Abstract

First and second atrial arms and first and second ventricular arms are advanced to the heart. The first ventricular arm is moved distally by applying a pulling force to the first ventricular arm at a first fixed connection point that is at a portion of the first ventricular arm that is farthest from a first articulation site of the first atrial arm and the first ventricular arm, and the second ventricular arm is moved distally by applying a pulling force to the second ventricular arm at a second fixed connection point that is at a portion of the second ventricular arm that is farthest from a second articulation site of the second atrial arm and the second ventricular arm. Subsequently, the first native leaflet is clamped between the first arms, and the second native leaflet is clamped between the second arms. Other embodiments are also described.

Claims (61)

1. A method, comprising:

(A) transcatheterally advancing, toward a native heart valve of a heart of a patient, in a proximal to distal direction, apparatus comprising:

(a) a first atrial arm and a first ventricular arm on a first side of a central longitudinal axis of the apparatus;

(b) a second atrial arm and a second ventricular arm on a second side of the central longitudinal axis of the apparatus; and

(c) a core, (i) the first atrial arm, the first ventricular arm, and a first articulation site of the first atrial arm and the first ventricular arm, being on a first side of the core, and (ii) the second atrial arm, the second ventricular arm, and a second articulation site of the second atrial arm and the second ventricular arm, being on a second side of the core;

(B) subsequently, preparing the apparatus to (i) clamp a first native leaflet of the native heart valve between the first atrial arm and the first ventricular arm, and (ii) clamp a second native leaflet of the native heart valve between the second atrial arm and the second ventricular arm by:

distally moving the first ventricular arm with respect to the central longitudinal axis by applying, via a distal opening in the core, a pulling force to the first ventricular arm at a first fixed connection point that is at a portion of the first ventricular arm that is farthest from the first articulation site of the first atrial arm and the first ventricular arm; and

distally moving the second ventricular arm with respect to the central longitudinal axis by applying, via the distal opening in the core, a pulling force to the second ventricular arm at a second fixed connection point that is at a portion of the second ventricular arm that is farthest from the second articulation site of the second atrial arm and the second ventricular arm; and

(C) subsequently:

clamping the first native leaflet of the native heart valve between the first atrial arm and the first ventricular arm; and

clamping the second native leaflet of the native heart valve between the second atrial arm and the second ventricular arm.

2. The method according to claim 1 , wherein the steps of clamping the first native leaflet and clamping the second native leaflet comprise forming the first and second native leaflets into a generally lemniscate arrangement.

3. The method according to claim 1 , wherein the steps of clamping the first native leaflet and clamping the second native leaflet are performed in a manner that does not eliminate native blood flow regulation functionality of the native heart valve.

The method according to claim 1 , wherein distally moving the first ventricular arm comprises distally moving the first ventricular arm while the first atrial arm is held in place, and distally moving the second ventricular arm comprises distally moving the second ventricular arm while the second atrial arm is held in place.

5. The method according to claim 1 , wherein distally moving the first ventricular arm and distally moving the second ventricular arm comprises simultaneously distally moving the first ventricular arm and distally moving the second ventricular arm.

6. The method according to claim 1 , wherein distally moving the first and second ventricular arms comprises distally moving the first and second ventricular arms while the first and second ventricular arms and the first and second atrial arms are disposed in an atrium of the heart.

7. The method according to claim 6 , further comprising, subsequently to the steps of distally moving the first and second ventricular arms and before the steps of clamping the first native leaflet and clamping the second native leaflet:

moving the first and second ventricular arms into a ventricle of the heart.

8. The method according to claim 1 , wherein:

applying the pulling force to the first ventricular arm at the first fixed connection point comprises applying the pulling force to the first ventricular arm via a conduit in the core, and

applying the pulling force to the second ventricular arm at the second fixed connection point comprises applying the pulling force to the second ventricular arm via the conduit in the core.

9. The method according to claim 1 , wherein distally moving the first and second ventricular arms comprises distally moving the first and second ventricular arms while the first and second atrial arms are held in place adjacent to the core.

10. The method according to claim 1 , further comprising, subsequently to the steps of distally moving the first and second ventricular arms:

advancing at least the first and second ventricular arms and the distal end of the core into a ventricle of the heart; and

subsequently, raising the first and second ventricular arms toward the central longitudinal axis.

The method according to claim 1 , wherein the core has: (1) a first portion having a first perimeter in a first transverse plane to a central longitudinal axis of the core, and (2) a second portion having a second perimeter in a second transverse plane to the central longitudinal axis of the core that is distal to the first transverse plane, the second perimeter being a smallest perimeter of the core.

12. The method according to claim 11 , wherein the first and second articulation sites are adjacent to the smallest perimeter of the core.

