Abstract

A system for restoring muscle function to the lumbar spine to treat low back pain is provided. The system may include electrodes coupled to an implantable pulse generator (IPG), a handheld activator configured to transfer a stimulation command to the IPG, and an external programmer configured to transfer programming data to the IPG. The stimulation command directs the programmable controller to stimulate the tissue in accordance with the programming data. The system may include a software-based programming system run on a computer such that the treating physician may program and adjust stimulation parameters.

Claims (43)

1. A system for alleviating a patient's back pain, the system comprising:

one or more electrodes configured to be positioned within the patient to deliver electrical stimulation to a medial branch of a dorsal ramus nerve that innervates a multifidus muscle which is associated with local segmental control of a lumbar spine within a back of a patient;

an electrode lead comprising an anchor at a distal end of the electrode lead, the anchor configured to anchor the one or more electrodes within the patient for delivering the electrical stimulation to the medial branch of the dorsal ramus nerve that innervates the multifidus muscle, wherein the anchor is configured to be angled relative to the electrode lead when implanted to anchor the electrode lead in place; and

a pulse generator configured to be coupled to the one or more electrodes, the pulse generator configured to be programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve via the one or more electrodes to improve neuromuscular control system performance to facilitate rehabilitation, thereby alleviating the patient's back pain.

2. The system of claim 1 , wherein the pulse generator is configured to be programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve via the one or more electrodes to cause the multifidus muscle to contract.

3. The system of claim 2 , wherein the multifidus muscle contraction caused by the electrical stimulation over time facilitates the improvement of neuromuscular control system performance.

4. The system of claim 1 , wherein the pulse generator is programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve at a frequency selected from 1-30 Hz.

5. The system of claim 1 , wherein the pulse generator is programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve at a pulse width selected from 20-500 μs.

6. The system of claim 1 , wherein the pulse generator is programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve at a pulse amplitude selected from 0.1-7 mA.

7. The system of claim 1 , wherein the pulse generator is programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve at a frequency selected from 1-30 Hz, at a pulse width selected from 20-500 μs, and at a pulse amplitude selected from 0.1-7 mA.

8. The system of claim 1 , wherein the pulse generator is programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve at a frequency selected from 12-30 Hz.

9. The system of claim 1 , wherein the pulse generator is programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve at an amplitude selected from less than 50 mA.

10. The system of claim 1 , wherein the one or more electrodes are configured to be positioned to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve that exits between L2 and L3 lumbar segments.

11. The system of claim 1 , wherein the neuromuscular control system performance is improved to restore function of the multifidus muscle to facilitate the rehabilitation.

12. The system of claim 1 , wherein the pulse generator is programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve at stimulation parameters titrated for the patient.

13. The system of claim 12 , wherein the stimulation parameters comprises at least one of: timing, magnitude, or duration of treatment.

14. The system of claim 12 , wherein the stimulation parameters comprises timing, magnitude, and duration of treatment.

15. The system of claim 1 , wherein the pulse generator is configured to be implanted.

16. The system of claim 1 , wherein the pulse generator is configured to be reprogrammed during a treatment period to facilitate rehabilitation of the multifidus muscle.

17. The system of claim 1 , wherein the electrode lead is configured to electrically couple the pulse generator to the one or more electrodes.

The system of claim 1 , wherein the electrode lead further comprises a second anchor proximal to the anchor, the second anchor configured to be angled in an opposite manner to the anchor when implanted, the anchor and the second anchor configured to anchor the electrode lead in a position for stimulating the medial branch of the dorsal ramus nerve.

19. The system of claim 1 , further comprising:

an external programmer configured to transmit programing data to the pulse generator,

wherein the pulse generator is programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve consistent with the programming data.

20. The system of claim 19 , wherein the programming data comprises at least one of: pulse amplitude, pulse width, stimulation rate, stimulation frequency, ramp timing, cycle timing, session timing, or electrode configuration.

21. The system of claim 1 , further comprising:

an activator configured to transmit a stimulation command to the pulse generator,

wherein the pulse generator is programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve responsive to the stimulation command.

22. The system of claim 21 , wherein the stimulation command comprises at least one of: a command to start a treatment session or stop the treatment session; a command to provide a status of the pulse generator; or a request to conduct an impedance assessment.

23. The system of claim 1 , further comprising one or more sensors configured to be operatively coupled to the pulse generator, the one or more sensors configured to sense muscle contraction.

24. A system for alleviating a patient's back pain, the system comprising:

a lead comprising one or more electrodes, the one or more electrodes configured to be positioned within the patient to deliver electrical stimulation to a medial branch of a dorsal ramus nerve that innervates a multifidus muscle which is associated with local segmental control of a lumbar spine within a back of a patient;

an anchor configured to anchor the one or more electrodes within the patient for delivering the electrical stimulation to the medial branch of the dorsal ramus nerve that innervates the multifidus muscle, wherein the anchor is configured to be angled relative to the lead when implanted to anchor the lead in place; and

a pulse generator configured to be coupled to the one or more electrodes, the pulse generator configured to be programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve via the one or more electrodes to improve neuromuscular control system performance, thereby alleviating the patient's back pain.

25. The system of claim 24 , wherein the pulse generator is programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve at a frequency selected from 12-30 Hz.

26. The system of claim 24 , wherein the pulse generator is configured to be programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve via the one or more electrodes to cause the multifidus muscle to contract.

27. The system of claim 26 , wherein the multifidus muscle contraction caused by the electrical stimulation over time facilitates the improvement of neuromuscular control system performance.

28. A system for alleviating a patient's back pain, the system comprising:

one or more electrodes configured to be positioned within the patient to deliver electrical stimulation to a medial branch of a dorsal ramus nerve that innervates a multifidus muscle which is associated with local segmental control of a lumbar spine within a back of a patient;

an electrode lead comprising an anchor at a distal end of the electrode lead, the anchor configured to anchor the one or more electrodes within the patient for delivering the electrical stimulation to the medial branch of the dorsal ramus nerve that innervates the multifidus muscle, wherein the anchor is configured to be angled relative to the electrode lead when implanted to anchor the electrode lead in place; and

a pulse generator configured to be coupled to the one or more electrodes, the pulse generator configured to be programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve via the one or more electrodes to cause the multifidus muscle to contract to improve neuromuscular control system performance, thereby alleviating the patient's back pain.

29. The system of claim 28 , wherein the pulse generator is programmed to deliver the electrical stimulation to the medial branch of the dorsal ramus nerve at a frequency selected from 12-30 Hz.

30. The system of claim 28 , wherein the pulse generator is configured to be implanted.
