Abstract

Occluding structures may be created within a body lumen by advancing a length of material distally through the body lumen. By drawing a distal location on the advanced length of material in a proximal direction, the material may be compacted into a structure which at least partially occludes the lumen. The occluding structure may be used for a variety of purposes, including removing obstructions from the body lumen, such as kidney stones from the ureter; providing hemostasis in a blood vessel; providing occlusion of a fallopian tube; temporary constraint of stone fragments in the urinary tract; capture or restraint of clot in blood vessels; and the like. Apparatus for performing the method may comprise a length of material attached at its distal end to tubular guide or other advancement member. Tensioning members may also be provided for collapsing and compacting the length of material within the body lumen.

Claims (33)

1. A method for engaging an obstruction in a body lumen, said method comprising:

advancing a wire carrying a flat polymeric film distally in the body lumen past the obstruction, wherein the wire penetrates the film and is attached to the film at a distal location thereon;

drawing the distal location on the advanced film proximally to compact the film over the wire into an axially compacted plug structure which at least partially occludes the body lumen, wherein the wire is not compacted; and

engaging a proximal side of the plug structure against the obstruction to move or contain the obstruction.

2. A method as in claim 1 , wherein drawing comprises pulling the wire proximally through the film to compact the film over a distal end of the wire.

3. A method as in claim 2 , further comprising pulling the wire proximally through the body lumen to remove the obstruction from the body lumen.

4. A method as in claim 1 , wherein the compacted plug structure prevents the obstruction from moving distally.

5. A method as in any one of claims 1 , 2 , 3 , and 4 , wherein the body lumen is a ureter, cystic duct, or common bile duct, and the obstruction comprises one or more stones.

6. A method as in claim 5 , wherein the body lumen is a ureter and the flat polymeric film is advanced from urinary bladder into the ureter and past the kidney stone(s).

7. A method as in claim 6 , further comprising directing energy to disrupt the kidney stone(s) while engaged by the compacted plug material.

8. A method as in claim 1 , wherein the body lumen is a blood vessel.

9. A method as in claim 8 , wherein the flat polymeric film is advanced distally past an obstruction and drawn proximally to remove the obstruction.

10. A method as in claim 8 , wherein the compacted plug structure provides substantially complete luminal occlusion.

11. A method as in claim 1 , wherein advancing comprises introducing the flat polymeric film within a tubular guide.

12. A method as in claim 11 , wherein the tubular guide and the flat polymeric film are introduced simultaneously.

13. A method as in claim 11 , wherein the tubular guide is positioned first and the flat polymeric film introduced after the tubular guide has been positioned.

14. A method as in any one of claims 11 to 13 , wherein the tubular guide is at least partially retracted to expose the flat polymeric film within the body lumen before drawing the distal location of the flat polymeric film proximally.

15. A method as in claim 1 , wherein the flat polymeric film has fold structures which impart a preferred folding pattern upon drawing the wire in the proximal direction.

16. A method as in claim 1 , wherein the wire penetrates the film through multiple spaced-apart locations on the film.

17. A method as in claim 1 , wherein the wire penetrates the film through an axial passage in the film.

18. A method for removing kidney stones from a ureter, said method comprising:

advancing a wire carrying a flat polymeric film distally in the body lumen past the obstruction, wherein the wire penetrates the film and is attached to the film at a distal location thereon;

compacting the flat polymeric film over the wire to form an axially compacted plug structure which at least partially occludes the ureter on at least one side of the kidney stone in the ureter; and

directing energy at the kidney stone to break the stone into fragments while the plug structure remains compacted to contain the fragments within the ureter.

19. A method as in claim 18 , wherein the. energy is delivered as part of a lithotripsy procedure.

20. A method as in claim 18 , wherein the energy is laser energy.

21. A method as in claim 18 , wherein the plug structure is compacted on the kidney side of the kidney stone.

22. A method as in claim 21 , further comprising withdrawing the compacted plug structure from the ureter to remove the fragments into the bladder.

23. A method as in claim 18 , wherein compacting the plug structure comprises compacting the flat polymeric film on both the kidney and bladder sides of the kidney stone.

24. A method as in claim 18 , wherein the compacted plug structure occludes the ureter.

25. A method as in claim 18 , further comprising advancing the wire carrying the flat polymeric film from the bladder through the ureter past the kidney stone prior to compacting the film.

26. A method as in claim 18 , wherein the wire penetrates the film through multiple spaced-apart locations on the film.

27. A method as in claim 18 , wherein the wire penetrates the film through an axial passage in the film.
