Abstract

Devices, systems, and methods to improve both the reliability of soft tissue repair procedures and the speed at which the procedures are completed are provided. The devices and systems include one or more tissue augmentation constructs, which include constructs that are configured to increase a footprint across which suture applied force to tissue when the suture is tied down onto the tissue. The tissue augmentation constructs can be quickly and easily associated with the repair suture, and can be useful in many different tissue repair procedures that are disclosed in the application. In one exemplary embodiment, one or more constructs are disposed on a suture threader, which can be used to associate the construct(s) with a repair suture(s) being used to repair the soft tissue. Tissue augmentation constructs can include various blocks and patches, among other formations. Exemplary methods for manufacturing the tissue augmentation constructs are also provided.

Claims (45)

1. A surgical method, comprising:

inserting an anchor into bone, the anchor being coupled to a first collapsible loop of a surgical filament and a second collapsible loop of the surgical filament, the first and second collapsible loops being defined by first and second self-locking mechanisms of the surgical filament, the first collapsible loop extending from a first side of the first self-locking mechanism, a first tail of the surgical filament and a first portion of the surgical filament extending separately from a second side of the first self-locking mechanism, the second collapsible loop extending from a first side of the second self-locking mechanism, and a second tail of the surgical filament and a second portion of the surgical filament extending separately from a second side of the second self-locking mechanism;

disposing the first and second collapsible loops through soft tissue adjacent to the bone;

applying tension to at least one of the first tail or the second tail of the surgical filament to collapse at least one of the first or second collapsible loops, thereby drawing a tissue augmentation tack coupled to the first and second portions of the surgical filament towards the soft tissue and bone such that a distal face of the tissue augmentation tack applies a force to the soft tissue to draw the soft tissue towards the bone, the tissue augmentation tack having a diameter that is at least two times greater than a diameter of the suture filament, wherein the first and second tails of the surgical filament are disposed through the tissue augmentation tack and applying tension to either or both of the first tail or the second tail moves the respective first or second tail through the tissue augmentation tack, and wherein the first and second portions of the surgical filament pass into the tissue augmentation tack; and

locking a location of the tissue augmentation tack with respect to the bone to set the location of the soft tissue with respect to the bone,

wherein a position of the first portion of the surgical filament and the second portion of the surgical filament remains fixed with respect to the tissue augmentation tack throughout application of tension to at least one of the first tail or the second tail of the surgical filament.

2. The surgical method of claim 1 , further comprising:

positioning a distal end of a guide adjacent to the soft tissue,

wherein inserting an anchor coupled to first and second collapsible loops of a surgical filament into bone further comprises passing the anchor through the guide.

3. The surgical method of claim 2 , further comprising:

advancing a punch through the guide to form a hole in the soft tissue,

wherein disposing the first and second collapsible loops through soft tissue adjacent to the bone further comprises passing the first and second collapsible loops through the hole in the tissue.

4. The surgical method of claim 3 , further comprising:

advancing the punch through the guide to form a hole in the bone,

wherein inserting an anchor coupled to first and second collapsible loops of a surgical filament into bone further comprises inserting the anchor into the hole in the bone.

5. The surgical method of claim 2 , wherein the guide is removed before applying tension to the at least one of the first or second tail of the surgical filament to collapse the at least one of the first or second collapsible loops.

6. The surgical method of claim 1 , wherein applying tension to at least one of the first tail or the second tail of the surgical filament further comprises:

applying tension to the first tail and the second tail of the surgical filament to collapse the first collapsible loop and the second collapsible loop.

7. The surgical method of claim 1 , further comprising toggling the anchor to set a location of the anchor with respect to the bone.

8. The surgical method of claim 1 , wherein the anchor is a soft anchor formed of a flexible construct, the method further comprising:

actuating the anchor from an unstressed configuration to a stressed configuration to set a location of the anchor with respect to the bone.

9. The method of claim 1 , wherein the diameter of the tissue augmentation tack is at least three times greater than a diameter of the surgical filament.

