Abstract

The present invention concerns the compounds of formula (I) wherein R 1 , R 2 , R 3 , R 4 , X, Ar 1 and Ar 2 are as described in the description, and their use in the treatment of CFTR-related diseases and disorders, especially in the treatment of cystic fibrosis, in combination with one or more therapeutically active ingredients acting as CFTR modulator(s), wherein said CFTR modulator(s) is/are CFTR one or more CFTR corrector(s) and/or a CFTR potentiator. The invention further relates to pharmaceutical compositions comprising the compounds of formula (I) in combination with one or more therapeutically active ingredients acting as CFTR modulator(s), wherein said CFTR modulator(s) is/are one or more CFTR corrector(s) and/or a CFTR potentiator.

Claims (116)

1 . A pharmaceutical composition comprising, as active principles, a compound of Formula (I):

wherein

X represents —CR X1 R X2 , wherein R X1 represents hydrogen, and R X2 represents

C 1-6 -alkyl;

C 1-4 -fluoroalkyl; or

C 3-6 -cycloalkyl;

R 1 represents C 1-4 -alkyl;

R 2 represents C 1-4 -alkyl;

R 3 represents C 1-6 -alkyl;

R 4 represents 5-membered heteroaryl, wherein said 5-membered heteroaryl independently is unsubstituted, mono-, or di-substituted, wherein the substitutents are independently selected from C 1-4 -alkyl, C 1-4 -alkoxy, C 1-3 -fluoroalkyl, C 1-3 -fluoroalkoxy, C 3-6 -cycloalkyl, or halogen;

Ar 1 represents 8- to 10-membered bicyclic heteroarylene, wherein said bicyclic heteroarylene independently is unsubstituted or mono-substituted with C 1-4 -alkyl, or halogen;

Ar 2 represents phenyl, wherein said phenyl is unsubstituted, mono- or di-substituted wherein the substituents are independently selected from C 1-4 -alkyl, C 1-3 -fluoroalkyl, halogen, C 1-6 -alkoxy, and C 1-3 -fluoroalkoxy;

or a pharmaceutically acceptable salt thereof;

in combination with one or more therapeutically active ingredients acting as CFTR modulator(s); wherein said CFTR modulator(s) is/are one or more CFTR corrector(s), and/or a CFTR potentiator; or a pharmaceutically acceptable salt thereof; as well as at least one pharmaceutically acceptable excipient.

2 . A pharmaceutical composition according to claim 1 , wherein the compound of Formula (I) is a compound of Formula (I E ):

or a pharmaceutically acceptable salt thereof.

3 . A pharmaceutical composition according to claim 1 , wherein X represents —CR X1 R X2 , wherein R X1 is hydrogen, and R X2 is 2,2,2-trifluoroethyl.

4 . A pharmaceutical composition according to claim 1 , wherein R 4 represents oxadiazolyl, wherein said 5-membered heteroaryl independently is mono-substituted with C 1-4 -alkoxy.

5 . A pharmaceutical composition according to claim 1 , wherein Ar 1 represents 10-membered bicyclic heteroarylene, wherein said bicyclic heteroarylene is mono-substituted with halogen.

6 . A pharmaceutical composition according to claim 1 , wherein Ar 2 represents unsubstituted phenyl.

7 . A pharmaceutical composition according to claim 1 , wherein the compound of Formula (I) is:

(3S,7S,10R,13R)-13-benzyl-7-isobutyl-N-(2-(3-methoxy-1,2,4-oxadiazol-5-yl)ethyl)-6,9,20-trimethyl-1,5,8,11-tetraoxo-10-(2,2,2-trifluoroethyl)-1,2,3,4,5,6,7,8,9,10,11,12,13,14-tetradecahydro-[1]oxa[4,7,10,14]tetraazacycloheptadecino[16,17-f]isoquinoline-3-carboxamide;

(3S,7S,10S,13R)-13-benzyl-7-isobutyl-N-(2-(3-methoxy-1,2,4-oxadiazol-5-yl)ethyl)-6,9,20-trimethyl-1,5,8,11-tetraoxo-10-(2,2,2-trifluoroethyl)-1,2,3,4,5,6,7,8,9,10,11,12,13,14-tetradecahydro-[1]oxa[4,7,10,14]tetraazacycloheptadecino[16,17-f]isoquinoline-3-carboxamide;

