Abstract

Microwave catheter apparatuses, systems, and methods for achieving renal neuromodulation by intravascular access are disclosed herein. One aspect of the present application, for example, is directed to apparatuses, systems, and methods that incorporate a catheter treatment device comprising an elongated shaft. The elongated shaft is sized and configured to deliver a microwave transmission element to a renal artery via an intravascular path. Renal neuromodulation may be achieved via dielectric heating in the presence of microwave irradiation that modulates neural fibers that contribute to renal function or alters vascular structures that feed or perfuse the neural fibers.

Claims (21)

1. A method for treatment of a human patient via renal denervation, the method comprising:

intravascularly positioning a catheter having a microwave transmitting element within a renal artery of the patient, wherein the microwave transmitting element is within an expandable centering element;

centering the microwave transmitting element within the renal artery via the expandable centering element,

wherein the expandable centering element is transformable between a low-profile delivery configuration and an expanded configuration within the renal artery, and further wherein, in the expanded configuration, the expandable centering element is sized and shaped to occlude blood flow through the renal artery;

omni-directionally radiating microwave energy from a microwave generator external to the patient via the microwave transmitting element to at least partially ablate renal nerves along the renal artery of the patient,

wherein the microwave energy is delivered omni-directionally in a plane relatively perpendicular to a longitudinal axis of the renal artery via the microwave transmitting element; and

actively cooling the microwave transmitting element while radiating the microwave energy to the target renal nerves.

2. The method of claim 1 wherein omni-directionally radiating microwave energy via the microwave transmitting element to at least partially ablate the renal nerves comprises dielectrically heating the renal nerves.

3. The method of claim 2 wherein dielectrically heating the renal nerves comprises inducing necrosis in the renal nerves.

4. The method of claim 1 wherein intravascularly positioning a catheter having a microwave transmitting element comprises intravascularly positioning a catheter having a microwave antenna.

5. The method of claim 4 wherein intravascularly positioning a catheter having a microwave antenna comprises positioning a catheter having a coaxial antenna.

6. The method of claim 5 wherein the coaxial antenna includes a radiating element, and wherein radiating microwave energy from a microwave generator external to the patient via the microwave transmitting element comprises dynamically varying a length of the radiating element.

7. The method of claim 5 wherein the coaxial antenna includes a radiating element, and wherein radiating the microwave energy from a microwave generator external to the patient via the microwave transmitting element further comprises shielding a portion of the radiating element of the coaxial antenna to preferentially direct the radiated microwave energy.

8. The method of claim 1 wherein omni-directionally radiating microwave energy from a microwave generator external to the patient via the microwave transmitting element comprises transferring the microwave energy through a coaxial cable to the coaxial antenna.

9. The method of claim 1 wherein intravascularly positioning a catheter having a microwave transmitting element within the renal artery comprises advancing the catheter to the renal artery over a guide wire via an intravascular path.

10. The method of claim 1 wherein actively cooling the microwave transmitting element comprises circulating a coolant within the expandable element surrounding the microwave transmitting element.

11. The method of claim 1 , further comprising monitoring a temperature of the microwave transmitting element and/or tissue of the patient while radiating microwave energy.

12. The method of claim 11 , further comprising altering the active cooling or the microwave energy in response to the monitored temperature.

13. The method of claim 1 , further comprising monitoring a temperature of the microwave transmitting element.

14. The method of claim 1 wherein at least partially ablating the renal nerves comprises at least partially denervating a kidney of the patient.

15. The method of claim 1 wherein at least partially ablating the renal nerves comprises thermally ablating the renal nerves from within the renal blood vessel.
