Abstract

According to various embodiments, systems, devices and methods for modulating targeted nerve fibers (e.g., hepatic neuromodulation) or other tissue are provided. The systems may be configured to access tortuous anatomy of or adjacent hepatic vasculature. The systems may be configured to target nerves surrounding (e.g., within adventitia of or within perivascular space of) an artery or other blood vessel, such as the common hepatic artery.

Claims (37)

1 . A method of ablating nerves surrounding a blood vessel comprising:

inserting a neuromodulation device within a blood vessel, the neuromodulation device comprising:

a catheter including an expandable element at a distal portion of the catheter, and

a therapeutic element within the expandable element, the therapeutic element configured to deliver energy to one or more nerves proximate the blood vessel,

wherein the therapeutic element is configured to act as a lesion spacing indicator;

positioning the therapeutic element at a first treatment location within the blood vessel;

causing the therapeutic element to deliver energy at the first treatment location;

repositioning the neuromodulation device axially within the blood vessel to a second treatment location based on a dimension indicated by the lesion spacing indicator such that the second treatment location is spaced apart from the first treatment location by at least a minimum threshold spacing; and

causing the therapeutic element to deliver energy at the second treatment location.

2 . The method of claim 1 , further comprising:

repositioning the neuromodulation device axially within the blood vessel to a third treatment location using the lesion spacing indicator such that a spacing between the first treatment location and the second treatment location is approximately equal to a spacing between the second treatment location and the third treatment location; and

causing the therapeutic element to deliver energy at the third treatment location.

3 . The method of claim 1 , further comprising:

repositioning the neuromodulation device to a third treatment location, wherein the third treatment location is located in a different branch of vasculature than the first treatment location and the second treatment location; and

causing the therapeutic element to deliver energy at the third treatment location.

4 . The method of claim 1 , wherein the first treatment location is spaced from the second treatment location by a distance 1 to 15 times a diameter of the therapeutic element.

5 . The method of claim 1 , wherein the minimum threshold spacing is 2 mm.

6 . The method of claim 1 , wherein the first treatment location and the second treatment location are space apart by 4 mm to 6 mm.

7 . The method of claim 1 , wherein a length of the therapeutic element is 4 mm to 6 mm.

8 . The method of claim 1 , wherein the first treatment location is selected based on a distance to a branch of the blood vessel.

9 . The method of claim 8 , wherein the distance to the branch of the blood vessel is at least 1 mm.

10 . The method of claim 1 , wherein the first treatment location is selected based on a distance into a target vessel.

11 . The method of claim 1 , wherein the therapeutic element includes an ultrasound transducer and the energy includes ultrasound energy.

12 . The method of claim 1 , wherein repositioning the neuromodulation device axially within the blood vessel to the second treatment location includes repositioning the neuromodulation device without delivering a contrast agent.

13 . The method of claim 1 , wherein positioning the therapeutic element at the first treatment location within the blood vessel comprises:

positioning the therapeutic element at the first treatment location such that the first treatment location is a minimum threshold distance from a dense structure.

14 . The method of claim 13 , wherein the minimum threshold distance from the dense structure is at least 2 mm.

15 . The method of claim 13 , wherein the dense structure is one or more of a liver, a pancreas, a stomach, a small intestine, or a kidney.

16 . The method of claim 1 , wherein the energy is sufficient to ablate at least some nerves surrounding the blood vessel.

17 . The method of claim 1 , further comprising causing the therapeutic element to center within the blood vessel by at least causing the expandable element to expand.

18 . The method of claim 1 , further comprising:

causing the expandable element to expand against an inner wall of the blood vessel at one or more of the first treatment location or the second treatment location; and

causing a fluid to circulate through the expandable element for a period of time prior to causing the therapeutic element to deliver energy to the inner wall of the blood vessel at the first treatment location or the second treatment location.

19 . A method comprising:

delivering, by an ultrasound transducer positioned within an expandable element expanded within a blood vessel at a first treatment location, energy sufficient to ablate one or more nerves; and

delivering, by the ultrasound transducer positioned within the expandable element expanded within the blood vessel at a second treatment location, energy sufficient to ablate one or more nerves a second treatment location, wherein the second treatment location is spaced apart from the first treatment location by at least a minimum threshold spacing defined by the ultrasound transducer.

20 . The method of claim 19 , wherein the first treatment location is spaced from the second treatment location by a distance 1 to 15 times a diameter of the ultrasound transducer.