A pharmaceutical composition comprising, as active principles, (3S,7S,10R,13R)-13-benzyl-20-fluoro-7-isobutyl-N-(2-(3-methoxy-1,2,4-oxadiazol-5-yl)ethyl)-6,9-dimethyl-1,5,8,11-tetraoxo-10-(2,2,2-trifluoroethyl)-1,2,3,4,5,6,7,8,9,10,11,12,13,14-tetradecahydro-[1]oxa[4,7,10,14]tetraazacycloheptadecino[16,17-f]quinoline-3-carboxamide;

or a pharmaceutically acceptable salt thereof;

in combination with one or more therapeutically active ingredients acting as CFTR modulator(s); wherein said CFTR modulator(s) is/are one or more CFTR corrector(s), and/or a CFTR potentiator; or a pharmaceutically acceptable salt thereof; as well as at least one pharmaceutically acceptable excipient.

9 . A pharmaceutical composition according to claim 1 , wherein the one or more therapeutically active ingredients acting as CFTR modulator(s) is/are a type-I corrector selected from lumacaftor, tezacaftor, and galicaftor; and/or a type-II corrector which is Corrector4a; and/or a type-III corrector selected from elexacaftor and vanzacaftor; and/or a CFTR potentiator selected from ivacaftor, navocaftor, icenticaftor, and deutivacaftor; or a pharmaceutically acceptable salt thereof.

10 . A pharmaceutical composition according to claim 1 , wherein said composition comprises the compound of formula (I), or a pharmaceutically acceptable salt thereof, and one therapeutically active ingredient acting as CFTR modulator, wherein said CFTR modulator is a CFTR potentiator; or a pharmaceutically acceptable salt thereof.

11 . A pharmaceutical composition according to claim 1 , wherein the composition comprises the compound of formula (I), or a pharmaceutically acceptable salt thereof, and two therapeutically active ingredients acting as CFTR modulators, wherein one of said CFTR modulators is a CFTR potentiator or a pharmaceutically acceptable salt thereof, and, the second CFTR modulator is a CFTR corrector or a pharmaceutically acceptable salt thereof.

12 . A pharmaceutical composition according to claim 1 , wherein said composition comprises the compound of formula (I), or a pharmaceutically acceptable salt thereof, and

ivacaftor and tezacaftor, or pharmaceutically acceptable salts thereof; or

ivacaftor and lumacaftor, or pharmaceutically acceptable salts thereof; or

navocaftor and galicaftor, or pharmaceutically acceptable salts thereof.

A pharmaceutical composition according to claim 8 , wherein the one or more therapeutically active ingredients acting as CFTR modulator(s) is/are a type-I corrector selected from lumacaftor, tezacaftor, and galicaftor; and/or a type-II corrector which is Corrector4a; and/or a type-III corrector selected from elexacaftor and vanzacaftor; and/or a CFTR potentiator selected from ivacaftor, navocaftor, icenticaftor, and deutivacaftor; or a pharmaceutically acceptable salt thereof.

17 . A pharmaceutical composition according to claim 8 , wherein said composition comprises one therapeutically active ingredient acting as CFTR modulator, wherein said CFTR modulator is a CFTR potentiator; or a pharmaceutically acceptable salt thereof.

A pharmaceutical composition according to claim 8 , wherein the composition comprises two therapeutically active ingredients acting as CFTR modulators, wherein one of said CFTR modulators is a CFTR potentiator or a pharmaceutically acceptable salt thereof, and, the second CFTR modulator is a CFTR corrector or a pharmaceutically acceptable salt thereof.

