Abstract

Embodiments consistent with the present disclosure allow the ring, arc, and linear offset values for a stereotactic frame to be determined for a cranial procedure without requiring a CT scan to be acquired with the fiducial frame attached. Having the ring, arc, and offset values available for a cranial procedure allows the surgeon to revert to traditional stereotactic ring-arc frame guidance as a fallback strategy in case a robot cannot be used. This disclosure also provides an alternate method for registering a ring-arc system to the medical image volume for facilities having no CT scanner or scanner with limited field of view such that the intended fiducial localizer cannot be used.

Claims (41)

1 . A surgical robot system comprising:

a tracking device having a plurality of cameras;

a stereotactic frame base configured to be attached to a patient head;

a dynamic reference base (DRB) adjustably attachable to the stereotactic frame base and having a plurality of first tracking markers trackable by the cameras;

a frame reference array (FRA) attachable to the stereotactic frame base and having a plurality of second tracking markers trackable by the cameras;

a ring-arc assembly attachable to the stereotactic frame base;

a surgical robot in communication with the tracking device and including a processor, a robot arm controllable by the processor and an end effector attachable to the robot arm, wherein the processor is configured to:

position the end effector along a planned trajectory using the DRB tracked by the cameras;

in case the surgical robot becomes unavailable, perform the following steps:

register an acquired medical image to the stereotactic frame base based on simultaneous tracking of the FRA and the DRB by the cameras;

determine manual dial positions of the ring-arc assembly for the planned trajectory based on the registered stereotactic frame base.

2 . The system of claim 1 , wherein the DRB includes at least one mounting arm that adjustably attaches the first tracking markers along a point on the mounting arm so as to change positions of the first tracking markers relative to the stereotactic frame base.

3 . The system of claim 1 , wherein the FRA always attaches to a predetermined position on the stereotactic frame base such that positions of the second tracking markers remain the same relative to the stereotactic frame base.

4 . The system of claim 1 , further comprising an intraoperative CT (ICT) fixture attachable to the frame base and including a plurality of third tracking markers trackable by the cameras and a plurality of radiopaque fiducials in predetermined positions relative to the third tracking markers.

5 . The system of claim 4 , wherein the processor is configured to receive the acquired medical image containing an image of the radiopaque fiducials for registration purposes.

6 . The system of claim 5 , wherein the processor is configured to receive a computerized tomography (CT) image of the brain as the acquired medical image.

7 . The system of claim 1 , further comprising a CT localizer attachable to the stereotactic frame base and including a plurality of fiducial markers.

8 . The system of claim 1 , further comprising a needle adapted to be received in and held by the end effector.

9 . The system of claim 1 , further comprising an electrode adapted to be received in and held by the end effector.

10 . The system of claim 1 , wherein the processor determines the manual dial positions for ring, arc, and xyz offset of the ring-arc assembly.

11 . A surgical robot system comprising:

a tracking device having a plurality of cameras;

a stereotactic frame base configured to be attached to a patient head;

a dynamic reference base (DRB) adjustably attachable to the stereotactic frame base and having a plurality of first tracking markers trackable by the cameras;

a frame reference array (FRA) non-adjustably attachable to the stereotactic frame base and having a plurality of second tracking markers trackable by the cameras;

a ring-arc assembly attachable to the stereotactic frame base;

a surgical robot in communication with the tracking device and including a processor, a robot arm controllable by the processor and an end effector attachable to the robot arm, wherein the processor is configured to:

register a medical image of the patient head to the DRB tracked by the cameras;

position the end effector along a planned trajectory using the registered DRB;

in case the surgical robot becomes unavailable, perform the following steps:

register the medical image to the stereotactic frame base based on simultaneous tracking of the FRA and the DRB by the cameras;

determine manual dial positions of the ring-arc assembly for the planned trajectory based on the registered stereotactic frame base.

12 . The system of claim 11 , wherein the DRB includes at least one mounting arm that adjustably attaches the first tracking markers along a point on the mounting arm so as to change positions of the first tracking markers relative to the stereotactic frame base.

13 . The system of claim 11 , wherein the FRA always attaches to a predetermined position on the stereotactic frame base such that positions of the second tracking markers remain the same relative to the stereotactic frame base.

14 . The system of claim 11 , further comprising an intraoperative CT (ICT) fixture attachable to the frame base and including a plurality of third tracking markers trackable by the cameras and a plurality of radiopaque fiducials in predetermined positions relative to the third tracking markers.

15 . The system of claim 14 , wherein the processor is configured to receive the registered medical image containing an image of the radiopaque fiducials for registration purposes.

16 . The system of claim 15 , wherein the processor is configured to receive a computerized tomography (CT) image of the brain as the registered medical image.

17 . The system of claim 11 , further comprising a CT localizer attachable to the stereotactic frame base and including a plurality of fiducial markers.

18 . The system of claim 11 , further comprising a needle adapted to be received in and held by the end effector.

19 . The system of claim 11 , further comprising an electrode adapted to be received in and held by the end effector.

20 . The system of claim 11 , wherein the processor determines the manual dial positions for ring, arc, and xyz offset of the ring-arc assembly.
