Abstract

With the above in mind, embodiments of the present invention are related to a method including the steps of: providing a graphical user interface to a medical provider, wherein the graphical user interface displays a first dashboard or a second dashboard based on a criteria, wherein the graphical user interface automatically displays the first dashboard having a patient rounding list when a device is located within a hospital geofence and automatically displays the second dashboard having remote monitoring data when the device is located outside the hospital geofence; sending a secure clinical message to the medical provider when a trigger is met; forming a care team by automatically and dynamically selecting a plurality of care team members from a user database based on a location of each user, shift data of each user, and role data of each user; sending a discharge notification to the provider; and remotely monitoring the patient after discharge via at least one remote monitoring device and patient feedback.

Claims (44)

1 . A method comprising the steps of:

providing a graphical user interface to a medical provider, wherein the graphical user interface displays a first dashboard or a second dashboard based on a criteria, wherein the graphical user interface automatically displays the first dashboard having a patient rounding list when a device is located within a hospital geofence and automatically displays the second dashboard having remote monitoring data when the device is located outside the hospital geofence;

sending a secure clinical message to the medical provider when a trigger is met;

forming a care team by automatically and dynamically selecting a plurality of care team members from a user database based on a location of each user, shift data of each user, and role data of each user;

sending a discharge notification to the provider; and

remotely monitoring the patient after discharge via at least one remote monitoring device and patient feedback.

2 . The method of claim 1 wherein the criteria is location and the first dashboard is displayed when the graphical user interface is located at a first location and the second dashboard is displayed when the graphical user interface is located at a second location.

3 . The method of claim 2 wherein the first dashboard includes a patient rounding list and the second dashboard includes remote monitoring data.

4 . The method of claim 1 wherein the criteria is a user role and the first dashboard is displayed when the medical provider has a first user role and the second dashboard is displayed when the medical provider has a second user role.

5 . The method of claim 1 further comprising the step of providing the secure clinical message to a second care provider upon receipt of a forwarding response.

6 . A method for monitoring patient care comprising the steps of:

providing a graphical user interface to a medical provider, wherein the graphical user interface displays a first dashboard or a second dashboard based on a criteria, wherein the graphical user interface automatically displays the first dashboard having a patient rounding list when a device is located within a hospital geofence and automatically displays the second dashboard having remote monitoring data when the device is located outside the hospital geofence;

providing an urgent care symptom checker interface to a patient;

receiving a plurality of symptoms from the patient through the urgent care symptom checker interface;

receiving sensor data;

sending a secure clinical message to the medical provider when a trigger is met;

receiving a response to the secure clinical message wherein the response is an escalation response, a forwarding response, or a delegation response;

sending a discharge notification to the medical provider; and

providing a discharge questionnaire interface to the patient.

7 . The method of claim 6 wherein the trigger is receipt of the plurality of symptoms.

8 . The method of claim 6 wherein the trigger is receipt of an admission record indicating the patient is being admitted to a hospital.

9 . The method of claim 6 wherein the trigger is receipt of sensor data.

10 . The method of claim 6 wherein the criteria is location and the first dashboard is displayed when the graphical user interface is located at a first location and the second dashboard is displayed when the graphical user interface is located at a second location.

11 . The method of claim 10 wherein the first dashboard includes a patient rounding list and the second dashboard includes remote monitoring data.

12 . The method of claim 6 wherein the criteria is a user role and the first dashboard is displayed when the medical provider has a first user role and the second dashboard is displayed when the medical provider has a second user role.

13 . The method of claim 6 further comprising the step of sending the secure clinical message to a second care provider upon receipt of the forwarding response.

14 . The method of claim 6 further comprising the step of forming a care team by automatically and dynamically selecting a plurality of care team members from a user database based on a location of each user, shift data of each user, role data of each user, and the plurality of symptoms.

The method of claim 14 further comprising the step of providing a virtual rounding list to a device of a care team member, wherein the virtual rounding list includes the patient, each patient on the list is selectable to provide information on a selected patient and a videoconferencing link to connect the care team member to a patient device.

16 . The method of claim 6 wherein the graphical user interface displays a patient rounding list.

17 . The method of claim 16 wherein the patient rounding list is dynamically updated as patients are admitted, discharged, or transferred.

18 . A method for monitoring patient care comprising the steps of:

providing a graphical user interface to a medical provider, wherein the graphical user interface displays a first dashboard or a second dashboard based on a criteria, wherein the graphical user interface automatically displays the first dashboard having a patient rounding list when a device is located within a hospital geofence and automatically displays the second dashboard having remote monitoring data when the device is located outside the hospital geofence;

providing an urgent care symptom checker interface to a patient;

receiving a plurality of symptoms from the patient through the urgent care symptom checker interface;

receiving sensor data;

forming a care team by automatically and dynamically selecting a plurality of care team members from a user database based on a location of each user, shift data of each user, role data of each care team member, and the plurality of symptoms;

providing a virtual rounding list to a device of a care team member, wherein the virtual rounding list includes the patient, each patient on the list is selectable to provide information on a selected patient and a videoconferencing link to connect the care team member to a patient device, and wherein the patient rounding list is dynamically updated as patients are admitted, discharged, or transferred;

sending a secure clinical message to at least one of the plurality of care team members when a trigger is met;

receiving a response to the message wherein the response is an escalation response, a forwarding response, or a delegation response;

sending the secure clinical message to a second care provider upon receipt of the forwarding response;

sending a discharge notification to the medical provider; and

providing a discharge questionnaire interface to the patient.

19 . The method of claim 18 wherein the trigger is selected from one of the group of receipt of the plurality of symptoms, receipt of an admission record indicating the patient is being admitted to a hospital, and receipt of sensor data.

The method of claim 18 wherein the criteria is location and the first dashboard, including a patient rounding list, is displayed when the graphical user interface is located at a first location and the second dashboard, including remote monitoring data, is displayed when the graphical user interface is located at a second location.
