Adjustable suture button construct and methods of tissue reconstruction
An adjustable construct for fixation of a tendon or ligament graft in a tunnel. The construct comprises a flexible, adjustable loop connected to tissue (for example, graft, ligament or tendon). The adjustable loop may be integrated with an additional tissue supporting device (for example, a wedge or a plug). The tissue is secured within a bone socket/tunnel by adjusting the length of the flexible adjustable loop.
1. A method of tissue reconstruction, comprising:
providing a construct comprising an adjustable button/loop construct having a button and one flexible, adjustable loop connected to the button, and a graft connected to the flexible, adjustable loop,
the flexible, adjustable loop having an adjustable length, the flexible, adjustable loop having two splices, the flexible, adjustable loop being formed by a first adjustable eyesplice loop and a second adjustable eyesplice loop that are interconnected at an interconnection where the first adjustable eyesplice loop passes through and interlocks with the second adjustable eyesplice loop, the interconnection and the two splices being located on opposite ends of the flexible, adjustable loop;
inserting the construct within a first bone tunnel in a first bone, and a second bone tunnel in an second bone; and
adjusting the length of the flexible, adjustable loop to secure the graft within the first and second bone tunnels.
2. The method of claim 1 , further comprising:
attaching the flexible, adjustable loop to the button by positioning the button over the interconnection;
passing the graft through the flexible, adjustable loop and looping the graft over the flexible, adjustable loop to form a looped graft;
advancing the construct through the first bone tunnel such that when the button exits the first bone, the button pivots and engages a cortical surface of the first bone; and
adjusting the length of the flexible, adjustable loop to position the looped graft within the first bone tunnel.
3. The method of claim 2 , further comprising securing the looped graft within the second bone tunnel.
4. The method of claim 1 , wherein the graft is soft tissue, tendon, ligament, synthetic material, biological material, bone, or a combination of such materials.
5. The method of claim 1 , wherein the construct is formed by:
providing a suture strand having a first end and a second end;
attaching the first end to a first suture passing device;
passing the first suture passing device through the center of the suture strand to form a first splice and the first adjustable eyesplice loop with the first end;
sliding the button over the first and second ends so that the button rests over the first splice;
attaching the second end to a second suture passing device; and
subsequently, passing the second suture passing device through the center of the suture strand and through the first adjustable eyesplice loop to form a second splice and the second adjustable eyesplice loop with the second end.
6. The method of claim 5 , wherein each of the first and second suture passing devices is a needle.
