Abstract

Restless Leg Syndrome (RLS) or Periodic Limb Movement Disorder (PLMD) can be treated using high frequency (HF) electrostimulation. This can include selecting or receiving a subject presenting with RLS or PLMD. At least one electrostimulation electrode can be located at a location associated with at least one of, or at least one branch of, a sural nerve, a peroneal nerve, or a femoral nerve. HF electrostimulation can be delivered to the subject, which can include delivering subsensory, subthreshold, AC electrostimulation at a frequency that exceeds 500 Hz and is less than 15,000 Hz to the location to help reduce or alleviate the one or more symptoms associated with RLS or PLMD. A charge-balanced controlled-current HF electrostimulation waveform can be used.

Claims (37)

1. A method for performing neurostimulation therapy for a Restless Legs Syndrome (RLS) or Periodic Limb Movement Disorder (PLMD) patient, the method comprising:

determining, based on a patient RLS or PLMD efficacy indicator, at least one electrostimulation signal parameter for treating one or more symptoms associated with RLS or PLMD using electrostimulation; and

delivering at least one a high-frequency (HF) electrostimulation signal at a frequency between 500 Hz and 15,000 Hz using the determined at least one electrostimulation signal parameter based on the patient RLS or PLMD efficacy indicator.

2. The method of claim 1 , wherein the delivering the at least one HF electrostimulation signal includes bilaterally delivering to each one of both legs of the patient.

3. The method of claim 1 , wherein the patient RLS or PLMD efficacy indicator includes a physiological response associated with the neurostimulation therapy.

The method of claim 3 , wherein determining the electrostimulation signal parameter includes determining a threshold electrostimulation signal parameter by gradually increasing an electrostimulation signal energy of the electrostimulation signal until obtaining an indication of at least one of 1) the electrostimulation signal affecting an ability of the patient to at least one of fall asleep or stay asleep or 2) the electrostimulation signal causing muscle activity of the patient.

5. The method of claim 3 , wherein determining the electrostimulation signal parameter includes determining a threshold electrostimulation signal parameter based on a flexion response of the patient or a compound action potential (CAP) response of the patient.

6. The method of claim 3 , comprising determining a drug therapy parameter based on at least one of an electrostimulation parameter or a physiological response associated with the neurostimulation therapy.

7. The method of claim 6 , wherein the drug therapy parameter includes a titration parameter of titration of a drug based on at least one of an electrostimulation parameter or the physiological response associated with the neurostimulation therapy.

8. The method of claim 6 , wherein the physiological response includes at least one of a flexion response or a compound action potential (CAP) response of the patient.

9. The method of claim 1 , wherein the patient RLS or PLMD efficacy indicator includes a physiological response in a patient receiving a drug therapy.

10. The method of claim 9 , wherein determining the electrostimulation signal parameter includes:

monitoring a desired physiological response associated with the drug therapy; and

establishing or adjusting an electrostimulation signal energy based on an indication of the drug therapy being downwardly adjusted.

The method of claim 9 , wherein determining the electrostimulation signal parameter includes establishing or adjusting an electrostimulation parameter of the electrostimulation signal to compensate for a change in a physiological response associated with the drug therapy being downwardly adjusted.

12. The method of claim 1 , comprising communicating information about at least one of the delivered electrostimulation, electrostimulation efficacy, or a symptom associated with at least one of RLS or PLMD to a local or remote external device.

13. The method of claim 12 , wherein the external device includes at least one of a neural network, an artificial intelligence model, or machine learning system configured to use efficacy data to determine the electrostimulation signal parameter.

14. The method of claim 1 , wherein delivering a high-frequency (HF) electrostimulation signal includes delivering a controlled current electrostimulation signal at a frequency that is between 4 kHz and 5 kHz.

15. A neurostimulation therapy system for treating a patient having one or more symptoms associated with at least one of Restless Legs Syndrome (RLS) and Periodic Limb Movement Disorder PLMD) using applied high-frequency electrostimulation, the system comprising:

a wearable device including an electrostimulation electronics unit including or coupled to first and second skin electrodes for delivering an electrostimulation therapy signal to the patient at an external target body location, wherein the electrostimulation therapy signal is delivered at a frequency of between 500 Hz and 15,000 Hz and at a current of between 5 mA and 50 mA; and

controller circuitry, included in or coupled to the electronics unit, the controller circuitry configured to establish or adjust an electrostimulation signal parameter based on a patient RLS or PLMD efficacy indicator.

The system of claim 15 , wherein the controller circuitry is configured to determine a threshold electrostimulation signal parameter by gradually increasing electrostimulation signal energy until receiving an indication of at least one of 1) the electrostimulation signal affecting an ability of the patient to at least one of fall asleep or stay asleep or 2) the electrostimulation signal causing muscle activity of the patient.

17. The system of claim 15 , comprising:

transceiver circuitry, configured to receive information about a desired physiological response associated with a drug therapy;

wherein the controller circuitry is configured to gradually increase an electrostimulation signal energy based on information about the desired physiological response due to downwardly adjusting the drug therapy.

18. The system of claim 15 , comprising:

transceiver circuitry, configured to communicate:

information about a desired physiological response associated with a drug therapy; and

information about a desired physiological response associated with a neurostimulation therapy;

wherein the controller circuitry is configured to adjust an electrostimulation parameter of the electrostimulation signal to compensate for a change in a physiological response associated with the drug therapy being downwardly adjusted.

19. The system of claim 15 , comprising at least one of a neural network, an artificial intelligence model, or a machine learning system configured to:

receive efficacy data;

determine, using the efficacy data, a desired electrostimulation signal parameter; and

communicate the desired electrostimulation signal parameter with the controller circuitry.

20. A method for performing neurostimulation therapy for a Restless Legs Syndrome (RLS) or Periodic Limb Movement Disorder (PLMD) patient, the method comprising:

determining, based on a patient RLS or PLMD efficacy indicator, at least one electrostimulation signal parameter for treating one or more symptoms associated with RLS or PLMD using electrostimulation, wherein the patient electrostimulation RLS or PLMD efficacy indicator includes at least one of a muscle activity of the patient, a flexion response of the patient, or a decrease in a RLS or PLMD symptom as measured in a patient using a SIT or other provocative test; and

delivering at least one a high-frequency (HF) electrostimulation signal at a frequency between 500 Hz and 15,000 Hz using the determined at least one electrostimulation signal parameter based on the patient RLS or PLMD efficacy indicator.
