Abstract

Described are methods, systems, devices for facilitation of intraluminal medical procedures within the neurovasculature. A catheter advancement device includes a flexible elongate body having a proximal end, a distal end, and a single lumen extending therebetween. The flexible elongate body has a proximal segment, an intermediate segment, and a tip segment. The proximal segment includes a hypotube coated with a polymer. The intermediate segment includes an unreinforced polymer having a durometer of no more than 72 D. The tip segment is formed of a polymer different from the intermediate segment and has a durometer of no more than about 35 D and a length of at least 5 cm. The tip segment has a tapered portion that tapers distally from a first outer diameter to a second outer diameter over a length of between 1 and 3 cm.

Claims (49)

1. A system for performing a medical procedure at a treatment site within a cerebral vessel of a patient, the system comprising:

an outer catheter comprising a catheter lumen and a distal end;

a catheter advancement device for advancing the outer catheter within the cerebral vessel, the catheter advancement device comprising:

a flexible elongate body having a proximal end, a distal end, and a single lumen extending therebetween, the flexible elongate body comprising:

a proximal segment configured to move the catheter advancement device axially within the catheter lumen of the outer catheter;

an intermediate segment comprising a polymer having a first durometer; and

a tip segment formed of a polymer different from the polymer of the intermediate segment, the polymer of the tip segment having a second durometer less than the first durometer, wherein the tip segment transitions distally from a first outer diameter of between 0.048″ and 0.080″ to a distal-most end of the catheter advancement device having a second outer diameter smaller than the first outer diameter, the second outer diameter being at a distal opening from the single lumen,

wherein the catheter advancement device has a working length configured to extend from outside a patient's body, through a femoral artery, and to the cerebral vessel and the single lumen extends along the entire working length to the distal opening and has a constant inner diameter of about 0.010″ to about 0.024″, and

wherein an outer diameter of the flexible elongate body is sized to be positioned within the catheter lumen such that the tip segment is positioned distally beyond the distal end of the outer catheter to aid in delivery of the outer catheter to an intracranial vessel.

2. The system of claim 1 , wherein a taper angle of a wall of the tip segment relative to a center line of the tip segment is between 0.9 to 1.6 degrees.

3. The system of claim 1 , wherein the second outer diameter is about ½ of the first outer diameter, about 40% of the first outer diameter, or about 65% of the first outer diameter.

4. The system of claim 1 , wherein the intermediate segment includes a first segment having a material hardness of no more than 55 D and a second segment located proximal to the first segment having a material hardness of no more than 72 D.

5. The system of claim 1 , wherein the tip segment comprises at least a first radiopaque marker.

6. The system of claim 5 , wherein the first radiopaque marker is positioned at the first outer diameter and is configured to identify a maximum outer diameter of the flexible elongate body.

7. The system of claim 5 , wherein a second radiopaque marker is positioned near the second outer diameter and is configured to identify a distal end region of the flexible elongate body.

8. The system of claim 1 , wherein a wall thickness of the intermediate segment and an untapered portion of the tip segment is constant.

9. The system of claim 8 , wherein the wall thickness of the intermediate segment and the untapered portion of the tip segment is about 0.050 inch to about 0.065 inch.

10. The system of claim 1 , further comprising a guidewire extending within the single lumen so that a distal-most end of the guidewire is housed within the catheter advancement device proximal to a distal opening from the single lumen.

11. The system of claim 1 , wherein the flexible elongate body comprises no inner liner, and wherein the polymer of the intermediate segment comprises a lubricious additive incorporated within the polymer.

12. The system of claim 1 , wherein the tip segment between the first outer diameter and the second, outer diameter has a length that is between 0.5 cm and 4.0 cm.

13. The system of claim 1 , wherein the flexible elongate body further comprises a reinforced segment comprising a metallic structure.

14. The system of claim 13 , wherein the metallic structure of the reinforced segment extends up to the first outer diameter of the tip segment.

