Abstract

Methods, non-transitory computer readable media, surgical tracking devices, and systems that facilitate improved tracking in surgical environments are disclosed. With this technology, a fiber optic shape sensing (FOSS) device is rigidly attached to patient anatomy and/or surgical instrument(s) to facilitate location and/or flexibility tracking. The data provided by the FOSS device can be analyzed and registered to preoperative or intraoperative 3D anatomy model(s). The FOSS device can be attached to a surgical instrument to provide intraoperative guidance, used to locate a hollow needle for tracking bones in a minimally invasive manner, and/or used to detect bending in an instrument such as an arthroscope or tissue removing burr shaft, for example. The tracked location and/or flexibility data provided by the FOSS device can also be used to automatically control the location and/or operation of a surgical instrument during a surgical proceeding to facilitate improved surgical accuracy and patient outcomes.

Claims (68)

1. A method for surgical instrument location tracking, the method comprising:

generating strain data using a shape sensor attached to a surgical instrument, the shape sensor comprising a plurality of distributed strain sensors along a length of the shape sensor;

determining, from the strain data generated by the plurality of distributed strain sensors of the shape sensor, a shape of the shape sensor;

tracking location of a distal end of the surgical instrument based on the shape of the shape sensor, wherein the tracking results in a tracked location; and

adjusting a graphical display to output a representation of at least a portion of the surgical instrument based on the tracked location,

wherein the surgical instrument comprises a flexible portion of a drill guide, and wherein the method further comprises placing a guidewire for a femoral tunnel based on the tracked location in order to define one or more of a trajectory or an orientation of the guidewire.

2. The method of claim 1 wherein attaching the shape sensing device comprises attaching a fiber optic shape sensing (FOSS) device, wherein the FOSS device comprises the distributed strain sensors in the form of an optical fiber comprising fiber Bragg grating (FBG) sensors.

3. The method of claim 2 wherein determining the shape of the shape sensor comprises:

introducing broadband light into the optical fiber of the FOSS device; and

generating reflectivity data based on reflection from the FBG sensors of the broadband light.

4. The method of claim 2 , further comprising controlling a location or an operation of the surgical instrument based on the tracked location of the FBG sensors.

5. The method of claim 1 , further comprising controlling an operation of the surgical instrument based on the tracked location.

6. The method of claim 1 , wherein the surgical instrument comprises two or more surgical instruments attached to the shape sensor.

7. A method for surgical instrument location tracking, the method comprising:

generating strain data using a shape sensor attached to a surgical instrument, the shape sensor comprising a plurality of distributed strain sensors along a length of the shape sensor;

determining, from the strain data generated by the plurality of distributed strain sensors of the shape sensor, a shape of the shape sensor;

tracking location of a distal end of the surgical instrument based on the shape of the shape sensor, wherein the tracking results in a tracked location;

adjusting a graphical display to output a representation of at least a portion of the surgical instrument based on the tracked location, wherein the surgical instrument comprises a limb or joint of a robotic arm;

determining when a collision of the limb or joint of the robotic arm, or an instrument held by the robotic arm, with another tracked object in a surgical environment is imminent based on the tracked location; and

preventing the collision based on control of the location of the limb or joint of the robotic arm.

8. A method for surgical instrument location tracking, the method comprising:

generating strain data using a shape sensor attached to a surgical instrument, the shape sensor comprising a plurality of distributed strain sensors along a length of the shape sensor;

determining, from the strain data generated by the plurality of distributed strain sensors of the shape sensor, a shape of the shape sensor;

tracking location of a distal end of the surgical instrument based on the shape of the shape sensor, wherein the tracking results in a tracked location; and

adjusting a graphical display to output a representation of at least a portion of the surgical instrument based on the tracked location,

wherein the surgical instrument comprises a shaver, and the method further comprises automatically controlling the shaver based on the tracked location to remove one or more portions of an anatomical structure of a patient.

9. A method for surgical instrument location tracking, the method comprising:

generating strain data using a shape sensor attached to a surgical instrument, the shape sensor comprising a plurality of distributed strain sensors along a length of the shape sensor;

determining, from the strain data generated by the plurality of distributed strain sensors of the shape sensor, a shape of the shape sensor;

tracking location of a distal end of the surgical instrument based on the shape of the shape sensor, wherein the tracking results in a tracked location; and

adjusting a graphical display to output a representation of at least a portion of the surgical instrument based on the tracked location,

wherein the surgical instrument comprises an ablator, and the method further comprises automatically controlling the ablator based on the tracked location to remove one or more portions of an anatomical structure of a patient.

10. A method for anatomical structure location tracking, the method comprising:

attaching a shape sensor to the anatomical structure of a patient;

generating strain data using the shape sensor attached to the anatomical structure, the shape sensor comprising a plurality of distributed strain sensors along a length of the shape sensor;

determining, from the strain data generated by the plurality of distributed strain sensors of the shape sensor, a shape of the shape sensor;

tracking a location of the anatomical structure based on the shape of the shape sensor, wherein the tracking results in a tracked location; and

adjusting a graphical display to output a representation of at least a portion of the anatomical structure based on the tracked location.

11. A surgical tracking system comprising:

a surgical instrument;

a shape sensor comprising a plurality of distributed strain sensors along a length of the shape sensor, the shape sensor coupled to the surgical instrument;

a surgical tracking device coupled to the shape sensor, the surgical tracking device comprising:

a processor;

a memory coupled to the processor; and

a display device coupled to the processor;

the memory storing instructions that, when executed by the processor, cause the processor to:

generate strain data from the plurality of distributed strain sensors;

determine, from strain data, a shape of the shape sensor;

track a location of the surgical instrument based on the shape of the shape sensor, wherein the tracking results in a tracked location; and

adjust the display device to output a representation of at least a portion of the surgical instrument based on the tracked location,

wherein the surgical instrument comprises at least one of a flexible portion of a drill guide, a shaver, and an ablator.

12. The surgical tracking system of claim 11 , wherein the shape sensing device comprises a fiber optic shape sensing (FOSS) device, wherein the FOSS device comprises the distributed strain sensors in the form of an optical fiber comprising fiber Bragg grating (FBG) sensors.

13. The surgical tracking system of claim 12 , wherein when the processor determines the shape, the instructions cause the processor to:

introduce broadband light into the optical fiber of the FOSS device; and

generate reflectivity data based on reflection from the FBG sensors of the broadband light.

14. The surgical tracking system of claim 11 , wherein the instructions further cause the processor to control an operation of the surgical instrument based on the tracked location.

15. The surgical tracking system of claim 11 , wherein the surgical instrument comprises two or more surgical instruments attached to the shape sensor.

16. A surgical tracking system, comprising:

a shape sensor comprising a plurality of distributed strain sensors along a length of the shape sensor, wherein the shape sensor is attached an anatomical structure of a patient;

a surgical tracking device coupled to the shape sensor, the surgical tracking device comprising:

a processor;

a memory coupled to the processor; and

a display device coupled to the processor;

the memory storing instructions that, when executed by the processor, cause the processor to:

generate strain data from the plurality of distributed strain sensors;

determine, from strain data, a shape of the shape sensor;

track a location of the anatomical structure based on the shape of the shape sensor, wherein the tracking results in a tracked location; and

adjust the display device to output a representation of at least a portion of the anatomical structure based on the tracked location.
