Abstract

Medical delivery devices include a needle and a flexible compressible impermeable tube attached to the needle. The compressible tube has an open interior channel. The device is adapted to releasably hold a length of a medical implant in the open interior channel of the tube. The medical implant can be slidably inserted into the interior channel of the tube and is loosely held by the tube.

Claims (26)

1. A medical delivery device, comprising:

a needle;

a tube comprising a first segment that is fixedly attached to the needle and a second compressible, flexible, and impermeable segment that increases in diameter relative to the first segment and extends rearward of the needle such that a maximum diameter of the second segment is greater than a maximum diameter of the first segment and a maximum diameter of the needle, wherein, prior to and during surgical use, the second segment defines an open elongate interior channel with an open trailing end; and

a flexible medical implant having a length with a leading end and an opposing trailing end, wherein the leading end is held as a free end inside the tube spaced apart a distance from a trailing end of the needle, wherein the leading end is not directly or indirectly attached to the tube or any needle, and wherein between 20-100% of the flexible medical implant length is held in the open elongate interior channel of the tube;

wherein the flexible medical implant is releasably and loosely held inside the tube such that the flexible medical implant is not in snug abutting contact with an inner surface of the tube thereby enabling the flexible medical implant to entirely slidably exit the open trailing end of the tube when the flexible medical implant is disposed within soft tissue during surgical use and when the needle and the tube are pulled out of and away from the soft tissue; and

wherein the tube is configured so that compression of a portion of the second segment caused by contact with the soft tissue during implantation temporarily compresses the tube such that only this compression tightly holds the implant therein.

A medical delivery device according to claim 1 wherein the implant length held in the interior channel of the tube is at least 3 cm and/or is at least 30% of the implant length, wherein the flexible medical implant is slidably insertable into the tube interior channel and is loosely held inside the tube interior channel.

3. A medical delivery device according to claim 2 , wherein the flexible medical implant is a partially hydrated or dry twisted or braided collagen fiber implant that is loosely held inside the interior channel of the tube second segment.

A medical delivery device according to claim 2 , wherein the flexible medical implant held in the second segment of the tube comprises a plurality of fibers derived from soluble collagen that are in a dry or partially hydrated state and arranged as parallel long fibers that are at least 1 cm long and are twisted and/or woven or braided together.

5. A medical delivery device according to claim 4 , wherein the fibers are NDGA-polymerized collagen fibers.

A medical delivery device according to claim 5 , wherein the fibers have a cross-sectional size, on average when dry, of between 0.001 inches to 0.2 inches, wherein the implant has a number of strands of fibers that is between 2-100, and wherein the number of fibers per strand is between 2 to 1200.

7. A medical delivery device according to claim 1 , wherein the tube has a forward end portion that resides in an interior chamber of a rearward portion of the needle.

8. A medical delivery device according to claim 1 , wherein the tube is swaged to the needle and has a smooth transition segment from the needle to the tube to inhibit snagging when inserted through the soft tissue during surgical use.

A medical delivery device according to claim 1 , wherein the needle has a length, and wherein the first segment of the compressible tube extends snugly against at least 1 cm of the length of the needle, wherein the flexible medical implant comprises an elongate flexible braided or twisted collagen fiber implant with at least 30% of its length held in a substantially straight arrangement inside the second segment of the tube.

A medical delivery device according to claim 1 , wherein the needle is shank-less and has a constant diameter outer wall upstream from a tip portion thereof, and wherein an inner wall of the first segment of the tube is affixed to the outer wall of the needle, wherein the flexible medical implant comprises a dry or partially hydrated collagen fiber implant, and wherein the tube second segment has a length that is sufficient to encase at least 30% of the implant therein.

11. A medical delivery device according to claim 10 , wherein the tube is heat-shrink tubing that is heated to shrink to conform and affix to an outer wall of the needle, and wherein the second segment holds at least 40% of the implant length therein.

A medical delivery device according to claim 1 , wherein the flexible medical implant is a tendon or ligament repair implant having an integrated suture that has free suture ends that extend out from each end of the implant with at least 30% of the implant length being held in the second segment of tube.

A medical delivery device according to claim 1 , wherein the flexible medical implant held in the interior channel of the second segment of the tube is a length of dry or partially hydrated biocompatible collagen fibers twisted or braided together that swell when exposed to a local fluidic environment in situ.

14. A medical delivery device according to claim 1 , wherein the flexible medical implant is sized and configured as a ligament repair device.

15. A medical delivery device according to claim 1 , wherein the flexible medical implant is sized and configured as a tendon repair device.

16. A medical delivery device according to claim 1 , wherein the flexible medical implant is at least one of a suture or a cable that is configured to repair an acromioclavicular (AC) joint.

The medical delivery device of claim 1 , wherein the flexible medical implant is held inside the tube interior channel so that the leading end is spaced apart from a trailing end of the needle to not contact the needle and wall of the tube encasing the interior channel between the trailing end of the needle and the leading end of the flexible implant.

18. A medical kit for a ligament or tendon repair, comprising:

a medical delivery device of claim 1 , wherein the flexible medical implant is a length of tendon or ligament repair or replacement material comprising dry or partially hydrated synthetic collagen fibers.

19. A medical kit according to claim 18 , wherein the repair or replacement material comprises a twisted or braided construct of a plurality of collagen fibers, and wherein the medical delivery device and the repair or replacement material are held in at least one sterile package.

20. The medical kit of claim 18 , wherein the flexible medical implant is held inside the tube interior channel so that the leading end is spaced apart from a trailing end of the needle to not contact the needle and wall of the tube encasing the interior channel between the trailing end of the needle and the leading end of the flexible implant.
