Abstract

The invention disclosed here generally relates to devices used to identify the location of a never in order to modify a property of the nerve. Specifically, the invention utilized pre-operative scans of a patient's nasal cavity in order to identify target treatment locations when a nerve to be treated is located. The image of the nasal cavity and target treatment location can be registered with the real time position of the nasal cavity using a surgical navigation system in order to assist in guiding a surgical probe to the target treatment location.

Claims (45)

1 . A surgical probe, comprising:

a handle comprising a port;

a shaft extending distally of the handle;

an end effector at a distal end of the handle, wherein the end effector is configured to ablate a tissue region in a nasal cavity;

one or more location sensors that are configured to generate a signal indicative of a location of the one or more location sensors in an electromagnetic field; and

one or more wires extending from the one or more location sensors to the port of the handle,

wherein the one or more wires are configured to communicate the signal generated by the one or more location sensors to a surgical navigation device such that the surgical navigation device can determine a location of the end effector relative to the tissue region in the nasal cavity.

2 . The surgical probe of claim 1 , wherein the handle comprises a source of liquid cryogen, and the end effector comprises an expandable structure that is configured to receive a cryogen from the source of liquid cryogen to ablate a tissue region in a nasal cavity.

3 . The surgical probe of claim 1 , wherein the one or more location sensors comprises three or more location sensors on the end effector.

4 . The surgical probe of claim 3 , wherein the end effector comprises a planar member,

wherein the three or more location sensors are on the planar member.

5 . The surgical probe of claim 4 , wherein the three or more location sensors comprises:

a first location sensor that is configured to be positioned at a first reference point corresponding to an intersection of a posterior fontanelle and a perpendicular plate of a palatine bone;

a second location sensor that is configured to be positioned at a second reference point corresponding to a sphenopalatine foramen; and

a third location sensor that is configured to be positioned at a third reference point corresponding to a ridge of an inferior turbinate,

wherein the end effector is configured such that the first location sensor is positionable at the first reference point, the second location sensor positionable at the second reference point, and the third location sensor is positionable at the third reference point at the same time.

6 . The surgical probe of claim 1 , wherein the end effector is configured to ablate the tissue region by at least one thermal therapy selected from a group of thermal therapies consisting of: cryotherapy, cryoneuromodulation, cryomodulation, cryolysis, cryoablation, and thermoablation.

7 . The surgical probe of claim 1 , wherein at least one location sensor of the one or more location sensors is coupled to a handle of the surgical probe.

8 . The surgical probe of claim 7 , wherein the handle comprises a detent that is configured to removably couple with the at least one location sensor.

9 . The surgical probe of claim 7 , wherein the at least one location sensor is coupled a distal end of the handle that is configured to remain outside of the nasal cavity when the end effector ablates the tissue region.

10 . The surgical probe of claim 7 , wherein the at least one location sensor is located at an internal portion of the handle.

11 . The surgical probe of claim 1 , wherein the end effector comprises a plurality of electrodes that are configured to a radiofrequency energy to ablate the tissue region.

12 . The surgical probe of claim 11 , wherein, for each electrode, a respective location sensor of the one or more location sensors is located proximate to the electrode.

13 . A method of treating tissue within a nasal cavity, comprising:

inserting an end effector of a surgical probe into the nasal cavity, wherein the surgical probe comprises:

a handle comprising a port;

a shaft extending distally of the handle;

an end effector at a distal end of the handle, wherein the end effector is configured to ablate a tissue region in a nasal cavity;

one or more location sensors that are configured to generate a signal indicative of a location of the one or more location sensors in an electromagnetic field; and

one or more wires extending from the one or more location sensors to the port of the handle,

wherein the one or more wires are configured to communicate the signal generated by the one or more location sensors to a surgical navigation device such that the surgical navigation device can determine a location of the end effector relative to the tissue region in the nasal cavity;

positioning the end effector at a target treatment location based on the signal generated by the one or more location sensors; and

while positioning the end effector at the target treatment location, performing ablation of the tissue region using the end effector.

14 . The method of claim 13 , wherein using the end effector to ablate the tissue region comprises applying to the tissue region at least one thermal therapy selected from a group consisting of: cryotherapy, cryoneuromodulation, cryomodulation, cryolysis, cryoablation, and thermoablation.

15 . The method of claim 13 , wherein the handle comprises a source of liquid cryogen, and wherein using the end effector to ablate the tissue region comprises supplying a liquid cryogen from the source of liquid cryogen to an expandable structure.

16 . The method of claim 13 , wherein using the end effector to ablate the tissue region comprises applying, by a plurality of electrodes, a radiofrequency energy to the tissue region.

17 . The method of claim 13 , wherein the one or more location sensors comprises three or more location sensors on the end effector.

18 . The method of claim 17 , wherein the end effector comprises a planar member,

wherein the three or more location sensors are on the planar member.

19 . The method of claim 18 , wherein the three or more location sensors comprises:

a first location sensor that is configured to be positioned at a first reference point corresponding to an intersection of a posterior fontanelle and a perpendicular plate of a palatine bone;

a second location sensor that is configured to be positioned at a second reference point corresponding to a sphenopalatine foramen; and

a third location sensor that is configured to be positioned at a third reference point corresponding to a ridge of an inferior turbinate,

wherein positioning the end effector at the target treatment location comprises simultaneously positioning the first location sensor at the first reference point, the second location sensor at the second reference point, and the third location sensor at the third reference point.

20 . The method of claim 13 , wherein at least one location sensor of the one or more location sensors is coupled to a handle of the surgical probe.
