Abstract

Provided are procedures and instruments for preparing and transplanting osteochondral allograft plugs to a host bone to repair an articular cartilage defect. An allograft bone plug having a cartilage plate and cancellous bone tissue attached thereto is removed from a donor bone. The allograft plug is further shaped by removing or cutting away cancellous bone tissue to form a cancellous stalk extending from the cartilage plate. The formed cancellous stalk can have any suitable shape including cylindrical, conical, and rectilinear. At the recipient site of the host bone, a cutout is formed corresponding in shape to the allograft plug. The allograft plug is inserted into the cutout such that the cancellous stalk is retained in the host bone and the cartilage plate aligns with the condyle surface of the host bone. Aspects of the invention may also be applicable to preparing and transplanting osteochondral autograft plugs.

Claims (21)

1. A transplantable osteochondral allograft plug for repairing a cartilage defect at a recipient site in a host bone, the allograft plug comprising: a cartilage plate having a first perimeter; and a cancellous bone stalk extending from the cartilage plate, the cancellous stalk having a second perimeter reduced in cross-section with respect to the first perimeter, wherein the cartilage plate includes subchondral bone tissue.

2. The osteochondral allograft plug of claim 1 , wherein the first perimeter of the cartilage plate is generally circular.

3. The osteochondral allograft plug of claim 2 , wherein the cancellous stalk tapers with respect to the cartilage plate.

4. The osteochondral allograft plug of claim 2 , wherein the cancellous stalk is conical or cylindrical.

5. The osteochondral allograft plug of claim 1 , wherein the first perimeter of the cartilage plate is generally rectangular.

6. The osteochondral allograft plug of claim 5 , wherein the second perimeter is generally rectangular.

7. The osteochondral allograft plug of claim 1 , wherein the first perimeter of the cartilage plate is generally triangular.

8. The osteochondral allograft plug of claim 1 wherein the cartilage plate defines subchondral bone tissue.

9. The osteochondral allograft plug of claim 8 wherein the cartilage plate defines a first portion formed of the subchondral bone tissue and a second portion formed of cancellous bone tissue.

10. The osteochondral allograft plug of claim 9 wherein the cancellous stalk extends from the second portion.

11. A transplantable osteochondral allograft plug comprising:

a cartilage plate having a first perimeter and formed of subehondral bone tissue and cancellous bone tissue; and a cancellous bone stalk extending from the cancellous bone tissue of the cartilage plate, the cancellous stalk having a proximal end and a distal end, the proximal end attached to the cancellous bone tissue and having a second perimeter reduced in cross-section with respect to the first perimeter.

12. The osteochondral allograft plug of claim 11 , wherein the first perimeter of the cartilage plate is generally circular.

13. The osteochondral allograft plug of claim 11 , wherein the cancellous stalk tapers from the proximal end to the distal end.

14. The osteochondral allograft plug of claim 12 , wherein the cancellous stalk is conical or cylindrical.

15. The osteochondral allograft plug of claim 11 , wherein the first perimeter of the cartilage plate is generally rectangular.

16. The osteochondral allograft plug of claim 15 , wherein the second perimeter is generally rectangular.

17. The osteochondral allograft plug of claim 11 , wherein the first perimeter of the cartilage plate is generally triangular.

18. A transplantable osteochondral allograft plug comprising:

a cartilage plate having a first diameter and formed of subchondral bone tissue and cancellous bone tissue; and a cancellous bone stalk extending from the cancellous bone tissue of the cartilage plate, the cancellous stalk having a proximal end and a distal end, the proximal end attached to the cancellous bone tissue and having a second diameter smaller than the first diameter; and wherein the cancellous stalk tapers from the proximal end to the distal end.

19. The osteochondral allograft plug of claim 18 , wherein an axial length of the cancellous stalk is greater than an axial length of the cartilage plate.
