Abstract

There is a need for a minimally invasive surgical treatment method for periodontitis for the removal of deep pockets, elimination of disease, creation of reattachment of the gingiva to the tooth surface and true regeneration of the attachment apparatus (new cementum, new periodontal ligament, and new alveolar bone) on a previously diseased root surface. The PerioLase® MVP-7™ including eGUI or another device capable of laser dosimetry, such as an original MVP-7™ type laser without the eGUI, achieves this with the LANAP protocol (laser-assisted new attachment procedure) and the LENAP protocol (laser excisional new attachment procedure).

Claims (23)

1. A periodontal disease treatment method using a laser system to stimulate formation of new cementum, new periodontal ligament, and new alveolar bone for providing a new cementum mediated periodontal ligament attachment on a previously diseased root surface, the method comprising the steps of:

anesthetizing gingival tissue corresponding to a targeted tooth of a patient, the tooth having a tooth surface;

visualizing a pocket in the gingival tissue surrounding the tooth;

measuring a pocket depth from an upper gingival margin to a mucogingival junction;

providing a laser, a laser fiber, a laser controller and a user interface for communicating with the laser controller;

displaying on the user interface a plurality of ranges of values of a plurality of laser parameters including fiber diameter, pulse duration, pulse energy and pulse repetition rate, the ranges of values provided by the laser controller;

choosing a laser power <=6.0 watts;

lasing to ablate an interior epithelial lining of the pocket to the depth of the pocket on all sides of the tooth and to prepare a new pocket tissue surface;

cleaning the tooth surface to the depth of the pocket on all sides of the tooth;

lasing the new pocket tissue surface to remove granulomatous tissue while leaving the granulation tissue in place;

rinsing the pocket to remove debris;

eliminating occlusal interferences using a mechanical tool or high speed handpiece;

lasing the new pocket tissue surface to adapt the interior epithelial lining of the pocket for adhesion to the tooth surface; and,

with finger pressure maintaining the interior epithelial lining of the pocket in contact with the tooth surface to advance adhesion;

wherein at least one of the lasing steps is preceded by locking some of the plurality of laser parameter values in the plurality of ranges of values displayed on the user interface to prohibit their selection such that laser parameter values that are not locked are available for selection.

2. The method of claim 1 further including the steps of:

providing a laser power meter connected to the laser controller; and,

measuring a laser power from the laser fiber with the laser power meter;

wherein the measured laser power is compared with a chosen laser power to detect a power difference.

3. The method of claim 1 , wherein the lasing to ablate step uses a lasing frequency of not more than 20 hertz.

4. The method of claim 3 , wherein:

the laser fiber is of a diameter of between approximately 100 microns and 1000 microns.

5. The method of claim 1 , wherein after said step of rinsing the pocket to remove debris, and before said step of eliminating occlusal interferences, the method further comprises: irrigating the pocket with a bactericidal solution.
