Abstract

A tool for assisting in the planning or performing of electrical neuromodulation of a patient's spinal cord. The tool may have various functions and capabilities, including calculating a volume of activation, registering an electrode(s) shown in a radiologic image, constructing functional images of the patient's spinal anatomy, targeting of neuromodulation, finding a functional midline between multiple electrodes, determining the three-dimensional position of multiple electrodes, and/or accommodating for electrode migration. In certain embodiments, the tool can be embodied as computer software or a computer system.

Claims (62)

1. A method for selecting stimulation settings, comprising:

individually activating a plurality of contacts of a leadwire implanted adjacent to an anatomic region of a patient;

comparing, by a computer processor, a parasthesia area respectively produced by each of the activated contacts to a target area of the patient's body;

based on the comparison, selecting, by the processor, a set of contacts and associated settings that provide the parasthesia areas that best match the target area; and

determining and outputting, by the processor, a combined set of settings that includes activations of the selected contacts, wherein an activation of at least one of the selected contacts is adjusted relative to the activation of the at least one contact in the individually activating step.

2. The method of claim 1 , wherein the comparing step includes determining at least one of a distance between the respective parasthesia area and the target area and a degree of overlap between the respective parasthesia area and the target area.

3. The method of claim 2 , wherein the adjustment is based on results of the comparing step.

4. The method of claim 3 , wherein the determining includes interpolating the selected settings.

5. A system for selecting stimulation settings, comprising:

a computer processor configured to:

provide signals for individually activating a plurality of contacts of a leadwire implanted adjacent to an anatomic region of a patient;

compare a parasthesia area respectively produced by each of the activated contacts to a target area of the patient's body;

based on the comparison, select a set of contacts and associated settings that provide at least two of the parasthesia areas that, of all of the compared parasthesia areas, best match the target area; and

determine and output a combined set of settings that includes activations of the selected contacts, wherein an activation of at least one of the selected contacts is adjusted relative to the activation of the at least one contact in the individually activating step.

6. A non-transitory computer-readable medium on which are stored instructions executable by a computer processor, the instructions which, when executed by the processor, cause the processor to perform a method for selecting stimulation settings, the method comprising:

individually activating a plurality of contacts of a leadwire implanted adjacent to an anatomic region of a patient;

comparing a parasthesia area respectively produced by each of the activated contacts to a target area of the patient's body;

based on the comparison, selecting a set of contacts and associated settings that provide the parasthesia areas that, of all of the compared parasthesia areas, best match the target area; and

determining and outputting a combined set of settings that includes activations of the selected contacts, wherein an activation of at least one of the selected contacts is adjusted relative to the activation of the at least one contact in the individually activating step.

7. A method for selecting stimulation settings, comprising:

obtaining, by a computer processor, first settings for a leadwire implanted adjacent to an anatomic region of a patient, the first settings being associated with production of parasthesia in a first location in the patient's body;

obtaining, by the processor, second settings for the leadwire, the second settings being associated with production of parasthesia in a second location in the patient's body; and

based on the obtaining of the first and second settings, determining, by the processor, a set of settings, non-additive of the first and second settings and estimated by the processor to produce parasthesia in both the first and the second locations.

8. A method for selecting stimulation settings, comprising:

obtaining, by a computer processor, first settings for a leadwire implanted adjacent to an anatomic region of a patient, the first settings estimated by the processor to produce a first volume of activation;

obtaining, by the processor, second settings for the leadwire, the second settings estimated by the processor to produce a second volume of activation;

overlapping, by the processor, the first and second estimated volumes of activation to produce a combined estimated volume of activation; and

determining, by the processor settings that produce the combined estimated volume of activation.

9. A method for selecting stimulation settings, comprising:

obtaining, by a computer processor, first settings for a leadwire implanted adjacent to an anatomic region of a patient, the first settings being associated with production of parasthesia in a first location of the patient's body;

obtaining, by the processor, second settings for the leadwire, the second settings being associated with production of parasthesia in a second location of the patient's body;

obtaining, by the processor, a new set of settings for targeting both the first and second locations by performing the following for each electrode of the leadwire activated by either of the first and second settings:

comparing settings of the respective electrode included in the first settings and settings of the respective electrode included in the second settings; and

selecting those of the compared settings that produces a larger area of tissue activation about the respective electrode.

10. A method for generating stimulation settings, comprising:

obtaining a functional midline for at least one leadwire implanted adjacent an anatomic region of a patient;

obtaining an identification of a target area of the patient's body relative to an actual midline of the patient, wherein the target area is located to one side of the actual midline of the patient;

responsive to the identified target area and based on the obtained functional midline, selecting, by a computer processor, stimulation settings determined by the processor to produce parasthesia on a side of the functional midline that is the same as the side of the patient's actual midline at which the target area is located; and

further adjusting, by the processor, the stimulation settings based on feedback from the patient to bring a location of the parasthesia closer to the target area.

11. The method of claim 10 , wherein the further adjusting includes:

determining a direction in which the parasthesia location deviates from the target area; and

adjusting the stimulation settings to shift the parasthesia location in a direction opposite the deviation.

12. The method of claim 11 , wherein the parasthesia location is shifted in accordance with a binary searching algorithm.

13. A system for generating stimulation settings, comprising:

a computer processor configured to:

obtain a functional midline for at least one leadwire implanted adjacent an anatomic region of a patient;

obtain an identification of a target area of the patient's body relative to an actual midline of the patient, wherein the target area is located to one side of the actual midline of the patient;

responsive to the identified target area and based on the obtained functional midline, select stimulation settings determined by the processor to produce parasthesia on a side of the functional midline that is the same as the side of the patient's actual midline at which the target area is located; and

further adjust the stimulation settings based on feedback from the patient to bring a location of the parasthesia closer to the target area.

14. A non-transitory computer-readable medium on which are stored instructions executable by a computer processor, the instructions which, when executed by the processor, cause the processor to perform a method for generating stimulation settings, the method comprising:

obtaining a functional midline for at least one leadwire implanted adjacent an anatomic region of a patient;

obtaining an identification of a target area of the patient's body relative to an actual midline of the patient, wherein the target area is located to one side of the actual midline of the patient;

responsive to the identified target area and based on the obtained functional midline, selecting stimulation settings determined by the processor to produce parasthesia on a side of the functional midline that is the same as the side of the patient's actual midline at which the target area is located; and

further adjusting the stimulation settings based on feedback from the patient to bring a location of the parasthesia closer to the target area.

15. The computer-readable medium of claim 14 , wherein the anatomic region of the patient is the patient's spinal cord.

16. The method of claim 1 , wherein the anatomic region of the patient is the patient's spinal cord.

17. The system of claim 5 , wherein the anatomic region of the patient is the patient's spinal cord.

18. The computer-readable medium of claim 6 , wherein the anatomic region of the patient is the patient's spinal cord.

19. The method of claim 7 , wherein the anatomic region of the patient is the patient's spinal cord.

20. The method of claim 10 , wherein the anatomic region of the patient is the patient's spinal cord.

21. The system of claim 13 , wherein the anatomic region of the patient is the patient's spinal cord.

22. The method of claim 10 , wherein the functional midline is a line that extends between two points, each of the points being at a respective location between a respective pair of contacts that corresponds to respective amplitude settings of the respective pair of contacts that is identified as producing parasthesia at a center of the patient's body.
