Abstract

Claims (75)

1. A computer implemented method for managing patient medication data comprising:

receiving, by a computing system, medication data for a patient from multiple sources;

reconciling, by the computing system, the medication data from multiple sources to generate a reconciled list of medications for the patient;

presenting the reconciled list of medications to the patient in a user-interface associated with the computing system;

identifying, by the computing system, a potential cost savings change for a prescribed medication on the reconciled list of medications;

presenting an indication of the cost savings change to the patient in the user-interface;

receiving a selection of the potential cost savings for the prescribed medication from the patient through the user interface;

in response to receiving, from the patient through the user interface, the selection of the potential cost savings,

transmitting, by the computing system via a network, a request for the cost savings change to a health-care professional, and

accumulating, by the computing system on behalf of the patient, reward points to be redeemed by the patient for savings on co-payment;

receiving, by the computing system from the health-care professional, an approval of the cost savings and an indication of an updated prescription according to the cost savings;

updating, by the computing system, the reconciled list of medications according to the approval and the indication of the updated prescription to generate an updated medication list; and

presenting to the patient the updated medication list in the user-interface.

2. The method of claim 1 , further comprising:

in response to the receiving an indication of an updated prescription, presenting, to the patient on the updated medication list in the user interface, the indication of the updated prescription.

3. The method of claim 1 , further comprising:

receiving, by the computing system, claims data indicating that the updated prescription has been purchased,

wherein presenting the updated medication list is in response to the receiving claims data indicating that the prescription has been purchased.

4. The method of claim 3 , further comprising transmitting, by the computing system, the updated prescription to a pharmacy.

5. The method of claim 3 , wherein presenting the updated medication list in the user interface comprises adding, by the computing system, a medication to the updated medication list corresponding to the prescription and removing, by the computing system, a medication from the updated medication list that was replaced by the prescription.

6. The method of claim 5 , further comprising receiving a confirmation, from the patient through the user interface, that the patient has discontinued taking the removed medication.

7. The method of claim 1 , wherein the cost savings comprises a therapeutic alternative.

8. The method of claim 1 , wherein the cost savings comprises a generic alternative.

9. A non-transitory computer readable medium encoded with instructions that when executed by a data processing apparatus cause the data processing apparatus to perform operations comprising:

receiving medication data for a patient from multiple sources;

reconciling the medication data from multiple sources to generate a reconciled list of medications for the patient;

presenting the reconciled list of medications to the patient in a user-interface associated with the data processing apparatus;

identifying a potential cost savings change for a prescribed medication on the reconciled list of medications;

presenting an indication of the cost savings change to the patient in the user-interface;

receiving a selection of the potential cost savings for the prescribed medication from the patient through the user interface;

in response to receiving, from the patient through the user interface, the selection of the potential cost savings,

transmitting via a network a request for the cost savings change to a health-care professional, and

accumulating, on behalf of the patient, reward points to be redeemed by the patient for savings on co-payments;

receiving, from the health-care professional, an approval of the cost savings and an indication of an updated prescription according to the cost savings;

updating the reconciled list of medications according to the approval and the indication of the updated prescription to generate an updated medication list; and

presenting to the patient the updated medication list in the user-interface.

10. The non-transitory computer readable medium of claim 9 , wherein the instructions when executed by the data processing apparatus cause the data processing apparatus to perform operations further comprising:

in response to the receiving an indication of an updated prescription, presenting, to the patient on the updated medication list in the user interface, the indication of the updated prescription.

11. The non-transitory computer readable medium of claim 9 , wherein the instructions when executed by the data processing apparatus cause the data processing apparatus to perform operations further comprising:

receiving claims data indicating that the updated prescription has been purchased,

wherein presenting the updated medication list is in response to the receiving claims data indicating that the prescription has been purchased.

12. The non-transitory computer readable medium of claim 11 , wherein the instructions when executed by the data processing apparatus cause the data processing apparatus to perform operations further comprising:

transmitting the updated prescription to a pharmacy.

The non-transitory computer readable medium of claim 11 , wherein presenting the updated medication list in the user interface comprises adding a medication to the updated medication list corresponding to the prescription and removing a medication from the updated medication list that was replaced by the prescription.

14. The non-transitory computer readable medium of claim 13 , wherein the instructions when executed by the data processing apparatus cause the data processing apparatus to perform operations further comprising:

receiving a confirmation, from the patient through the user interface, that the patient has discontinued taking the removed medication.

15. The non-transitory computer readable medium of claim 9 , wherein the cost savings comprises a therapeutic alternative.

16. The non-transitory computer readable medium of claim 9 , wherein the cost savings comprises a generic alternative.

17. A system for managing patient medication data comprising,

a data storage device for storing medication data received from multiple sources over one or more networks;

a computing system including processor electronics configured to perform operations comprising:

receiving and storing medication data for a patient from multiple sources;

reconciling the medication data from multiple sources to generate a reconciled list of medications for the patient;

presenting the reconciled list of medications to the patient in a user-interface associated with the processor electronics;

identifying a potential cost savings change for a prescribed medication on the reconciled list of medications;

presenting an indication of the cost savings change to the patient in the user-interface;

receiving a selection of the potential cost savings for the prescribed medication from the patient through the user interface;

in response to receiving, from the patient through the user interface, the selection of the potential cost savings,

transmitting via a network a request for cost savings change to a health-care professional, and

accumulating, on behalf of the patient, reward points to be redeemed by the patient for savings on co-payment; and

receiving, from the health-care professional, an approval of the cost savings and an indication of an updated prescription according to the cost savings;

updating the reconciled list of medications according to the approval and the indication of the updated prescription to generate an updated medication list; and

presenting to the patient the updated medication list in the user-interface.

18. The system of claim 17 , wherein the processor electronics are further configured to perform the operations comprising:

in response to the receiving an indication of the updated prescription, presenting, to the patient on the updated medication list in the user interface, the indication of the updated prescription.

19. The system of claim 17 , wherein the processor electronics are further configured to perform the operations comprising:

receiving claims data indicating that the updated prescription has been purchased,

wherein presenting the updated medication list is in response to the receiving claims data indicating that the prescription has been purchased.

20. The system of claim 19 , wherein the processor electronics are further configured to perform the operations comprising:

transmitting the updated prescription to a pharmacy.

21. The system of claim 19 , wherein presenting the updated medication list in the user interface comprises adding a medication to the updated medication list corresponding to the prescription and removing a medication from the updated medication list that was replaced by the prescription.

22. The system of claim 21 , wherein the processor electronics are further configured to perform the operations comprising:

receiving a confirmation, from the patient through the user interface, that the patient has discontinued taking the removed medication.

23. The system of claim 17 , wherein the cost savings comprises a therapeutic alternative.

24. The system of claim 17 , wherein the cost savings comprises a generic alternative.
