Abstract

An aspiration thrombectomy system including an aspiration catheter having a proximal portion, a distal end and a lumen extending through the catheter, a vacuum source fluidically coupled to the catheter, a vacuum valve connected between the vacuum source and the proximal portion of the catheter to control a vacuum flow in the catheter lumen, a vent valve fluidically coupled to the proximal portion of the catheter, the vent valve being configured to control a flow of a vent fluid in the catheter lumen, and a controller configured to open and close the vacuum valve and the vent valve in a predetermined cycle to change a level of vacuum at the distal end of the catheter and to control fluid flow in and out from the distal end of the catheter.

Claims (23)

1 . An aspiration thrombectomy system, comprising:

an aspiration catheter having a proximal portion, a distal end, and a lumen extending through the catheter;

a vacuum source fluidically coupled to the catheter;

a first valve fluidically connected between the vacuum source and the proximal portion of the catheter to control a level of vacuum in the catheter lumen;

a second valve fluidically coupled to the proximal portion of the catheter, the second valve being configured to further control the level of vacuum in the catheter lumen; and

a controller configured to open and close the first valve and the second valve in a plurality of predetermined pressure protocols to change the level of vacuum in the catheter and to classify a flow state in the catheter, and to:

receive, during an aspiration thrombectomy procedure, pressure readings associated with a current flow state in the aspiration catheter from one or more pressure sensors associated with the catheter;

classify, during the aspiration thrombectomy procedure, using an artificial neural network, the current flow state in the aspiration catheter as one of a restricted flow state, an obstructed flow state, or an unrestricted flow state based at least in part on the pressure readings associated with the current flow state in the aspiration catheter, wherein the artificial neural network is trained to classify a catheter flow state based on training data comprising numerous pressure readings and flow state classifications associated with previous aspiration thrombectomy procedures; and

operate, responsive to the artificial neural network classifying the current flow state during the aspiration thrombectomy procedure, the first valve and the second valve in one of the pressure protocols based on the current flow state being classified as the restricted flow state, the obstructed flow state, or the unrestricted flow state.

2 . The system of claim 1 , further comprising a vent fluid source, and wherein:

the vent fluid source is configured to provide vent fluid at a pressure greater than atmospheric pressure;

the first valve is a vacuum valve configured to control a level of negative pressure provided by the vacuum source;

the second valve is a vent valve configured to control a level of positive pressure provided by the vent fluid source; and

the controller is configured to cyclically open and close the vacuum valve and the vent valve to change a level of negative pressure and positive pressure in the catheter during each cycle.

3 . The system of claim 2 , wherein the controller is configured to cyclically open and close the vacuum valve and the vent valve such that movement of a fluid column at the distal end of the catheter is limited to a positive amount of exit flow before fluid is drawn back into the catheter lumen.

4 . The system of claim 2 , wherein the controller is configured to cyclically open and close the vacuum valve and the vent valve in a repeated cycle comprising an off-off state in which the vacuum valve is closed while the vent valve is closed.

5 . The system of claim 2 , wherein:

the controller is configured to cyclically open and close the vacuum valve and the vent valve in a cycle comprising (a) a first off-off state in which the vacuum valve is closed while the vent valve is closed, (b) a vacuum-only state following the first off-off state in which the vacuum valve is open while the vent valve is closed, (c) a second off-off state following the vacuum-only state in which the vacuum valve is closed while the vent valve is closed, and (d) a vent-only state following the second off-off state in which the vacuum valve is closed while the vent valve is open; and

the cycle is repeated.

6 . The system of claim 5 , wherein the cycle is repeated at a predetermined frequency.

7 . The system of claim 2 , wherein the vacuum valve and the vent valve are solenoid pinch valves.

8 . The system of claim 2 , wherein the vacuum valve and the vent valve are cammed pinch valves.

9 . The system of claim 2 , wherein the controller is configured to cyclically open and close the vacuum valve and the vent valve such that an amount of acceleration force aspirates thromboembolic material at the distal end of the catheter.