A pharmaceutical composition according to claim 8 , wherein said composition comprises (3S,7S,10R,13R)-13-benzyl-20-fluoro-7-isobutyl-N-(2-(3-methoxy-1,2,4-oxadiazol-5-yl)ethyl)-6,9-dimethyl-1,5,8,11-tetraoxo-10-(2,2,2-trifluoroethyl)-1,2,3,4,5,6,7,8,9,10,11,12,13,14-tetradecahydro-[1]oxa[4,7,10,14]tetraazacycloheptadecino[16,17-f]quinoline-3-carboxamide;

or a pharmaceutically acceptable salt thereof;

and

ivacaftor and tezacaftor, or pharmaceutically acceptable salts thereof; or

ivacaftor and lumacaftor, or pharmaceutically acceptable salts thereof; or

navocaftor and galicaftor, or pharmaceutically acceptable salts thereof.

20 . A method of treatment of cystic fibrosis in a subject in need thereof, said method comprising the administration of an effective amount of a pharmaceutical composition as defined in claim 1 to the subject.

21 . A method of treatment of cystic fibrosis in a subject in need thereof, said method comprising the administration of an effective amount of a pharmaceutical composition as defined in claim 8 to the subject.

22 . A method of treatment of cystic fibrosis in a subject in need thereof, said method comprising the administration of an effective amount of a compound of Formula (I):

wherein

X represents —CR X1 R X2 , wherein R X1 represents hydrogen, and R X2 represents

C 1-6 -alkyl;

C 1-4 -fluoroalkyl; or

C 3-6 -cycloalkyl;

R 1 represents C 1-4 -alkyl;

R 2 represents C 1-4 -alkyl;

R 3 represents C 1-6 -alkyl;

R 4 represents 5-membered heteroaryl, wherein said 5-membered heteroaryl independently is unsubstituted, mono-, or di-substituted, wherein the substitutents are independently selected from C 1-4 -alkyl, C 1-4 -alkoxy, C 1-3 -fluoroalkyl, C 1-3 -fluoroalkoxy, C 3-6 -cycloalkyl, or halogen;

Ar 1 represents 8- to 10-membered bicyclic heteroarylene, wherein said bicyclic heteroarylene independently is unsubstituted or mono-substituted with C 1-4 -alkyl, or halogen;

Ar 2 represents phenyl, wherein said phenyl is unsubstituted, mono- or di-substituted wherein the substituents are independently selected from C 1-4 -alkyl, C 1-3 -fluoroalkyl, halogen, C 1-6 -alkoxy, and C 1-3 -fluoroalkoxy;

or a pharmaceutically acceptable salt thereof;

and an effective amount of one or more therapeutically active ingredients acting as CFTR modulator(s);

wherein said CFTR modulator(s) is/are one or more CFTR corrector(s), and/or a CFTR potentiator; or a pharmaceutically acceptable salt thereof.

A method of treatment of cystic fibrosis in a subject in need thereof, said method comprising the administration of an effective amount of (3S,7S,10R,13R)-13-benzyl-20-fluoro-7-isobutyl-N-(2-(3-methoxy-1,2,4-oxadiazol-5-yl)ethyl)-6,9-dimethyl-1,5,8,11-tetraoxo-10-(2,2,2-trifluoroethyl)-1,2,3,4,5,6,7,8,9,10,11,12,13,14-tetradecahydro-[1]oxa[4,7,10,14]tetraazacycloheptadecino[16,17-f]quinoline-3-carboxamide;

or a pharmaceutically acceptable salt thereof;

and an effective amount of one or more therapeutically active ingredients acting as CFTR modulator(s);

wherein said CFTR modulator(s) is/are one or more CFTR corrector(s), and/or a CFTR potentiator; or a pharmaceutically acceptable salt thereof.

24 . A method according to claim 22 , wherein said CFTR modulators are

ivacaftor and tezacaftor, or pharmaceutically acceptable salts thereof; or

ivacaftor and lumacaftor, or pharmaceutically acceptable salts thereof; or

navocaftor and galicaftor, or pharmaceutically acceptable salts thereof.

25 . A method according to claim 23 , wherein said CFTR modulators are

ivacaftor and tezacaftor, or pharmaceutically acceptable salts thereof; or

ivacaftor and lumacaftor, or pharmaceutically acceptable salts thereof; or

navocaftor and galicaftor, or pharmaceutically acceptable salts thereof.