The surgical method of claim 1 , wherein the tissue augmentation tack comprises at least one of: fabric, plastic, synthetic polymer, natural polymer, collagen, collagen scaffold, reconstituted collagen, biological autograft connective tissue, biological allograft connective tissue, biological xenograft connective tissue, human dermal matrix, porcine dermal matrix, bovine dermal matrix, periosteal tissue, pericardial tissue, and fascia.

11. The surgical method of claim 10 , wherein the tack comprises collagen.

12. The surgical method of claim 1 , wherein at least one of the first and second self-locking mechanisms is disposed between the tissue augmentation tack and the first and second collapsible loops.

13. The surgical method of claim 1 , wherein at least one of the first and second self-locking mechanisms comprises a self-locking sliding knot.

14. The surgical method of claim 1 , wherein inserting an anchor into bone comprises first passing the anchor through the soft tissue.

15. A surgical method, comprising:

inserting an anchor into bone, the anchor being coupled to a first collapsible loop of a surgical filament and a second collapsible loop of the surgical filament, the first and second collapsible loops being defined by first and second self-locking mechanisms of the surgical filament, the first and second self-locking mechanisms being disposed separate from a tissue augmentation tack such that the first and second self-locking mechanisms are disposed between a distal face of the tissue augmentation tack and the collapsible loop, the tissue augmentation tack being coupled to the surgical filament;

disposing the first and second collapsible loops through soft tissue adjacent to the bone;

applying tension to at least one of a first tail of the surgical filament extending from the first self-locking mechanism or a second tail extending from the second self-locking mechanism to collapse at least one of the first or second collapsible loops, thereby drawing the tissue augmentation tack towards the soft tissue and bone such that the distal face of the tissue augmentation tack applies a force to the soft tissue to draw the soft tissue towards the bone, the tissue augmentation tack having a diameter that is at least two times greater than a diameter of the suture filament, wherein the first and second tails of the surgical filament are disposed through the tissue augmentation tack and applying tension to either or both of the first or second tails moves the respective tail through the tissue augmentation tack, and wherein a first portion and a second portion of the surgical filament extending respectively from the first and second self-locking mechanisms—are coupled to the tissue augmentation tack; and

locking a location of the tissue augmentation tack with respect to the bone to set the location of the soft tissue with respect to the bone,

wherein a position of the first portion of the surgical filament and the second portion of the surgical filament remains fixed with respect to the tissue augmentation tack throughout application of tension to at least one of the first tail or the second tail of the surgical filament.

16. The surgical method of claim 15 , further comprising:

positioning a distal end of a guide adjacent to the soft tissue,

wherein inserting an anchor coupled to first and second collapsible loops of a surgical filament into bone further comprises passing the anchor through the guide.

17. The surgical method of claim 15 , wherein applying tension to at least one of a first tail of the surgical filament extending from the first self-locking mechanism or a second tail extending from the second self-locking mechanism further comprises:

applying tension to the first tail and the second tail of the surgical filament to collapse the first collapsible loop and the second collapsible loop.

18. The surgical method of claim 15 , wherein the anchor is a soft anchor formed of a flexible construct, the method further comprising:

actuating the anchor from an unstressed configuration to a stressed configuration to set a location of the anchor with respect to the bone.

19. The method of claim 15 , wherein the diameter of the tissue augmentation tack is at least three times greater than a diameter of the surgical filament.

20. The surgical method of claim 15 , wherein the tissue augmentation tack comprises at least one of: fabric, plastic, synthetic polymer, natural polymer, collagen, collagen scaffold, reconstituted collagen, biological autograft connective tissue, biological allograft connective tissue, biological xenograft connective tissue, human dermal matrix, porcine dermal matrix, bovine dermal matrix, periosteal tissue, pericardial tissue, and fascia.

21. The surgical method of claim 20 , wherein the tack comprises collagen.

22. The surgical method of claim 15 , wherein at least one of the first or second self-locking mechanisms comprises a self-locking sliding knot.

23. The surgical method of claim 15 , wherein inserting an anchor into bone comprises first passing the anchor through the soft tissue.
