , further comprising:

securing the widened central support portion to the corpus spongiosum.
