Abstract

A nerve mapping system includes an elongate medical device, a non-invasive mechanical sensor, and a processor. The elongate medical device includes a distal end portion configured to explore an intracorporeal treatment area of a subject, and the distal end portion includes an electrode. The non-invasive mechanical sensor is configured to provide a mechanomyography output signal corresponding to a monitored mechanical response of a muscle innervated by the nerve. The processor is in communication with the electrode and the sensor, and is configured to provide a plurality of electrical stimuli to the electrode. Each of the plurality of stimuli is provided when the electrode is located at a different position within the intracorporeal treatment area. The processor determines the likelihood of a nerve existing at a particular point using the magnitudes of each of the stimuli and the detected response of the muscle.

Claims (14)

1 . A surgical planning system comprising:

an electrode for engaging a patient;

a sensor disposed on the patient, the sensor producing a mechanomyography output signal associated with a response by a nerve stimulated by the electrode;

a processor configured to:

provide a plurality of electrical stimuli with differing magnitudes to the electrode at different locations and/or positions on the patient;

determine, via the mechanomyography output signals, the presence of a nerve;

use the location of the determined nerve to create a nerve model.

2 . The surgical planning system of claim 1 , wherein the processor is further configured to update the nerve model iteratively.

3 . The surgical planning system of claim 1 , wherein the processor is further configured to create a nerve probability model.

4 . The surgical planning system of claim 1 , wherein the processor is further configured to overlay a graphical representation of the nerve model over an anatomical representation of the patient.

5 . The surgical planning system of claim 4 , wherein the anatomical representation is based on three-dimensional CT or MRI images.

6 . The surgical planning system of claim 4 , wherein the anatomical representation is based on two-dimensional fluoroscope or ultrasound images.

7 . The surgical planning system of claim 4 , wherein the processor is further configured to use the overlay to plan a surgical approach toward an anatomical target on the patient.

8 . The surgical planning system of claim 7 , wherein the processor is further configured to an optimal surgical approach.
