Abstract

A system for percutaneous spinal fusion may include two spaced apart blades connected together by a coupling such that the blades define a percutaneous pathway from a skin incision to an implanted pedicle fastener. The coupling may be c-shaped and may have at least one flexible tab for engaging one or more holes along the length of the blades. If one of the blades becomes disconnected from the pedicle fastener, a supplemental access device may be provided comprising a tubular body having a channel therein for receiving the other of the blades. If both of the blades become disconnected, a supplemental access device may be provided comprising a gripping member received within a locking member. The gripping member may have two legs engageable with the pedicle fastener, and the locking member may move along the gripping member to prevent the legs from disengaging the pedicle fastener.

Claims (24)

1 . A method for restoring a percutaneous pathway to a pedicle fastener connected to a vertebra of a patient, the method comprising:

inserting a percutaneous access device into a body of a patient through an incision, such that the percutaneous access device provides a first pathway extending from the incision to a head of a pedicle fastener connected to a vertebra of the patient;

removing at least a portion of the percutaneous access device from a first attachment point of the head of the pedicle fastener; and

attaching a supplemental access device to the head of the pedicle fastener at a second attachment point, the supplemental access device providing a second pathway extending from the incision to the head of the pedicle fastener,

wherein the first attachment point is at a different location on the head of the pedicle fastener from the second attachment point.

The method of claim 1 , wherein a cross-sectional area of the first pathway transverse to a longitudinal dimension of the percutaneous access device has a first size, wherein the step of removing at least the portion of the percutaneous access device causes the cross-sectional area of the first pathway to be substantially reduced or eliminated, and wherein a cross-sectional area of the second pathway transverse to a longitudinal dimension of the supplemental access device has a second size, the second size being generally the same as the first size.

3 . The method of claim 1 , wherein the supplemental access device has a tubular body defining the second pathway therein.

4 . The method of claim 1 , wherein the first pathway extends along a longitudinal dimension of the percutaneous access device.

The method of claim 4 , wherein the percutaneous access device includes first and second slots elongated along the longitudinal dimension, the first and second slots being diametrically opposed to one another so as to permit passage of a fixation rod therethrough along a direction transverse to the longitudinal dimension.

The method of claim 5 , wherein the supplemental access device includes first and second slots elongated along a longitudinal dimension of the supplemental access device, the first and second slots of the supplemental access device being diametrically opposed to one another so as to permit passage of the fixation rod therethrough along a direction transverse to the longitudinal dimension of the supplemental access device.

7 . The method of claim 6 , further comprising inserting the fixation rod into the body of the patient along at least a portion of the second pathway, such that the fixation rod passes through at least one of the first and second slots of the supplemental access device.

The method of claim 5 , wherein the percutaneous access device includes a first blade and a second blade, the first and second blades being spaced apart from one another so as to define the first pathway therebetween, and the first and second slots of the percutaneous access device being defined between the first and second blades.

9 . The method of claim 8 , wherein the first and second blades extend substantially parallel to one another along the longitudinal dimension when connected to the head of the pedicle fastener.

10 . The method of claim 8 , wherein the first and second blades are integrally formed with the head of the pedicle fastener, the first and second blades each being connected to the head of the pedicle fastener by a frangible portion.

11 . The method of claim 8 , wherein the step of removing at least the portion of the percutaneous access device comprises removing the first blade from the head of the pedicle fastener.

12 . The method of claim 11 , wherein the step of attaching the supplemental access device to the head of the pedicle fastener includes receiving the second blade in a receiving structure of the supplemental access device.

13 . The method of claim 8 , wherein the step of removing at least the portion of the percutaneous access device comprises removing both of the first and second blades from the head of the pedicle fastener.

The method of claim 13 , wherein the step of attaching the supplemental access device to the head of the pedicle fastener includes engaging a gripping member and a locking member of the supplemental access device with the head of the pedicle fastener, the locking member being adapted to prevent disengagement between the gripping member and the head of the pedicle fastener.

15 . The method of claim 1 , wherein the step of inserting the percutaneous access device into the body of the patient occurs concurrently with a step of inserting the pedicle fastener into the body and connecting the pedicle fastener to the vertebra.

The method of claim 1 , wherein the step of attaching the supplemental access device to the head of the pedicle fastener includes advancing the supplemental access device distally towards the head of the pedicle fastener until a distal end of the supplemental access device receives a structure of an outside of the head of the pedicle fastener in an interlocking arrangement.

The method of claim 16 , wherein the distal end of the supplemental access device is configured to snap into the interlocking arrangement with the outside of the head of the pedicle fastener due to distal advancement of the supplemental access device while the distal end of the supplemental access device receives the structure of the outside of the head of the pedicle fastener.

18 . The method of claim 1 , wherein the pedicle fastener includes a pedicle screw anchorable in a pedicle of the vertebra.

19 . The method of claim 1 , wherein the first attachment point is a top of the head of the pedicle fastener and the second attachment point is at a side of the head of the pedicle fastener.

20 . The method of claim 1 , wherein the pedicle fastener includes an engagement feature at the second location on the head of the pedicle fastener, and wherein the engagement feature is exposed to an exterior of the percutaneous access device prior to the removing of the at least the portion of the percutaneous access device.