15. The system of claim 13 , wherein the metallic structure of the reinforced segment comprises a coil or a braid.

16. An intravascular catheter advancement device for advancing a catheter within a vasculature of a patient, the device comprising:

a flexible elongate body having a proximal end, a distal end, and a single lumen extending therebetween, the flexible elongate body comprising:

a proximal segment configured to move the catheter advancement device axially within the catheter;

an intermediate segment comprising a polymer having a first durometer; and

a tip segment formed of a polymer different from the polymer of the intermediate segment, the polymer of the tip segment having a second durometer less than the first durometer, wherein the tip segment comprises a radiopaque material, and wherein the tip segment has a continuous transition in outer diameter from a first outer diameter of between 0.048″ and 0.080″ to a second, smaller outer diameter at a distal-most end of the catheter advancement device,

wherein the catheter advancement device has a working length configured to extend from outside a patient's body, through a femoral artery, and to the vasculature, wherein the single lumen extends along the entire working length to the distal-most end and has a constant inner diameter of about 0.010″ to about 0.024″.

17. The device of claim 16 , wherein the radiopaque material is configured to identify a maximum outer diameter of the flexible elongate body.

18. The device of claim 16 , wherein a wall thickness of the intermediate segment and an untapered portion of the tip segment is constant.

19. The device of claim 18 , wherein the wall thickness of the intermediate segment and the untapered portion of the tip segment is about 0.050 inches to about 0.065 inches.

20. The device of claim 18 , wherein the wall thickness of the intermediate segment and the untapered portion of the tip segment is about 0.050 inches to about 0.065 inches and wherein the intermediate segment comprises an unreinforced polymer having a durometer of no more than 72 D.

21. The device of claim 16 , further comprising a guidewire extending within the single lumen so that a distal-most end of the guidewire is housed within the catheter advancement device proximal to a distal opening from the single lumen.

22. The device of claim 16 , wherein the flexible elongate body further comprises an inner liner.

23. The device of claim 16 , wherein the flexible elongate body comprises no inner liner, and wherein the polymer of the intermediate segment comprises a lubricious additive incorporated within the polymer.

24. The device of claim 16 , wherein the flexible elongate body comprises no inner liner, and wherein the polymer of the tip segment comprises a lubricious additive incorporated within the polymer of the tip segment.

25. The device of claim 16 , wherein the distal portion transitions from the first outer diameter to the second, smaller outer diameter over a length that is between 0.5 cm and 4.0 cm.

26. The device of claim 16 , wherein the tip segment has a constant taper from the first outer diameter to the distal-most end of the catheter advancement device.

27. An intravascular catheter advancement device for advancing a catheter within a cerebral vessel, the catheter advancement device comprising:

a flexible elongate body having a proximal end, a distal end, and a single lumen extending therebetween, the flexible elongate body comprising:

a proximal segment configured to move the catheter advancement device axially within a catheter lumen;

an intermediate segment comprising an unreinforced first polymer having a first durometer; and

a tip segment formed of a second polymer different from the unreinforced first polymer of the intermediate segment, the second polymer of the tip segment having a second durometer less than the first durometer, wherein the tip segment transitions from a first outer diameter of between 0.048″ and 0.080″ to a second, smaller outer diameter at a distal-most end of the catheter advancement device,

wherein an inner diameter of the intermediate segment and the tip segment is a constant inner diameter of about 0.010″ to about 0.024″,

wherein the catheter advancement device has a working length configured to extend from outside a patient's body, through a femoral artery, and to the cerebral vessel, wherein the single lumen extends the entire working length.

28. The system of claim 1 , wherein a flexibility of the tip segment at the distal end is greater than a flexibility of the distal end of the outer catheter, wherein the flexibility of the tip segment gradually transitions to approach the flexibility of the distal end of the outer catheter.

29. The system of claim 1 , wherein the intermediate segment comprises an unreinforced polymer having a durometer of no more than 72 D.

30. The system of claim 1 , wherein the catheter advancement device is pre-packaged coaxially positioned within the outer catheter such that the proximal section of the flexible elongate body is locked with the outer catheter.
