ENDOVASCULAR STENT-GRAFT WITH AN EXTRAVASCULAR EXTENSION
An endovascular stent-graft with an extravascular extension is presented including: a stent-graft having an aperture and a vascular graft having an and attached to the aperture and a free end. Methods of using the endovascular stent-graft with an extravascular extension include providing the endovascular stent-graft with an extravascular extension to a target vessel of the patient and using free end to either provide access for a circulatory medical procedure or attaching the free end to another point in the circulatory system to perform a bypass operation or to provide a graft for providing hemodialysis. The methods may be performed noninvasively through incisions in the back of the patient.
1 . A stent-graft with branching graft comprising:
a stent-graft having a first end, a second end, and an aperture through the stent-graft between the first end and the second end, and
a vascular graft having a first end and a second end, where the first end of the vascular graft is attached to the aperture of the stent-graft and where the second end of the vascular graft away from the aperture of the stent-graft,
where a lumen of the stent-graft with branching graft includes a lumen of the sent-graft and a lumen of the vascular graft.
2 . The stent-graft with branching graft of claim 1 , where the aperture through the stent-graft is midway between the first end of the stent-graft and the second end of the stent-graft.
3 . The stent-graft with branching graft of claim 1 , where the aperture through the stent-graft is closer to the first end of the stent-graft than to a location midway between the first end of the stent-graft and the second end of the stent-graft.
4 . The stent-graft with branching graft of claim 1 where the first end of the vascular graft is sutured to the aperture of the stent-graft.
5 . The stent-graft with branching graft of claim 1 , where the stent-graft is sized to fit within an artery or a vein.
6 . The stent-graft with branching graft of claim 1 , where the diameter of the stent-graft is from 1 to 5 cm.
7 . The stent-graft with branching graft of claim 1 , where the length of the stent-graft is from 15 to 40 cm.
8 . The stent-graft with branching graft of claim 1 , where the diameter of the vascular graft is from 5 to 10 cm.
9 . The stent-graft with branching graft of claim 1 , where the length of the vascular graft is from 3 to 70 cm.
10 . The stent-graft with branching graft of claim 1 , further comprising a sealing assembly including an inflatable balloon attached to and extending circumferentially around the vascular graft near the aperture of the stent-graft.
11 . The stent-graft with branching graft of claim 10 , where when the balloon is inflated, the balloon contacts an outer surface of the stent-graft.

21 . The stent-graft with branching graft of claim 1 , where the stent includes a metal framework or mesh and a stent-graft graft.
22 . The stent-graft with branching graft of claim 21 , where the metal framework or mesh includes nitinol.
23 . The stent-graft with branching graft of claim 21 , where the stent-graft graft includes expanded polytetrafluoroethylene.
24 . The stent-graft with branching graft of claim 21 , where the graft includes expanded polytetrafluoroethylene.
25 . The stent-graft with branching graft of claim 1 , where the first end of the vascular graft is attached to the aperture of the stent graft with medical suture.
26 . The stent-graft with branching graft of claim 1 , where the first end of the vascular graft is bonded to the aperture of the stent graft with medical suture.
27 . The stent-graft with branching graft of claim 1 , where the vascular graft and the stent graft are formed as a single piece in the manufacturing process of the stent-graft with branching graft.
