Abstract

Intraocular pressure in an eye is reduced by delivering a high resolution optical coherence tomography (OCT) beam and a high resolution laser beam through the cornea, and the anterior chamber into the irido-corneal angle along an angled beam path. The OCT beam provides OCT imaging for surgery planning and monitoring, while the laser beam is configured to modify tissue or affect ocular fluid by photo-disruptive interaction. In one implementation, a volume of ocular tissue within an outflow pathway in the irido-corneal angle is modified to create a channel opening in one or more layers of the trabecular meshwork. In another implementation, a volume of fluid in the Schlemm's canal is affected by the laser to bring about a pneumatic expansion of the canal. In either implementation, resistance to aqueous flow through the eye is reduced.

Claims (38)

1 . An integrated surgical system for treating an eye, the integrated surgical system comprising:

a laser source configured to output a laser beam;

an exit lens configured to optically couple to a cornea, the exit lens having an input surface, an output surface opposite the input surface, and a first optical axis that extends through an apex of the input surface and the apex of the output surface;

one or more laser scanner components optically coupled to receive the laser beam;

an optical assembly optically coupled to receive the laser beam and direct the laser beam into the input surface of the exit lens along an angled beam path that extends through the cornea, through an anterior chamber, and into an irido-corneal angle, wherein the optical assembly is configured to rotate around the first optical axis independent of corresponding movement of the exit lens; and

a control system coupled to the laser source and the one or more laser scanner components, and configured to:

instruct the one or more laser scanner components to scan the laser beam through a volume within a Schlemm's canal, and

instruct the laser source to apply laser energy through the laser beam while the laser beam is scanned through the volume, wherein the laser energy is configured to create a pneumatic effect that expands the Schlemm's canal.

2 . The integrated surgical system of claim 1 , wherein:

the exit lens is axially symmetric with the first optical axis.

3 . The integrated surgical system of claim 1 , wherein one or more components of the one or more laser scanner components is configured to rotate with the optical assembly.

4 . The integrated surgical system of claim 1 , wherein the laser source is configured to rotate with the optical assembly.

5 . The integrated surgical system of claim 1 , wherein the optical assembly comprises one or more beam folding mirrors.

6 . The integrated surgical system of claim 1 , wherein the optical assembly comprises a prism.

7 . The integrated surgical system of claim 1 , wherein the optical assembly comprises an optical grating.

8 . The integrated surgical system of claim 1 , further comprising:

an OCT apparatus configured to output an OCT beam; and

one or more OCT scanner components optically coupled to receive the OCT beam,

wherein, the optical assembly is optically coupled to receive the OCT beam and direct the OCT beam into the input surface of the exit lens along the angled beam path that extends through the cornea, through an anterior chamber, and into an irido-corneal angle.

9 . The integrated surgical system of claim 8 , wherein:

one or more components of the one or more OCT scanner components is configured to rotate with the optical assembly.

10 . The integrated surgical system of claim 8 , wherein:

the OCT apparatus is configured to rotate with the optical assembly.

11 . A method of treating an eye, the method comprising:

directing, by an optical assembly, a laser beam into an input surface of an exit lens along an angled beam path that exits the exit lens through an output surface and extends through a cornea, through an anterior chamber, and into an irido-corneal angle, wherein the exit lens is optically coupled to a cornea of the eye and has a first optical axis that extends through an apex of the input surface and the apex of the output surface, and the optical assembly is configured to rotate around the first optical axis independent of corresponding movement of the exit lens;

scanning the laser beam through a volume within a Schlemm's canal; and

applying laser energy through the laser beam while the laser beam is scanned through the volume, wherein the laser energy creates a pneumatic effect that expands the Schlemm's canal.

12 . The method of claim 11 , further comprising:

prior to directing a laser beam, rotating the optical assembly around the first optical axis independent of corresponding movement of the exit lens to align the angled beam path with a region of the irido-corneal angle around a circumference of the irido-corneal angle.

13 . The method of claim 12 , wherein the exit lens is axially symmetric with the first optical axis.

14 . The method of claim 12 , wherein the laser beam is scanned by one or more laser scanner components and rotating the optical assembly comprises rotating one or more components of the one or more laser scanner components with the optical assembly.

15 . The method of claim 12 , wherein the laser beam is output by a laser source and rotating the optical assembly comprises rotating the laser source with the optical assembly.

16 . The method of claim 11 , further comprising:

directing, by the optical assembly, an OCT beam into the input surface of the exit lens along the angled beam path.

17 . The method of claim 16 , further comprising:

prior to directing the OCT beam, rotating the optical assembly around the first optical axis independent of corresponding movement of the exit lens to align the angled beam path with a region of the irido-corneal angle around a circumference of the irido-corneal angle.

18 . The method of claim 17 , wherein the OCT beam is scanned by one or more OCT scanner components and rotating the optical assembly comprises rotating one or more components of the one or more OCT scanner components with the optical assembly.

19 . The method of claim 17 , wherein the OCT beam is output by an OCT beam source and rotating the optical assembly comprises rotating the OCT beam source with the optical assembly.
