Abstract

An anchor and suture assembly is provided for use with an endoscopic suturing device with a needle holder arm. The anchor assembly includes an anchor tip and an anchor body. The anchor tip has a blunt end that may be rounded or beaded, a capture groove, a tab groove and a plug portion positioned between the capture groove and the tab groove. The anchor body has first and second ends, tip tabs, retainers for removably retaining the anchor body relative to the needle holder arm, and a suture opening. The anchor tip is fixed relative to the anchor body by plastic deformation of the tip tabs into the tab groove. A suture extends into the suture opening of the anchor body and is fixed therein.

Claims (40)

1. A method of performing a suturing operation using an endoscopic suturing device provided on a distal end of an endoscope, said method comprising:

positioning the endoscopic suturing device adjacent a first location along a patient's tissue, the suturing device having a needle holder arm with a sharp tip and a needle guard, wherein the suturing device is initially positioned adjacent the first location with the sharp tip of the needle holder arm shrouded by the needle guard and distally spaced from the distal end of the endoscope;

extending the sharp tip of the needle holder arm through tissue at the first location;

extending the sharp tip proximally towards the distal end of the endoscope and into engagement with a first end of an anchor assembly distal to a second end positioned within a needle capture device extending from the endoscope;

releasing the anchor assembly from the needle capture device to retain the anchor assembly on the needle holder arm; and

returning the needle holder arm to a position shrouded by the needle guard.

The method of claim 1 , wherein a suture is coupled to the anchor assembly, said method further comprising pulling the suture with the anchor assembly from the needle capture device through tissue to return the needle holder arm carrying the anchor assembly to the position shrouded by the needle guard.

3. The method of claim 2 , further comprising:

moving the endoscopic suturing device to a second location along the patient's tissue spaced apart from the first location; and

moving the needle holder arm to extend the distal end of the anchor assembly, including the suture, through the second location.

4. The method of claim 3 , further comprising extending the distal end of the anchor assembly into the needle capture device to remove the anchor assembly from the needle holder arm and to retain the anchor assembly with respect to the needle capture device.

5. The method of claim 4 , further comprising tying the suture after a desired number of stitches have been completed.

6. The method of claim 3 , wherein the distal end of the anchor assembly is sharp, and extending the distal end of the anchor assembly through the second location comprises piercing tissue with the distal end of the anchor assembly.

7. A method of performing a suturing operation using an endoscopic suturing device provided on a distal end of an endoscope and having a needle holder arm movable from a first position to a second position, said method comprising:

positioning the endoscopic suturing device adjacent a first location along a patient's tissue with the needle holder arm in the first position;

extending the distal end of the needle holder arm through tissue at the first location to move the needle holder arm from the first position to the second position;

extending a distal end of the needle holder arm into engagement with a first end of an anchor assembly having a second end positioned proximal to the first end and within a needle capture device extending from the endoscope; and

returning the needle holder arm to the first position.

8. The method of claim 7 , wherein a suture is coupled to the anchor assembly, said method further comprising pulling the suture with the anchor assembly from the needle capture device through tissue to return the needle holder arm carrying the anchor assembly to the first position.

9. The method of claim 8 , further comprising:

moving the endoscopic suturing device to a second location along the patient's tissue spaced apart from the first location; and

moving the needle holder arm to extend the distal end of the anchor assembly, including the suture, through the second location.

The method of claim 9 , further comprising moving the needle holder arm to the second position to extend the distal end of the anchor assembly into the needle capture device to remove the anchor assembly from the needle holder arm and to retain the anchor assembly with respect to the needle capture device.

11. The method of claim 10 , further comprising tying the suture after a desired number of stitches have been completed.

12. The method of claim 9 , wherein the distal end of the anchor assembly is sharp, and extending the distal end of the anchor assembly through the second location comprises piercing tissue with the distal end of the anchor assembly.

13. The method of claim 7 , wherein the anchor assembly is securely held by the needle capture device, said method further comprising releasing the anchor assembly from the needle capture device to be carried by the needle holder arm through tissue.

14. The method of claim 7 , wherein the first position of the needle holder arm is a position in which the distal end of the needle holder arm is shrouded from the patient's tissue.

15. The method of claim 7 , wherein the second position of the needle holder arm is a position engaging the anchor assembly held by the needle capture device.

16. The method of claim 7 , wherein the needle holder arm is closer to the patient's tissue when in the first position than when in the second position.

17. A method of performing a suturing operation using an endoscopic suturing device, said method comprising:

transluminally advancing the endoscopic suturing device operatively associated with a distal end of an endoscope to a first location along a patient's tissue;

advancing a distal end of a needle holder arm of the suturing device from a first position thereof through tissue at the first location;

extending a distal end of the needle holder arm to a second position thereof closer to the endoscope and into engagement with an anchor assembly held by a needle capture device of the endoscopic suturing device;

releasing the anchor assembly from the needle capture device to retain the anchor assembly on the needle holder arm; and

returning the needle holder arm to the first position thereof, distal to the endoscope.

18. The method of claim 17 , wherein a suture is coupled to the anchor assembly, said method further comprising pulling the suture with the anchor assembly from the needle capture device through tissue to return the needle holder arm carrying the anchor assembly to the first position.

19. The method of claim 18 , further comprising:

moving the endoscopic suturing device to a second location along the patient's tissue spaced apart from the first location; and

moving the needle holder arm to extend the anchor assembly, including the suture, through the second location.

20. The method of claim 19 , further comprising capturing the anchor assembly with the needle capture device to remove the anchor assembly from the needle holder arm and to retain the anchor assembly with respect to the needle capture device.
