Abstract

A surgical instrument is inserted through a guide tube. The surgical instrument exits at an intermediate position of the guide tube and is oriented to be substantially parallel to the guide tube's longitudinal axis as it exits. A stereoscopic image capture component is on the guide tube between the intermediate position and the guide tube's distal end. The image capture component's field of view is generally perpendicular to the guide tube's longitudinal axis. The guide tube is jointed to allow the image capture component to be moved. The surgical instruments and the guide tube are telemanipulatively controlled.

Claims (56)

1. A surgical instrument assembly comprising:

a first body segment having a proximal end, a distal end, and a longitudinal axis defined between the proximal and distal ends:

a parallel motion mechanism comprising a first joint, a second joint, a second body segment, and a first set of cables, the first joint, the second body segment, and the second joint being connected in series,

the first joint being coupled to the distal end first body segment,

the first joint comprising a first hinge having a first hinge pivot,

the second joint comprising a second hinge having a second hinge pivot, and

the first set of cables each having a first end connected to the parallel motion mechanism proximal of the first hinge pivot, and each having a second end connected to the parallel motion mechanism distal of the second hinge pivot;

a second set of cables extending through the first body segment to the parallel motion mechanism, each of the second set of cables having a distal end connected to the parallel motion mechanism distal of the first hinge pivot;

a wrist joint coupled to the second joint of the parallel motion mechanism; and

a distal end component coupled to the wrist joint, wherein the first set of cables is coupled in the parallel motion mechanism to allow the parallel motion mechanism to heave and sway the distal end component with reference to the longitudinal axis of the first body segment without changing pitch and yaw of the distal end component with reference to the longitudinal axis of the first body segment.

2. The surgical instrument assembly of claim 1 :

wherein the first body segment is a body segment of a surgical instrument; and

wherein the distal end component comprises an end effector.

3. The surgical instrument assembly of claim 2 , further comprising:

a transmission mechanism coupled to each of the second set of cables, the transmission mechanism being positioned proximal to the first body segment, and the transmission mechanism configured to apply a tensile force on one or more of the cables in the second set of cables to change the heave or the sway of the end effector.

4. The surgical instrument assembly of claim 2 , further comprising:

a guide tube comprising a proximal end, a distal end, and an intermediate position between the proximal and distal ends,

wherein the surgical instrument passes through the guide tube, and

wherein the entire parallel motion mechanism of the surgical instrument exits and extends out of the guide tube at the intermediate position; and

a stereoscopic image capture component positioned on the guide tube between the intermediate position and the distal end of the guide tube, wherein a field of view of the image capture component is generally perpendicular to a longitudinal axis of the guide tube near the image capture component.

5. The surgical instrument assembly of claim 4 , further comprising:

a second surgical instrument, wherein the second surgical instrument passes through the guide tube, and wherein the second surgical instrument exits the guide tube at the intermediate position.

6. The surgical instrument assembly of claim 5 :

wherein the second surgical instrument comprises a parallel motion mechanism; and

wherein the parallel motion mechanism of the second surgical instrument heaves and sways a distal end of the second surgical instrument without changing pitch and yaw of the distal end of the second surgical instrument.

7. The surgical instrument assembly of claim 4 :

wherein the guide tube comprises a joint that allows the image capture component to move while the intermediate position remains stationary.

8. The surgical instrument assembly of claim 4 , further comprising:

a second surgical instrument fixed to the guide tube at the intermediate position.

9. The surgical instrument assembly of claim 8 :

wherein the second surgical instrument comprises a parallel motion mechanism.

10. The surgical instrument assembly of claim 1 , further comprising:

a guide tube comprising a proximal end and a distal end, wherein the distal end of the first body segment is the distal end of the guide tube.

11. The surgical instrument assembly of claim 10 , wherein the distal end component comprises an endoscopic image capture component.

12. The surgical instrument assembly of claim 10 , further comprising:

a transmission mechanism coupled to each of the second set of cables, the transmission mechanism being positioned proximal to the guide tube, and the transmission mechanism configured to apply a tensile force on one or more of the cables in the second set to change the heave or sway of the distal end component.

13. The surgical instrument assembly of claim 10 , wherein the guide tube further comprises an intermediate position between the proximal and distal ends, the surgical instrument assembly further comprising:

a surgical instrument, wherein the surgical instrument passes through the guide tube, and wherein the surgical instrument exits the guide tube at the intermediate position.

14. The surgical instrument assembly of claim 13 :

wherein the surgical instrument comprises a parallel motion mechanism, the entire parallel motion mechanism being distal to the intermediate position; and

wherein the parallel motion mechanism of the surgical instrument heaves and sways a distal end of the surgical instrument without changing pitch and yaw of the distal end of the second surgical instrument.

15. A surgical instrument assembly comprising:

a first body segment having a longitudinal axis;

a parallel motion mechanism coupled to the first body segment, the parallel motion mechanism comprising:

a serial combination of a first joint comprising a first hinge having a first hinge pivot, a second body segment, and a second joint comprising a second hinge having a second hinge pivot, the first joint being coupled to the first body segment; and

a first set of cables connected to said parallel motion mechanism proximal to the first hinge pivot, and connected to said parallel motion mechanism distal to the second hinge pivot;

a wrist joint coupled to the second joint of the parallel motion mechanism;

a distal end component coupled to the wrist joint;

a second set of cables connected to the parallel motion mechanism distal to the first hinge pivot; and

a transmission mechanism coupled to each of the second set of cables, the transmission mechanism being positioned proximal to the first body segment, and the transmission mechanism configured to apply a tensile force on one or more of the cables in the second set to change heave or sway of the distal end component with reference to the longitudinal axis of the first body segment without changing pitch or yaw of the distal end component.

16. The surgical instrument assembly of claim 15 :

wherein the first body segment is a body segment of a surgical instrument; and

wherein the distal end component comprises an end effector.

17. The surgical instrument assembly of claim 15 :

wherein the first body segment is a distal end of a guide tube; and

wherein the distal end component comprises an endoscopic image capture component.
