Abstract

Apparatus, systems, and methods are provided for optimizing intracardiac filling pressures and cardiac output in patients with heart failure, conduction disease, and atrial fibrillation. The system is able to adjust and optimize intracardiac filling pressures and cardiac output by adjusting heart rate and the effective amount of total body blood volume. The device includes an adjustable member that may create a mean pressure differential in order to manifest an effective “mechanical diuresis” by sequestering extraneous blood volume to the high-capacitance of the venous vasculature. The system is therefore designed to reduce intracardiac filling pressures while maintaining or even increasing cardiac output.

21 . A system for treating a patient with impaired renal function, comprising:

at least one elongate member comprising a proximal end and a distal end sized for introduction into a venous side of the patient's body;

at least one sensor configured to provide sensor data corresponding to pressures on the venous side of the heart;

at least one adjustable component carried on the distal end and configured to be expandable to partially occlude blood flow and create a pressure gradient; and

a controller coupled to the at least one sensor and the at least one adjustable component, the controller programmed to selectively expand the adjustable component based at least in part on sensor data to create the pressure gradient to improve renal function.

22 . The system of claim 21 , wherein the distal end is sized for introduction into one of an inferior vena cava and a superior vena cava of the patient's body.

23 . The system of claim 21 , wherein the controller is programmed to adjust the pressure gradient to induce redistribution of blood volume to the patient's high-capacitance venous system, thereby reducing intracardiac filling pressures and improving renal perfusion.

24 . A method for treating a patient with impaired renal function, comprising:

introducing an elongate member carrying an adjustable component into at least one venous side of the patient's body;

providing a controller configured to adjust the adjustable component based at least in part on sensor data from at least one sensor to create a pressure gradient within the at least one venous side of the patient's body; and

expanding, by the controller, the adjustable component to partially occlude blood flow and create the pressure gradient to improve renal function.

25 . The method of claim 24 , wherein the pressure gradient is created in a superior vena cava of the patient's body to optimize redistribution of blood volume and reduce central venous pressure.

26 . The method of claim 24 , wherein the pressure gradient is created in an inferior vena cava of the patient's body to optimize redistribution of blood volume and reduce central venous pressure.

27 . A system for treating a patient with impaired renal function, comprising:

at least one elongate member comprising a proximal end and a distal end sized for introduction into a venous side of the patient's body;

at least one adjustable component carried on the distal end and configured to be expandable to temporarily occlude blood flow; and

a controller coupled to the at least one adjustable component, the controller programmed to selectively expand the adjustable component to improve renal function.

The system of claim 27 , wherein the adjustable component is an expandable member configured to be expanded within the venous side of the patient's body to provide an occlusion to sequester blood volume in the patient's venous system below the occlusion to reduce central venous pressure and improve the patient's hemodynamic status.

29 . The system of claim 27 , further comprising at least one sensor configured to provide sensor data corresponding to pressures on the venous side.

30 . The system of claim 29 , wherein the controller is coupled to the at least one sensor and the at least one adjustable component, the controller programmed to selectively expand the adjustable component based at least in part on sensor data to improve renal function.

31 . The system of claim 27 , wherein the controller is configured to adjust the at least one adjustable component over time.
