Abstract

The present invention relates to an apparatus and method for near-simultaneous and integrated delivery of bone graft material during the placement of surgical cages or other medical implants in a patient's spine. The integrated fusion cage and graft delivery device according to various embodiments delivers and disperses biologic material through a fusion cage to a disc space and, without withdrawal from the surgical site, may selectably detach the fusion cage for deposit to the same disc space. The integrated fusion cage and graft delivery device is formed such that a hollow tube and plunger selectively and controllably place bone graft material and a fusion cage in or adjacent to the bone graft receiving area. In one embodiment, the integrated fusion cage is an expandable integrated fusion cage.

Claims (39)

1. A bone graft delivery system, comprising:

an elongate hollow tube constructed to receive bone graft material, said elongate hollow tube having an extended axis, a proximal end, a distal end, and a lumen extending from said proximal end to said distal end;

an implant detachably interconnected to said distal end of said elongate hollow tube, said implant having a distal tip having a tapered surface and having at least two lateral openings, wherein said implant has a first textured surface for engaging a first vertebrae and a second textured surface for engaging a second vertebrae;

a handle operatively associated with said elongate hollow tube; and

a plunger adapted to extend in said elongate hollow tube, said plunger having an exterior surface contoured to form a substantially congruent fit with said lumen of said elongate hollow tube, wherein said elongate hollow tube and said plunger are configured to deliver bone graft material to said implant so that the bone graft material exits through said at least two lateral openings of said implant.

2. The system of claim 1 , wherein said implant includes: a radiographic marker; and wherein said first textured surface and said second textured surface include a series of sawtooth-like ridges.

3. The system of claim 1 , wherein said implant comprises metal or a polymer comprising thermoplastic polycarbonate.

4. The system of claim 1 , wherein said elongate hollow tube includes one or more of:

a cylindrical shape; and

marks configured to indicate how far said elongate hollow tube has been inserted into a surgical site.

5. A bone graft delivery system comprising:

a handle;

an elongate hollow tube constructed to receive bone graft material, said elongate hollow tube having an extended axis, a proximal end, a distal end, and an interior cross-section, said elongate hollow tube configured to be coupled to said handle;

a plunger configured to be received in said elongate hollow tube, said plunger adapted to urge bone graft material through said elongate hollow tube; and

an interbody device coupled to said distal end of said elongate hollow tube, said interbody device having at least two openings configured to deliver bone graft material, said interbody device having a first surface and a second surface comprising a series of lateral saw tooth-like ridges, said first surface for engaging a first vertebrae and said second surface for engaging a second vertebrae.

6. The system of claim 5 , wherein said elongate hollow tube has a cylindrical shape.

7. The system of claim 5 , wherein said bone graft delivery system is configured to position said interbody device within a disc space, and wherein said bone graft delivery system is adapted to deliver a volume of the bone graft material to the disc space that is at least approximately two times greater than a volume of disk material removed from the disc space.

8. The system of claim 5 , wherein said interbody device comprises a radiographic marker.

9. The system of claim 5 , wherein said elongate hollow tube is flexible.

10. The system of claim 5 , wherein said interbody device has a distal end with a bullet shape.

11. The system of claim 5 , wherein said plunger has a distal end exterior surface contoured to form a substantially congruent fit with said elongate hollow tube interior cross-section.

12. The system of claim 5 , wherein said plunger is precluded from rotating when inserted into said elongate hollow tube.

13. The system of claim 5 , wherein said elongate hollow tube is rigid.

14. The system of claim 5 , wherein said plunger is flexible.

15. The system of claim 5 , wherein both said plunger and said elongate hollow tube are flexible.

16. The system of claim 5 , wherein said interbody device is configured to be coupled to said elongate hollow tube before said interbody device has been placed within a disc space, and wherein said interbody device couples to said elongate hollow tube by one or more of a press fit, a bayonet mount, a tongue and groove, male/female interlocking structures, a clamp, and a break-off collar.

17. A method for delivering bone graft material to a disc space, comprising:

providing a bone graft delivery device comprising an elongate tube and a plunger, the elongate tube constructed to receive bone graft material and having an extended axis from a proximal end to a distal end, the plunger configured to deliver bone graft material through the elongate tube;

coupling the elongate tube of the bone graft delivery device to an interbody device, the interbody device including a first textured surface, a second textured surface, and at least one opening for delivering the bone graft material to the disc space, the first textured surface configured to engage a first vertebrae and the second textured surface configured to engage a second vertebrae;

placing the interbody device within the disc space; and

advancing the bone graft material through the elongate tube with the plunger, wherein at least some of the bone graft material is delivered into the disc space through the at least one opening of the interbody device.

18. The method of claim 17 , further comprising disengaging the interbody device from the elongate tube.

19. The method of claim 17 , wherein the interbody device includes one or more of:

a metal or a polymer;

a radiographic marker;

a sawtooth-like ridge on one or more of the first textured surface and the second textured surface;

two lateral openings; and

a tapered distal tip.

20. The method of claim 17 , wherein the interbody device is configured to be coupled to the elongate tube before the interbody device has been placed within the disc space, and wherein a volume of the bone graft material delivered to the disc space by the bone graft delivery device is at least approximately two times greater than a volume of debrided disk material removed from the disc space.
