Abstract

Disclosed embodiments relate to devices, systems, and methods of shaping, shrinking, opening, dilating, stiffening, or otherwise modifying a Eustachian tube and its surrounding tissue in order to improve the Eustachian tube's function. For example, patients with blocked, closed, or hypertrophic Eustachian tubes may be able to achieve improved function including easier equalization of pressure between the inner ear and environment.

Claims (33)

1 . A device for treatment, the device comprising:

an elongate shaft comprising a proximal end and a distal end;

a treatment element positioned at the distal end of the elongate shaft, the treatment element having a circular shape and comprising a flat treatment surface; and

a plurality of electrodes disposed on the flat treatment surface, the plurality of electrodes being positioned in a plurality of circular rows,

wherein the plurality of electrodes is configured to come into contact with tissue and apply radiofrequency energy to the tissue.

2 . The device of claim 1 , further comprising a passive positioner extending from the treatment element and configured to come in contact with a Eustachian tube.

3 . The device of claim 2 , wherein the passive positioner comprises an elongate protrusion having a proximal end coupled to the treatment element and a distal end having a bulb sized and shaped to be inserted into a portion of the Eustachian tube.

4 . The device of claim 2 , wherein the passive positioner comprises an elongate flexible probe that is configured to enter and align with the tissue.

5 . The device of claim 2 , wherein the passive positioner is configured to contact tissue of an ostium of the Eustachian tube when positioned for treatment.

6 . The device of claim 2 , wherein the flat treatment surface is circular and the plurality of circular rows of electrodes are positioned circumferentially around the passive positioner.

7 . The device of claim 2 , wherein the passive positioner comprises a visualization device to provide one or more images of the tissue to facilitate placement of the treatment element.

8 . The device of claim 1 , wherein the plurality of electrodes comprises bipolar, radiofrequency electrodes.

9 . The device of claim 1 , wherein the plurality of circular rows comprises a plurality of pairs of bipolar, radiofrequency electrodes.

10 . The device of claim 1 , wherein the flat treatment surface is configured to be positioned parallel to a longitudinal axis of the elongate shaft.

11 . A device for treatment, the device comprising:

an elongate shaft comprising a proximal end and a distal end;

a treatment element positioned at the distal end of the elongate shaft, the treatment element having an arcuate shape and comprising a flat treatment surface; and

a plurality of electrodes disposed on the flat treatment surface, the plurality of electrodes being positioned in a plurality of arcuate rows;

wherein the plurality of electrodes is configured to apply radiofrequency energy to tissue, and

wherein the arcuate shape of the treatment element is configured to be sized to be positioned around a portion of a Eustachian tube.

12 . The device of claim 11 , further comprising at least one light positioned on the treatment element, the at least one light being configured to illuminate the tissue.

13 . The device of claim 11 , wherein the plurality of electrodes are configured to be bipolar radiofrequency electrode pairs.

14 . A method of treating a Eustachian tube, the method comprising:

advancing a treatment element of a treatment device through a nostril, the treatment element having a circular shape and comprising a flat treatment surface;

inserting a passive positioner extending from a central portion of the flat treatment surface of the treatment element into the Eustachian tube;

contacting a tissue of the Eustachian tube with the flat treatment surface of the treatment element, wherein the flat treatment surface comprises a plurality of electrodes positioned on the flat treatment surface, the plurality of electrodes being positioned in a plurality of circular rows; and

applying radiofrequency energy from the plurality of electrodes on the flat treatment surface to the tissue.

15 . The method of claim 14 , wherein the passive positioner comprises an elongate flexible probe that is configured to enter and align with the tissue.

16 . The method of claim 15 , wherein the passive positioner is configured to contact tissue of an ostium of the Eustachian tube.

17 . The method of claim 14 , wherein the plurality of electrodes is disposed on the flat treatment surface circumferentially around the passive positioner.

18 . The method of claim 14 , wherein applying the radiofrequency energy through the tissue results in shaping, shrinking, opening, dilating, tightening, or stiffening the Eustachian tube or a Eustachian tube ostium.

19 . The method of claim 14 , wherein applying the radiofrequency energy through the tissue alters at least one property of the tissue, thereby improving a function of the Eustachian tube or a Eustachian tube ostium.

20 . The method of claim 14 , further comprising advancing the treatment element into the Eustachian tube.
