9. The spinal fixation system of claim 7 , wherein the second subassembly includes a threaded portion that is configured to engage a portion of the at least one of the first and second plates of the second clamp to advance one of the first and second plates of the second clamp towards the other.

10. The spinal fixation system of claim 6 , wherein the offset location of the receiving portions of the first and second clamps is such that when portions of the spinal rod are disposed therein, the spinal rod is offset from the openings of the first and second plates of the first and second clamps.

11. The spinal fixation system of claim 6 , wherein at least one of the first and second clamps can be slid along the spinal rod to change a distance between the first and second clamps.

12. A spinal fixation system, comprising:

a bone anchor having a bone-engaging portion and a spinal fixation element receiving portion with opposed arms configured to receive a spinal fixation element therebetween;

a connecting member having first and second ends with a spanning portion extending therebetween;

a connecting assembly having a distal surface that bears against a proximal terminal end surface of each of the opposed arms of the rod receiving portion of the bone anchor and a receiving portion that is configured to receive a portion of the connecting member.

13. The system of claim 12 , wherein the receiving portion of the connecting assembly is offset from the rod receiving portion of the bone anchor.

14. The system of claim 12 , wherein the connecting assembly further comprises a first plate disposed above a second plate, the two plates each having an opening extending therebetween, the two plates being configured such that as one plate advances towards the other plate, the receiving portion of the connecting assembly tightens around a portion of the connecting member disposed therein.

15. The system of claim 14 , wherein the connecting assembly further comprises a threaded portion that is configured to engage a portion of at least one of the first and second plates to advance one of the first and second plates towards the other.

16. The system of claim 12 , wherein the connecting member is a spinal rod of a substantially uniform thickness.

17. The system of claim 12 , wherein the connecting assembly is configured to lock both a location of a spinal fixation element with respect to the spinal fixation element receiving portion of the bone anchor and a location of the connecting member with respect to the connecting assembly.

18. The system of claim 12 , further comprising:

a second bone anchor having a bone-engaging portion and a spinal fixation receiving portion with opposed arms configured to receive a spinal fixation element therebetween; and

a second connecting assembly having a distal surface that bears against a proximal terminal end surface of each of the opposed arms of the rod receiving portion of the bone anchor and a receiving portion that is configured to receive a portion of the connecting member.

19. The system of claim 18 , wherein the receiving portions of each of the first and second connecting assemblies are offset from the rod receiving portions of the respective first and second bone anchors, and the connecting member is also offset from the rod receiving portions of the first and second bone anchors.

20. The system of claim 18 , wherein at least one of the first and second connecting assemblies can be slid along the connecting member to change a distance between the first and second connecting assemblies.
