SURGICAL DEVICES AND METHODS FOR BARIATRIC AND GASTROESOPHAGEAL SURGERY
Disclosed are various embodiments for improved surgical devices and methods of using the same in connection with bariatric and gastroesophageal surgery. The present disclosure includes a lighted bougie device that can include an elongate member and a light source. The light source can be configured to emit near-infrared light. The light source can also be positioned about the bougie such that light emitted from the light source illuminates along a portion of the bougie.

51 . A method for using a lighted device in a surgical procedure, wherein the lighted device comprises an elongated member and at least one light source coupled to the elongate member, the method comprising:
inserting at least a first portion of the elongated member of the lighted device into an operative field of a patient;
illuminating the operative field with a light emitted from the at least one light source disposed at the elongate member of the lighted device;
capturing one or more images of the operative field while the first portion of the elongated member of the lighted device is in the operative field with an image capturing device disposed outside the operative field; and
determining a location of the first portion of the elongated member of the lighted device in the operative field of the patient based on the one or more images captured by the image capturing device.
52 . The method of claim 51 , wherein:
the at least one light source comprises a plurality of light sources disposed at the first portion of the elongated member, wherein adjacent light sources in the plurality of light sources are spaced apart along a length direction of the elongated member at a first predetermined interval;
the lighted device comprises a plurality of markings disposed at the first portion of the elongated member, wherein adjacent markings in the plurality of markings are spaced apart along the length direction of the elongated member at a second predetermined interval; or
a combination thereof.
53 . The method of claim 52 , further comprising:
using at least the first portion of the elongated member of the lighted device as a guide or calibration tool to perform one or more intraoperative measurements.
54 . The method of claim 53 , wherein a measurement in the one or more intraoperative measurements is selected from the group consisting of:
a measurement of a length of a Heller Myotomy during a laparoscopic or robotic Heller operation;
a measurement of a distance between an esophageal hiatus and a gastric angle of His during a laparoscopic or robotic hiatal hernia repair;
a measurement of a length of a gastric pouch in laparoscopic or robotic bypass surgery; length of gastric sleeve during a laparoscopic or robotic sleeve gastrectomy; and
a measurement to localize gastric and lower esophageal tumors during laparoscopic or robotic operations.
55 . The method of claim 54 , wherein the measurement in the one or more intraoperative measurements is performed during a surgery taking place in a rectum, colon, upper and lower gastrointestinal tract, urologic organ, or gynecological organ.
56 . The method of claim 52 , wherein the surgical procedure comprises laparoscopic sleeve gastrectomy, and the elongated member of the lighted device is used as a guide or calibration tool in creating a gastric sleeve during sleeve gastrectomy.
57 . The method of claim 52 , wherein the surgical procedure comprises a gastric bypass, and the elongated member of the lighted device is used as a guide or calibration tool in creating a gastric pouch during gastric bypass.
58 . The method of claim 52 , wherein the surgical procedure comprises a laparoscopic hiatal hernia repair, and the lighted device is used as a guide or calibration tool during gastric fundoplication.
59 . The method of claim 52 , wherein adjacent light sources in the plurality of light sources or adjacent markings in the plurality of markings are spaced apart evenly along the length direction of the elongated member of the lighted device.
60 . The method of claim 52 , wherein the plurality of light sources is configured to emit light in a first wavelength range that is able to transmit through one or more first tissues of the patient but unable to transmit through one or more second tissues of the patient and a second wavelength range that is able to transmit through both the one or more first tissues and the one or more second tissues of the patient.
61 . The method of claim 60 , wherein the first wavelength range is about 400 nm to about 650 nm, and the second wavelength range is about 700 nm to about 1300 nm, or about 700 nm to about 850 nm.
62 . The method of claim 52 , wherein the plurality of light sources comprises five or more light sources, or the plurality of markings comprises five or more markings.
63 . The method of claim 51 , wherein the at least one light source is configured to generate a white light and a near-infrared light.
64 . The method of claim 63 , wherein the illuminating of the operative field comprises:
illuminating the operative field alternately with the white light and the near-infrared light.
65 . The method of claim 51 , wherein the at least one light source comprises at least one LED light, at least one optic fiber, a polymer containing a near infrared fluorescent material, or any combination thereof.
66 . The method of claim 51 , wherein the elongate member of the lighted device has a diameter of 34 French, 36 French, 38 French, or 40 French, or a length of about 32 inches.
67 . The method of claim 51 , wherein the elongated member of the lighted device comprises a suction channel, and one or more apertures extending from an exterior surface of the elongate member into the suction channel.
68 . The method of claim 51 , wherein the lighted device further comprises a handle, wherein the elongate member extends from the handle.
69 . The method of claim 51 , wherein the surgical procedure is a robotic assisted surgery.
70 . The method of claim 51 , wherein the operative field comprises an esophagus and/or stomach of the patient.
71 . The method of claim 51 , wherein the image capturing device comprises a white light camera, a near-infrared camera, or a combination thereof.