13. The method according to claim 11 , wherein the core tapers in a distal direction from the first portion to the second portion of the core, and wherein the tapering of the core defines a minimum nonzero angle of the first and second atrial arms with respect to the central longitudinal axis.

14. The method according to claim 1 , wherein:

the core is shaped so as to define a conduit therethrough, and

the method further comprises controlling the first and second ventricular arms using an elongate controller that is slidably disposed within the conduit.

15. The method according to claim 14 , wherein controlling the first and second ventricular arms using the elongate controller comprises controlling the first and second ventricular arms using the elongate controller while not controlling the first and second atrial arms using the elongate controller.

16. The method according to claim 14 , wherein the elongate controller comprises a control rod.

17. A method, comprising:

(A) transcatheterally advancing, toward a native heart valve of a heart of a patient, in a proximal to distal direction, apparatus comprising:

(a) a first atrial arm and a first ventricular arm on a first side of a central longitudinal axis of the apparatus;

(b) a second atrial arm and a second ventricular arm on a second side of the central longitudinal axis of the apparatus; and

(c) a core, (i) the first atrial arm, the first ventricular arm, and a first articulation site of the first atrial arm and the first ventricular arm, being on a first side of the core, and (ii) the second atrial arm, the second ventricular arm, and a second articulation site of the second atrial arm and the second ventricular arm, being on a second side of the core;

(B) subsequently, preparing the apparatus to (i) clamp a first native leaflet of the native heart valve between the first atrial arm and the first ventricular arm, and (ii) clamp a second native leaflet of the native heart valve between the second atrial arm and the second ventricular arm by:

distally moving the first ventricular arm with respect to the central longitudinal axis by applying a pulling force to the first ventricular arm at a first fixed connection point that is at a portion of the first ventricular arm that is farthest from the first articulation site of the first atrial arm and the first ventricular arm; and

distally moving the second ventricular arm with respect to the central longitudinal axis by applying a pulling force to the second ventricular arm at a second fixed connection point that is at a portion of the second ventricular arm that is farthest from the second articulation site of the second atrial arm and the second ventricular arm; and

(C) subsequently:

clamping the first native leaflet of the native heart valve between the first atrial arm and the first ventricular arm; and

clamping the second native leaflet of the native heart valve between the second atrial arm and the second ventricular arm,

wherein distally moving the first and second ventricular arms comprises distally moving the first and second ventricular arms while the first and second ventricular arms, the first and second atrial arms, and a distal end of the core are disposed in an atrium of the heart.

18. A method, comprising:

(A) transcatheterally advancing, toward a native heart valve of a heart of a patient, in a proximal to distal direction, apparatus comprising:

(a) a first atrial arm and a first ventricular arm on a first side of a central longitudinal axis of the apparatus;

(b) a second atrial arm and a second ventricular arm on a second side of the central longitudinal axis of the apparatus; and

(c) a core, (i) the first atrial arm, the first ventricular arm, and a first articulation site of the first atrial arm and the first ventricular arm, being on a first side of the core, and (ii) the second atrial arm, the second ventricular arm, and a second articulation site of the second atrial arm and the second ventricular arm, being on a second side of the core;

(B) subsequently, preparing the apparatus to (i) clamp a first native leaflet of the native heart valve between the first atrial arm and the first ventricular arm, and (ii) clamp a second native leaflet of the native heart valve between the second atrial arm and the second ventricular arm by:

distally moving the first ventricular arm with respect to the central longitudinal axis by applying a pulling force to the first ventricular arm at a first fixed connection point that is at a portion of the first ventricular arm that is farthest from the first articulation site of the first atrial arm and the first ventricular arm; and

distally moving the second ventricular arm with respect to the central longitudinal axis by applying a pulling force to the second ventricular arm at a second fixed connection point that is at a portion of the second ventricular arm that is farthest from the second articulation site of the second atrial arm and the second ventricular arm; and

(C) subsequently:

clamping the first native leaflet of the native heart valve between the first atrial arm and the first ventricular arm; and

clamping the second native leaflet of the native heart valve between the second atrial arm and the second ventricular arm,

wherein preparing the apparatus further comprises positioning the core between the first and second native leaflets of the native heart valve.

19. The method according to claim 18 , wherein positioning the core comprises:

positioning the first atrial arm and the first ventricular arm such that the first native leaflet is disposed between the first atrial arm and the first ventricular arm, and

positioning the second atrial arm and the second ventricular arm such that the second native leaflet is disposed between the second atrial arm and the second ventricular arm.

20. The method according to claim 19 , wherein (1) positioning the first atrial arm and the first ventricular arm and (2) positioning the second atrial arm and the second ventricular arm comprises positioning the first atrial arm and the first ventricular arm and the second atrial arm and the second ventricular arm while a proximal end portion of the core is disposed at an atrial side of the native heart valve and a distal portion of the core is disposed at a ventricular side of the native heart valve.
